Background

Evaluation of the association between treatment satisfaction and health-related quality of life (HRQoL) may enable health-care providers to understand the issues that influence quality of life and to recognize the aspects of hypertension treatment that need improvement to enhance the long-term treatment outcomes.



Objective

The aim of this study was to determine the relationship between HRQoL and treatment satisfaction in a sample of Palestinian hypertensive patients.



Methods

A cross-sectional study was conducted, adopting the Treatment Satisfaction Questionnaire for Medication (TSQM 1.4) for the assessment of treatment satisfaction and using the European Quality of Life scale (EQ-5D-5L) for the assessment of HRQoL. Descriptive and comparative statistics were used to describe socio-demographic and disease-related characteristics of the patients. All analyses were performed using SPSS v 15.0.



Results

Four hundred and ten hypertensive patients were enrolled in the study. This study findings indicate a positive correlation between all satisfaction domains and HRQoL. Significant differences were observed between this study variables (P < 0.001). After adjustment for covariates using multiple linear regression, an increase of one point in the global satisfaction scale was associated with a 0.16 increase in EQ-5D index scores (r = 0.16; P < 0.001).



Conclusions

Patients with reportedly higher satisfaction scores have reported relatively higher EQ-5D-5L index values. These study findings could be helpful in clinical practice, mainly in the early treatment of hypertensive patients, at a point where improving treatment satisfaction and HRQoL is still possible.

Relationship of treatment satisfaction to health-related quality of life: findings from a cross-sectional survey among hypertensive patients in Palestine

Purpose Indigenous conceptions of health have been shown to differ from that of their non-Indigenous counterparts. As a result, there remains uncertainty over the appropriateness and value of using existing health-related quality-of-life (HRQoL) instruments in Indigenous communities. The objective of this review was to identify studies that either measure the HRQoL of an Indigenous population or validated a measure used to elicit the HRQoL in an Indigenous population. Methods A systematic review of the published literature was conducted to (1) investigate the extent to which HRQoL instruments are used in Indigenous populations; (2) to identify which instruments have been validated in which populations; and (3) to identify which instruments have been tailored for use with Indigenous populations. Results Forty-one studies were included in the review. Only three of the 41 studies utilised Indigenous-specific instruments. The remainder (38 studies) utilised generic population or disease-specific instruments. Four studies found specific HRQoL instruments to be valid in these populations and 32 estimated the HRQoL of an Indigenous population. The limited examples of Indigenous-specific instruments highlighted the potential importance to the HRQoL of these populations of domains that lie outside of traditional measures including social and community domains as well as domains relating to culture, diet and land use on top of more traditional HRQoL domains. Conclusion Ensuring that the HRQoL of Indigenous populations is being appropriately measured is vital to prioritising available resources to the most effective interventions. HRQoL instruments present an opportunity to directly elicit and incorporate Indigenous preferences and conceptions of health into these decisions. Further work is required in the field to ensure that this potential is realised.

The health-related quality of life of Indigenous populations: a global systematic review.

Osteoarthritis knee hip quality of life questionnaire assessment in Egyptian primary knee osteoarthritis patients: Relation to clinical and radiographic parameters

Knee complaints and their accompanying functional impairments are frequent problems encountered by healthcare practitioners worldwide. Plenty of functional scoring systems were developed and validated to give a relative estimation about the knee function. Despite the wide geographic distribution of Arabic language in the Middle East and North Africa, it is rare to find a validated knee function scale in Arabic. The present study is aimed to translate, validate, and culturally adjust the Knee Outcome Survey: Activities of Daily Living Scale (KOS-ADLS) into Arabic language for future use among Arabic-speaking patients. Permission for translation was obtained from the copyrights holder. Two different teams of high-level clinical and linguistic expertise conducted translation process blindly. Forward–backward translation technique was implemented to ensure preservation of the main conceptual content. Main study consisted of 280 subjects. Reliability was examined by test–retest pilot study. Visual Analogue Scale (VAS), Get Up and Go (GUG) Test, Ascending/Descending Stairs (A/D Stairs), and Subjective Assessment of Function (SAF) were conducted concurrently to show the validity of Arabic KOS-ADLS statistically in relation to these scales. Final translated version showed no significant discrepancies. Minor adaptive adjustment was required to fit Arabian cultural background. Internal consistency was favourable (Cronbach’s alpha 0.90). Patients’ scoring on Arabic KOS-ADLS appeared relatively consistent with their scoring on VAS, GUG, A/D Stairs, and SAF. A significant linear relationship was demonstrated between SAF and total KOS-ADLS scores on regression analysis (adj. R
 2 = 0.548). Arabic KOS-ADLS, as its English counterpart, was found to be a simple, valid, and useful instrument for knee function evaluation. Arabic version of KOS-ADLS represents a promising candidate for unconditional use among Arabic-speaking patients with knee complaints.

Arabic translation, cultural adaptation, and validation study of Knee Outcome Survey: Activities of Daily Living Scale (KOS-ADLS).

