Comments on "Short-Term Outcome of Spontaneous Intracerebral Hemorrhage in Algarve, Portugal: Retrospective Hospital-Based Study".

EDITORS, We read enthusiastically the paper by Yanai et al. that was published in Alimentary Pharmacology & Therapeutics. The authors proposed to determine predictors of pouch outcome in a cohort of patients with ulcerative colitis. As one of the main results, in the multivariable model, they found that refractory indication for surgery was associated with 3.43-fold risk for pouchitis. However, some methodological issues need to be taken into account. First, the authors conducted a multivariable Cox proportional hazard regression model, where it has not been clarified which strategy of model building was followed. The standard model building strategy is conducted in two steps: (1) the multicollinearity between the independent variables is initially checked, (2) then, variables with no multicollinearity with other independent variables and P<.2 are considered to be imported into the multivariable model. Yanai and colleagues imported gender, age at ileal pouch-anal anastomosis surgery and Family history of inflammatory bowel disease along with Refractory indication for ileal pouch-anal anastomosis in their multivariable Cox proportional hazard regression model; where based on the results provided in their Table 2, aforementioned variables are highly correlated with Refractory indication for ileal pouch-anal anastomosis. Hence, their results of multivariable model may be biased because of multicollinearity among the included independent variables. Second, the authors used Cox proportional hazard regression model in their study where its main assumption has not been checked. The proportional hazard assumption should be checked earlier; then, Cox PH regression model is used if this assumption is established. Otherwise, their results would be misleading. Scientists have suggested variant of Cox model such as Extended Cox model to give valid results when proportional hazard assumption is violated. Finally, Yanai and colleagues have not validated their prediction model—each prediction model needs to be validated internally or externally through appropriate techniques. Otherwise, its results are optimistic and should be interpreted with caution.

Efficacy and safety of endoscopic submucosal tunnel dissection for superficial esophageal squamous cell carcinoma: a propensity score matching analysis: methodologic issues

We have, meticulously, read the article authored by Clemente and colleagues published in the Substance Use and Misuse (Clemente et al., 2017). The authors aimed to examine erectile dysfunction (ED) symptom prevalence, sexual behavior conditions, and risk factors associated to ED in a male polydrug dependent sample. They reported that ED risk decreased when marital status was married [odds ratio (OR) = 3.2; 95% confidence interval (CI): 1.411–7.518] and with chronic disease [OR = 0.06; 95% CI: 0.00–0.97], whereas ED risk was 14 times higher in occasional sexual partnership groups compared to stable group [OR = 14.07; 95% CI: 1.62–122.18]. Although the valuable study has been conducted, some methodological points need to be taken into account. First, the authors built a multivariable logistic regression model but it was not clarified how this model was constructed. The scientists usually use Forward or Backward step-wise logistic regression model such that the multicollinearity between the independent variables are examined in the first step; then, those variables without multicollinearity and P-value < 0.2 are included into the final multivariable model (Jewell, 2003; W., Lemeshow, & Sturdivant, 2013). Clemente and colleagues have constructed a multivariable model on 102 substancedependent male polydrug users where the multicollinearity have not been examined and their results may be biased because of multicollinearity (Clemente et al., 2017). Also, authors have included 9 variables (more than 20 strata) into the multivariable model where only 102 patients have studied and over-parameterization is expected in their final model (Clemente et al., 2017).
