Summary Background Since December, 2019, Wuhan, China, has experienced an outbreak of coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Epidemiological and clinical characteristics of patients with COVID-19 have been reported but risk factors for mortality and a detailed clinical course of illness, including viral shedding, have not been well described. Methods In this retrospective, multicentre cohort study, we included all adult inpatients (≥18 years old) with laboratory-confirmed COVID-19 from Jinyintan Hospital and Wuhan Pulmonary Hospital (Wuhan, China) who had been discharged or had died by Jan 31, 2020. Demographic, clinical, treatment, and laboratory data, including serial samples for viral RNA detection, were extracted from electronic medical records and compared between survivors and non-survivors. We used univariable and multivariable logistic regression methods to explore the risk factors associated with in-hospital death. Findings 191 patients (135 from Jinyintan Hospital and 56 from Wuhan Pulmonary Hospital) were included in this study, of whom 137 were discharged and 54 died in hospital. 91 (48%) patients had a comorbidity, with hypertension being the most common (58 [30%] patients), followed by diabetes (36 [19%] patients) and coronary heart disease (15 [8%] patients). Multivariable regression showed increasing odds of in-hospital death associated with older age (odds ratio 1·10, 95% CI 1·03–1·17, per year increase; p=0·0043), higher Sequential Organ Failure Assessment (SOFA) score (5·65, 2·61–12·23; p Interpretation The potential risk factors of older age, high SOFA score, and d-dimer greater than 1 μg/mL could help clinicians to identify patients with poor prognosis at an early stage. Prolonged viral shedding provides the rationale for a strategy of isolation of infected patients and optimal antiviral interventions in the future. Background: At present, there are no comprehensive validated instruments for measuring adolescents’ beliefs regarding tobacco smoking in the Iranian society. This study aimed to evaluate the validity, reliability and feasibility of the belief-based tobacco smoking scale using the Theory of Planned Behavior’s (TPB) constructs as a theoretical framework.Methods: This cross-sectional validation study was carried out on 410 male adolescents of Hamadan, west of Iran, recruited through multi-stage random sampling method. Reliability was assessed by internal consistency and Intraclass Correlation Coefficient (ICC). In addition, Confirmatory Factor Analyses (CFA) and Exploratory Factor Analyses (EFA) were performed to test construct valid-ity. Content validity was examined using Content Validity Index (CVI) and Con-tent Validity Ratio (CVR).Results: Results obtained from factor analysis showed that the data was fit to the model (X2=391.43, P<0.001) and TPB consisted of 22 items measuring sev-en components which explaining 69.7% of the common variance. The mean scores for the CVI and CVR were 0.89 and 0.80; respectively. Additional anal-yses indicated acceptable results for internal consistency reliability values ranging from 0.55 to 0.92.Conclusion: The belief-based tobacco smoking questionnaire is a reliable and valid instrument and now is acceptable and suitable and can be used in future studies.

Belief-based Tobacco Smoking Scale: Evaluating the PsychometricProperties of the Theory of Planned Behavior’s Constructs

Background: Tramadol is an opioid analgesic indicated for the treatment of moderate and severe pains. In the case of long administration, tramadol has a potential to cause dependency, tolerance and drug abuse. Objectives: This study aimed to investigate the prevalence and factors associated with tramadol abuse among college students based on the Theory of Planned Behavior (TPB) as a theoretical framework. Patients and Methods: This descriptive-analytical study was performed on 400 college students of Hamadan universities recruited with a stratified sampling method. The data-gathering tool was a questionnaire based on the TPB constructs. Data was analyzed by logistic regression and bivariate correlations using SPSS-19 software. Results: The subjects reported 50%, 35% and 77% of maximum receivable scores of attitude, subjective norm and perceived behavior control, respectively. In total, 12.5% of participants indicated drug abuse in the past. Subjective norms and perceived behavioral control were better predictors for tramadol abuse. Conclusions: Results demonstrated the unique importance of subjective norms and perceived behavioral control when examining substance abuse among college students.

Effect of a Web-Based Educational Program on Prevention of Tobacco Smoking among Male Adolescents: An Application of Prototype Willingness Model

Background: Depression is a serious mental health illness among adolescents especially girls. Web-based treatment can possibly become a solution for reducing mental health problems in adolescents. This study is one of the first trials aimed to evaluate the efficiency of web-based depression improvement program among female adolescents based on the Social Cognitive Theory (SCT). Study design: Randomized Controlled Trial. Methods: A six-month randomized controlled trial based on the SCT was implemented in female schools in Hamadan City, west of Iran in 2018 on 128 female students with mild to moderate depressive symptoms (CES-D =10-45). They were randomly assigned to either intervention or control group (n= 64 in each group). Depression improvement program was implemented through the website via short videos, animations and Power-Point slides. Depression was evaluated using Center for Epidemiologic Depression Scale. A researcher-made questionnaire based on the sheerer and Perceived-Social-Support-Scale-Revised (PSSS-R) questionnaires were used to evaluate the SCT constructs. The data were analyzed using SPSS software. Results: The intervention program resulted in the improvement of depression in 12 wk based on Intent-to-treat analyses. Nevertheless, these achievements seem to have decreased by 24 wk in intervention group. The intervention increased the mean scores of the constructs of social support and self-regulation from baseline to 12 wk in the intervention group ( P 0.05). Conclusion: The web-based intervention improved depression in female students. Future training using strategies for the sustainable improvement of depression in female students are needed.