The ability to identify indicators of poor health-related quality of life (HRQoL) is crucial for both improving clinical care and determining targets of intervention for the prevention and treatment of disease. The main objectives of this study were to assess the HRQoL profile of the hypertensive population from Palestine, and to determine the socio-demographic and clinical characteristics associated with poor HRQoL. A cross sectional study was conducted, adopting the EuroQoL-5 Dimensions scale (EQ-5D-5 L) for the assessment of HRQoL. Hypertensive patients attending outpatients’ clinics at Al-Makhfyah primary health care clinic and from Alwatani Hospital, Nablus, Palestine were approached for study. Four hundred and ten hypertensive patients were enrolled in the study. Of these, 213 patients (52 %) were female. The average age of the study population was 58.38 ± 10.65 years. HRQoL was good, with a mean EQ-5D-5 L index value and EQ visual analogue scale (EQ-VAS) score of 0.80 ± 0.16 and 74.1 ± 15.6 respectively. There was a significant positive correlation (r = 0.56; p < 0.001) between the EQ-5D-5 L index values and the reported EQ-VAS scores. A significant difference in EQ-5D-5 L index values was found among participants when grouped according to age, occupation, marital status, income, educational level, duration of disease, total number of chronic diseases, and total number of medications (Kruskal–Wallis test; p-value < 0.05), as well as gender and therapy type (Mann–Whitney test, p-value < 0.05). This study highlighted that specific socio-demographic and disease-related characteristics of hypertensive patients as well as treatment factors were strongly associated with HRQoL. The study findings could be helpful in clinical practice, mainly in the early treatment of hypertensive patients, at a point where improving HRQoL is still possible.

Assessment of health-related quality of life among hypertensive patients: a cross-sectional study from Palestine

The aim of this study was to adapt the knee and hip osteoarthritis quality of life questionnaire (OAKHQoL) into Moroccan Arabic and to determine its psychometric properties. After translation, back-translation and pretesting, the translated version was submitted to an expert committee. The psychometric properties were tested on patients with hip or knee osteoarthritis. Internal consistency was tested using Cronbach’s alpha coefficient (α), and the test–retest reliability using intraclass correlation coefficients (ICC). Construct validity was assessed by examining item-convergent and divergent validity and by comparing the average scores between age groups and walk perimeter groups. The study was conducted on 131 patients (115 with osteoarthritis of the knee and 16 with osteoarthritis of the hip). The “physical activities” (α = 0.93), “mental health” (α = 0.84) and “pain” (α = 0.88) dimensions of the Arabic version were internally reliable. The ICC were adequate to good; 0.83 for “physical activities”, 0.65 for “mental health” and 0.70 for “pain” dimensions. The instrument demonstrated good construct validity; all items exceeded the 0.4 criterion for convergent validity, except items 13 and 41 and most of the correlations between items and their own scale were significantly higher than their correlations with other scales. A semantically equivalent translation has been developed with cultural adaptation of OAKHQoL. It is quite reliable and a valid measure of the effect of osteoarthritis on the quality of life on Moroccan patients.

Les problèmes du genou sont les plus invalidants et les plus fréquents des pathologies au maroc, rendant le patient incapable de travailler et de profiter pleinement de la vie d'où l'intérêt des échelles de mesure pour quantifier et mesurer l'ampleur de ce problème. L'objectif de ce travail est de réaliser une validation et une adaptation culturelle de la version marocaine de l’échelle KOOS-adulte en arabe dialecte. L'échelle KOOS-adulte contient 42 items et cinq sous-échelles : douleur ; symptômes ; difficultés dans les activités quotidiennes (ADL) ; sports/jeux et qualité de vie (QOL). Le score final de chaque sous-échelle varie de 0 pour des problèmes de genou extrêmes à 100 pour l'absence de problèmes de genou. Après avoir terminé la procédure de traduction et de retraduction, une version marocaine pré-finale a été obtenue et testée sur un échantillon de 20 patients souffrant de problèmes de genoux pour obtenir la version marocaine finale. On a mené en 2019, une étude transversale au niveau du CHU Hassan II de Fès. Ont été inclus les patients présentant des symptômes douloureux du genou, vus en consultation ou hospitalisés dans un service de rhumatologie ou un service de traumatologie pour adultes. Deux questionnaires de la version arabe du KOOS ont été administrés aux patients par deux enquêteurs différents, les participants ont été invités à remplir l'échelle KOOS deux fois avec un intervalle minimal de 15 jours. La validité interne a été évaluée par le coefficient α Cronbach, une valeur de 0,70 ou plus était considérée comme adéquate. La Fiabilité inter-évaluateur et test-retest ont été évalués par le coefficient de corrélation intraclasse (ICC). Nous avons utilisé l'analyse d'échelle multitrait pour examiner la mesure dans laquelle les items du questionnaire pouvaient être combinés dans les échelles multi-items hypothétiques. La validité du critère a été testée en utilisant la corrélation de Spearman entre les échelles de Koos et le SF36. Toutes les analyses statistiques ont été effectuées à l'aide du logiciel R Au total, 98 sujets ont été inclus dans l'étude. L'âge moyen était de 53,3 ans+/-16, 53,1 % était des hommes et généralement sans diplôme (30,9 %). L'arthrose du genou représentait 6,0 % de l'ensemble des lésions. Les scores pour différentes échelles variaient de 30,0 à 59,1. L'échelle « Sports/jeux » avait le score le plus bas. Aucun effet de plancher ni de plafond n'a été trouvé. Toutes les sous-échelles avaient une excellente validité interne (alpha de Cronbach >0,9), à l'exception de la sous-échelle des symptômes (alpha de Cronbach à 0,470). La fiabilité Test-retest a été évaluée par L'ICC, qui variait de 0,85 pour la «douleur» à 0,97 pour «ADL» et «QOL». La Fiabilité inter-évaluateur, variait de 0,90 pour «QOL» à 0,97 pour «ADL». Les résultats de l'analyse multitrait : ADL, Sport/jeux et QOL présentent une convergence d'items á 100 % (r : 0,69-0,86 ; 0,84-0,93 et 0,84-0,93, respectivement) et une discrimination d'items á 100 % (r : 0,19-0,68, 0,24-0,74 et 0,30-30). La douleur corporelle du SF-36 était fortement corrélée avec la sous-échelle douleur et qualité de vie du KOOS. L'échelle marocaine KOOS-adulte présente une bonne fiabilité, bonne capacité discriminante et une bonne validité interne, à l'exception de la sous-échelle des symptômes.
