Isolated pancreatic laceration is a rare injury. The typical mechanism by which it occurs is overstretching of the pancreas across the vertebral column during blunt abdominal trauma. The management depends on the location and extent of the injury. Disruption of the pancreatic duct usually requires operative treatment. Operative options for pancreatic laceration at the neck include distal pancreatectomy or suturing of the cephalic remnant and Roux‑en‑Y pancreatico‑jejunostomy on the left remnant. We are reporting two cases of isolated pancreatic injury with disruption of the pancreatic duct but preserved posterior surface of the pancreas. These patients were managed by performing anterior Roux‑en‑Y pancreatico‑jejunostomy at the lacerated area. Both the patients had successful outcome with removal of drains by 6th postoperative day. Anterior Roux‑en‑Y pancreatico‑jejunostomy in this particular scenario is easy, less time consuming, and has the advantage of preserving the pancreas and the spleen. Anterior roux-en-Y Pancreatico-jejunostomy for pancreatic trauma

A rare case of malignant Brenner tumour of ovary manifesting with intestinal perforation due to colonic infiltration is elaborated in the present report. Brenner’s tumour accounts for 1%–2% of all ovarian neoplasms and malignant Brenner tumour is even rarer and only about 5% of Brenner tumours are malignant. A 62-year-old woman came to surgical emergency with 1-month history of abdominal pain, vomiting and constipation with a palpable mass in right iliac fossa. Abdominal radiograph was suggestive of colonic obstruction. Contrast-enhanced CT of the abdomen revealed cystic right ovarian mass of 10.2×8.8 cm2 with pneumoperitoneum. Exploratory laparotomy was done, which revealed mass arising from right ovary involving terminal ileum, cecum and ascending colon. Possibility of ovarian malignancy was kept. Patient underwent debulking surgery along with ileostomy and descending colon mucous fistula was created. Histology was compatible with malignant Brenner tumour of the ovary.

Single crystal of Tm-doped LaCl3 (LaCl3:Tm3+) with the dimension of diameter 7 mm and length 55 mm was successfully grown using an in-house-developed Bridgman furnace. The grown crystal has the self-trapped exciton (STE) emission in the range of 300–500 nm along with the characteristic emissions corresponding to the 4f-4f transition of Tm3+ ions under X-ray excitation. The scintillation characterizations of the grown crystal were performed by pulse height spectra, scintillation decay time, as well as pulse shape discrimination studies under γ-ray and α-particle excitation sources. The LaCl3:Tm3+ crystal was found to have a high light yield (42,000 ± 4,200 ph/MeV) with 4.2% energy resolution under γ-rays excitation. Moreover, the pulse shape discrimination was studied using 137Cs and 241Am as the radiation sources with a good FOM value. The results conclude the LaCl3:Tm3+ single crystal to be a promising candidate for X-ray and γ-ray detection purposes.

Traditional Indian (Ayurvedic) and Chinese herbal medicines have been used in the treatment of a variety of diseases for thousands of years because of their natural origin and lesser side effects. However, the safety and efficacy data (including dose and quality parameters) on most of these traditional medicines are far from sufficient to meet the criteria needed to support their world-wide therapeutic use. Also, the mechanistic understanding of most of these herbal medicines is still lacking due to their complex components which further limits their wider application and acceptance. Metabolomics -a novel approach to reveal altered metabolism (biochemical effects) produced in response to a disease or its therapeutic intervention- has huge potential to assess the pharmacology and toxicology of traditional herbal medicines (THMs). Therefore, it is gradually becoming a mutually complementary technique to genomics, transcriptomics and proteomics for therapeutic evaluation of pharmaceutical products (including THMs); the approach is so called pharmaco-metabolomics. The whole paradigm is based on its ability to provide metabolic signatures to confirm the diseased condition and then to use the concentration profiles of these biomarkers to assess the therapeutic response. Nuclear magnetic resonance (NMR) spectroscopy coupled with multivariate data analysis is currently the method of choice for pharmaco-metabolomics studies owing to its unbiased, non-destructive nature and minimal sample preparation requirement. In recent past, dozens of NMR based pharmaco-metabolomic studies have been devoted to prove the therapeutic efficacy/safety and to explore the underlying mechanisms of THMs, with promising results. The current perspective article summarizes various such studies in addition to describing the technical and conceptual aspects involved in NMR based pharmaco-metabolomics.

NMR based Pharmaco-metabolomics: An efficient and agile tool for therapeutic evaluation of Traditional Herbal Medicines

Gastrointestinal stromal tumors are rare malignancies of the gastrointestinal tract. We report a case of a 65-year-old female, who presented with abdominal pain and a mobile lump in left upper abdomen. On exploration, a pedunculated mass of 11 x 10cm was found to be arising from the greater curvature of the stomach. The mass was excised with a wedge of stomach. Pathological evaluation diagnosed it as gastrointestinal stromal tumor. This type of presentation of gastrointestinal stromal tumor of stomach as mobile intra-abdominal lump is very rare and poses diagnostic problems.
