The effects of insulin and insulin-like growth factors (IGFs) on ovarian androgen production were examined in ovarian stroma obtained from four women with hyperandrogenism and three women without hyperandrogenism. In incubations of stroma obtained from all four hyperandrogenic patients, insulin alone (500 ng/ml) significantly stimulated androstenedione and testosterone release. LH alone (25 ng/ml) significantly stimulated androstenedione release in incubations of stroma obtained from three of the four hyperandrogenic patients and testosterone release in incubations of stroma obtained from one of the four hyperandrogenic patients. In stromal incubations from three of the four hyperandrogenic patients, insulin alone (500 ng/ml) resulted in a significantly greater release of androstenedione and testosterone than did LH alone (25 ng/ml). Dihydrotestosterone was released in measurable quantities in incubations of stromal tissue obtained from three of the four hyperandrogenic women. In all three instances in which dihydrotestosterone was detectable, insulin alone (500 ng/ml), but not LH alone (25 ng/ml), significantly stimulated dihydrostestosterone release. Incubations of stroma obtained from three nonhyperandrogenic, normally cycling women demonstrated low levels of androstenedione release and negligible testosterone and dihydrotestosterone release. Insulin alone (500 ng/ml) and LH alone (25 ng/ml) produced no significant increase in androstenedione release. Insulin (500 ng/ml) plus LH (25 ng/ml) significantly stimulated androstenedione accumulation in stroma obtained from two of the nonhyperandrogenic women. One insulin dose-response experiment was performed using stromal tissue obtained from a hyperandrogenic woman. In this experiment, insulin, at a dose of 50 ng/ml, was as effective as insulin at a dose of 500 ng/ml in stimulating androstenedione and testosterone release. In addition to insulin, IGF-I/somatomedin C (50 ng/ml) stimulated androstenedione and testosterone release. Relaxin (1 microgram/ml) and multiplication-stimulating activity (50 ng/ml) did not stimulate androstenedione and testosterone release. These studies suggest that human ovarian stroma may be a target tissue for insulin and IGF-I, and that hyperinsulinemia may be an important factor contributing to ovarian hyperandrogenism.

Insulin Stimulates Androgen Accumulation in Incubations of Ovarian Stroma Obtained from Women with Hyperandrogenism

The ovarian insulin and insulin-like growth factor system with an emphasis on domestic animals

The objectives of the present studies were to determine the effect of insulin, insulin-like growth factor I (IGF-I), testosterone, and FSH on proliferation, progesterone production, and(or) estradiol production of bovine granulosa cells. In addition, existence of IGF-I mRNA in granulosa cells and in vitro IGF-I production by granulosa cells were assessed. Cells from small (1 to 5 mm) and large (> or = 8 mm) follicles were collected from cattle and cultured for either 3 or 4 d. When cells from small follicles were cultured, insulin (.1 to 10 micrograms/mL) and IGF-I (100 to 400 ng/mL) increased (P or = 5 micrograms/mL) had no effect on estradiol production by cells from small and large follicles. Granulosa cells contained four IGF-I mRNA transcripts and produced IGF-I in vitro. These results support the hypothesis that insulin and IGF-I may have direct local effects on bovine ovarian function, and that these effects are influenced by dose and size of follicle.

The role of hyperinsulinemia in the pathogenesis of ovarian hyperandrogenism

Substantial advances in our understanding of placental function have resulted from recent establishment of in vitro approaches, such as cell culture, and application of molecular methods to study placental steroidogenesis. Insight into the processes of placental cell differentiation and hormonal function has been gained from culture of relatively pure preparations of cytotrophoblast. Various factors, e.g. cAMP and peptide growth factors, have been shown to have striking effects on progesterone and estrogen formation by placental tissue under in vitro conditions. Using advanced molecular approaches, the genes governing specific enzymes critical to placental steroidogenesis have been identified. Regulation of the mRNAs encoding specific enzyme peptides and thus expression of the genes by factors, such as cAMP, have been elucidated by Northern analysis and other techniques. It is critical that these contemporary approaches continue to be implemented aggressively to further elucidate placental function. However, it is clear from a survey of the literature, particularly of the past decade, that the vast majority of investigation in the area has been conducted in vitro. It is essential to determine whether the factors that have been observed to regulate placental endocrine function in vitro are operable in vivo. It is only with in vivo study that the dynamics of steroidogenesis and the complex functional relationships between placenta, fetus, and mother will be uncovered and understood. It is increasingly evident that the regulation of placental steroidogenesis involves autocrine and/or paracrine mechanisms, similar to those integral to hormone biosynthesis within other reproductive organs, e.g. ovary and testis. For example, as discussed above, estrogen regulates LDL uptake and P-450scc, and thus apparently is involved in generating substrate for progesterone production within the placenta. Conversely, progesterone has effects on 17 beta-hydroxysteroid oxidoreductase and thus the metabolism of estradiol, while androgens exert marked inhibitory effects on placental progesterone formation, at least in vitro. Not surprisingly, the regulation of placental progesterone and estrogen formation also is multifactorial. Thus, aromatase activity is stimulated synergistically by cAMP and phorbol esters, an effect that is suppressed by peptide growth factors. Therefore, the autocrine/paracrine and multifactorial regulation of hormone biosynthesis that has been relatively well documented in other tissues should be recognized as important in the primate placenta. Finally, the basic mechanisms underlying regulation of steroidogenesis within the fetoplacental unit during primate pregnancy appear similar, in important ways, to those of widely used laboratory animals, such as the rat and rabbit.(ABSTRACT TRUNCATED AT 400 WORDS)

Placental steroid hormone biosynthesis in primate pregnancy.

We have investigated the responsiveness of swine granulosa cells to somatomedin C/insulin-like growth factor I (IGF I) under serum-free conditions in vitro. Somatomedin C/IGFI (>80% pure) exerted dose-dependent stimulatory effects on the production of progesterone and its reduced metabolite, 20α-hydroxypregn-4-en-3-one. A 50-fold stimulation of progesterone production could be observed after 48-96 h of hormone treatment. These effects were not mimicked by platelet-xderived growth factor, epidermal growth factor, fibroblast growth factor, desoctapeptide insulin, or porcine relaxin. The stimulatory action of somatomedin C/IGF I was not attributable to changes in cell protein or DNA content or cell number and reflected enhanced steroidogenesis per se rather than simply release of stored progesterone. In addition, augmented progesterone production was accompanied by increased biosynthesis of its precursor, pregnenolone, measured in the presence of trilostane to block 3/3-hydroxysteroid dehydrogenase activity....
