Convolutional neural networks for seizure prediction using intracranial and scalp electroencephalogram

Epilepsy is defined by the seemingly random occurrence of spontaneous seizures. The ability to anticipate seizures would enable preventative treatment strategies. A central but unresolved question concerns the relationship of seizure timing to fluctuating rates of interictal epileptiform discharges (here termed interictal epileptiform activity, IEA), a marker of brain irritability observed between seizures by electroencephalography (EEG). Here, in 37 subjects with an implanted brain stimulation device that detects IEA and seizures over years, we find that IEA oscillates with circadian and subject-specific multidien (multi-day) periods. Multidien periodicities, most commonly 20-30 days in duration, are robust and relatively stable for up to 10 years in men and women. We show that seizures occur preferentially during the rising phase of multidien IEA rhythms. Combining phase information from circadian and multidien IEA rhythms provides a novel biomarker for determining relative seizure risk with a large effect size in most subjects.

Multi-day rhythms modulate seizure risk in epilepsy

Epilepsy is a common disorder characterized by recurrent seizures. An overwhelming majority of people with epilepsy regard the unpredictability of seizures as a major issue. More than 30 years of international effort have been devoted to the prediction of seizures, aiming to remove the burden of unpredictability and to couple novel, time-specific treatment to seizure prediction technology. A highly influential review published in 2007 concluded that insufficient evidence indicated that seizures could be predicted. Since then, several advances have been made, including successful prospective seizure prediction using intracranial EEG in a small number of people in a trial of a real-time seizure prediction device. In this Review, we examine advances in the field, including EEG databases, seizure prediction competitions, the prospective trial mentioned and advances in our understanding of the mechanisms of seizures. We argue that these advances, together with statistical evaluations, set the stage for a resurgence in efforts towards the development of seizure prediction methodologies. We propose new avenues of investigation involving a synergy between mechanisms, models, data, devices and algorithms and refine the existing guidelines for the development of seizure prediction technology to instigate development of a solution that removes the burden of the unpredictability of seizures.

Epileptic Seizure Prediction Using Big Data and Deep Learning: Toward a Mobile System.

Objective: This paper investigates the hypothesis that focal seizures can be predicted using scalp electroencephalogram (EEG) data. Our first aim is to learn features that distinguish between the interictal and preictal regions. The second aim is to define a prediction horizon in which the prediction is as accurate and as early as possible, clearly two competing objectives. Methods: Convolutional filters on the wavelet transformation of the EEG signal are used to define and learn quantitative signatures for each period: interictal, preictal, and ictal. The optimal seizure prediction horizon is also learned from the data as opposed to making an a priori assumption. Results: Computational solutions to the optimization problem indicate a 10-min seizure prediction horizon. This result is verified by measuring Kullback–Leibler divergence on the distributions of the automatically extracted features. Conclusion: The results on the EEG database of 204 recordings demonstrate that (i) the preictal phase transition occurs approximately ten minutes before seizure onset, and (ii) the prediction results on the test set are promising, with a sensitivity of 87.8% and a low false prediction rate of 0.142 FP/h. Our results significantly outperform a random predictor and other seizure prediction algorithms. Significance: We demonstrate that a robust set of features can be learned from scalp EEG that characterize the preictal state of focal seizures.

Focal Onset Seizure Prediction Using Convolutional Networks

Accurate forecasting of epileptic seizures has the potential to transform clinical epilepsy care. However, progress toward reliable seizure forecasting has been hampered by lack of open access to long duration recordings with an adequate number of seizures for investigators to rigorously compare algorithms and results. A seizure forecasting competition was conducted on kaggle.com using open access chronic ambulatory intracranial electroencephalography from five canines with naturally occurring epilepsy and two humans undergoing prolonged wide bandwidth intracranial electroencephalographic monitoring. Data were provided to participants as 10-min interictal and preictal clips, with approximately half of the 60 GB data bundle labelled (interictal/preictal) for algorithm training and half unlabelled for evaluation. The contestants developed custom algorithms and uploaded their classifications (interictal/preictal) for the unknown testing data, and a randomly selected 40% of data segments were scored and results broadcasted on a public leader board. The contest ran from August to November 2014, and 654 participants submitted 17 856 classifications of the unlabelled test data. The top performing entry scored 0.84 area under the classification curve. Following the contest, additional held-out unlabelled data clips were provided to the top 10 participants and they submitted classifications for the new unseen data. The resulting area under the classification curves were well above chance forecasting, but did show a mean 6.54 ± 2.45% (min, max: 0.30, 20.2) decline in performance. The kaggle.com model using open access data and algorithms generated reproducible research that advanced seizure forecasting. The overall performance from multiple contestants on unseen data was better than a random predictor, and demonstrates the feasibility of seizure forecasting in canine and human epilepsy.
