The Coding Manual for Qualitative Researchers

OBJECTIVE — To systematically review the incidence of posttransplantation diabetes (PTD), risk factors for its development, prognostic implications, and optimal management. RESEARCH DESIGN AND METHODS — We searched databases (MEDLINE, EMBASE, the Cochrane Library, and others) from inception to September 2000, reviewed bibliographies in reports retrieved, contacted transplantation experts, and reviewed specialty journals. Two reviewers independently determined report inclusion (original studies, in all languages, of PTD in adults with no history of diabetes before transplantation), assessed study methods, and extracted data using a standardized form. Meta-regression was used to explain between-study differences in incidence. RESULTS — Nineteen studies with 3,611 patients were included. The 12-month cumulative incidence of PTD is lower (10% in most studies) than it was 3 decades ago. The type of immunosuppression explained 74% of the variability in incidence (P 0.0004). Risk factors were patient age, nonwhite ethnicity, glucocorticoid treatment for rejection, and immunosuppression with high-dose cyclosporine and tacrolimus. PTD was associated with decreased graft and patient survival in earlier studies; later studies showed improved outcomes. Randomized trials of treatment regimens have not been conducted. CONCLUSIONS — Physicians should consider modification of immunosuppressive regimens to decrease the risk of PTD in high-risk transplant recipients. Randomized trials are needed to evaluate the use of oral glucose-lowering agents in transplant recipients, paying particular attention to interactions with immunosuppressive drugs. The past decade has witnessed increased public awareness and use of alternative therapies for the management of diseases and symptoms (Schwartz, Chesney, Irvine, & Keefe, 1997). Spiro (1997) relates the increased use of alternative therapies to the need for personal connection, belonging, and comfort. A variety of biochemical, psychophysiological, and psychological therapies are available, including homeopathic preparations such as sulphur for dermatoses (Linde et al, 1997), acupuncture, reflexology, massage therapy, therapeutic touch, and self-help approaches. Margo (1999) reports that alternative therapies are used by 20% to 50% of persons in industrialized countries, with some $14 billion being spent on such therapies in the United States. A large number of publications have been devoted to alternative therapies (e.g., Journal of the American Medical Association, 280, 1998); medical and nursing schools are offering courses in alternative therapies; and funding has been made available to systematically investigate the effects of alternative therapies (e.g., National Institutes of Health) (Kwekkeboom, 1997).
Despite the interest in alternative therapies and the accumulating empirical evidence supporting their effectiveness, some scholars view these therapies with scepticism. Many consider them as placebos (Linde et al., 1997; Shapiro & Shapiro, 1997). Preferences for treatment contribute to attrition. Providing participants with their preferred treatment, as done in a partially randomized clinical or preference trial (PRCT), is a means to mitigate the influence of treatment preferences on attrition. This study examined attrition in an RCT and a PRCT. Persons with insomnia were randomly assigned (n = 150) or allocated (n = 198) to the preferred treatment. The number of dropouts at different time points in the study arms was documented and the influence of participant characteristics and treatment-related factors on attrition was examined. The overall attrition rate was higher in the RCT arm (46%) than in the PRCT arm (33%). In both arms, differences in sociodemographic and clinical characteristics were found between dropouts and completers. The type of treatment significantly predicted attrition (all p ≤ .05). The results provide some evidence of a lower attrition rate in the PRCT arm, supporting the benefit of accounting for preferences as a method of treatment allocation.

Discourse / Discours - Attrition in Randomized and Preference Trials of Behavioural Treatments for Insomnia

Attending to treatment preferences is an element of person-centred care, reported as beneficial in improving treatment adherence, satisfaction, and outcome, in practice. The results of preference trials were inconsistent in supporting these benefits in intervention evaluation research. Informed by the conceptualisation of treatment preferences positing their indirect impact on outcomes, this narrative review aimed to summarise the evidence on the effects of preferences on enrolment; withdrawal or attrition; engagement, enactment, and satisfaction with treatment; and outcomes. The search yielded 72 studies (57 primary trials and 15 reviews). The results of vote counting indicated that (1) offering participants the opportunity to choose treatment enhances enrolment (reported in 87.5% of studies), and (2) providing treatments that match participants' preferences reduces attrition (48%); enhances engagement (67%), enactment (50%) and satisfaction with (43%) treatment; and improves outcomes (35%). The results are attributed to conceptual and methodological issues including less-than-optimal assessment of treatment preferences, which contributes to ill-identified preferences, accounting for withdrawal, low enactment, and limited satisfaction with treatment. These treatment processes, in turn, mediate the impact of treatment preferences on outcomes. It is important to refine and standardise the methods for assessing preferences and to examine their indirect impact (mediated by treatment processes) on outcomes in future preference trials to validly identify their benefits.

The impact of treatment preferences: A narrative review.

Background: Insomnia increases the risk for cardiovascular diseases. Stimulus control therapy (SCT) and sleep restriction therapy (SRT) are behavioural interventions which are effective for managing insomnia, but their effectiveness in alleviating symptoms precursor to or indicative of cardiovascular diseases is unknown. Aims: To evaluate the effects of SCT and SRT, on insomnia and cardiovascular symptoms, in persons with chronic insomnia. Methods: A randomised controlled trial was used. Participants (n=55) randomised to either SCT, SRT or no treatment group completed the study. Outcome data were collected before and after a 4-week treatment period. Results: Participants in the SRT group reported a reduction in perceived insomnia, and those in the SRT and SCT groups showed a reduction in cardiovascular symptoms.
