Abstract: Drug hypersensitivity syndrome to both vancomycin and teicoplanin has not been previously reported. We describe here a 50-yr-old male patient with vertebral osteomyelitis and epidural abscess who developed hypersensitivity syndrome to both vancomycin and teicoplanin. Skin rash, fever, eosinophilia, interstitial pneumonitis, and interstitial nephritis developed following the administration of each drug, and resolved after withdrawing the drugs and treating with high dose corticosteroids. The vertebral osteomyelitis was successfully treated with 6-week course of linezolid without further complications. Skin patch tests for vancomycin and teicoplanin was done 2 months after the recovery; a weak positive result for vancomycin (10% aq.,+at D2 and +at D4 with erythema and vesicles; ICDRG scale), and a doubtful result for teicoplanin (4% aq.-at D2 and+/-at D4 with macular erythema; ICDRG scale). We present this case to alert clinicians to the hypersensitivity syndrome that can result from vancomycin and teicoplanin, with possible cross-reactivity, which could potentially be life-threatening.

Abstract: Background Vascular endothelial growth factor (VEGF) has been suggested to be a key mediator in the development of atopy and T H 2 inflammation. Objective We sought to evaluate the effects of variations in the gene coding VEGF receptor (VEGFR) 2 on intermediate phenotypes of asthma in the Korean population. Methods A cohort of 2055 children and adolescents responded to a questionnaire concerning asthma symptoms and risk factors and underwent methacholine bronchial challenge and skin tests. The VEGFR2 gene, including the promoter area, was sequenced on 24 healthy subjects to discover informative single nucleotide polymorphisms (SNPs; minor allele frequency >2%). After haplotype reconstruction, 4 tagging SNPs (IVS6+54A>G, +889G>A, +1416T>A, and IVS25-92G>A) were scored. These SNPs were also scored in 480 adult asthmatic patients to verify the above genetic association study. Results The prevalence of atopy was associated with a single SNP (+889G>A) of VEGFR2 with borderline significance ( P = .048; relative risk, 1.13; 95% CI, 1.00-1.28). However, haplotype analysis showed that the atopy prevalence was strongly associated with a haplotype (AGAG) of VEGFR2 ( P = .002; relative risk, 1.25; 95% CI, 1.09-1.42). As for airway hyperresponsiveness, neither individual SNPs nor haplotypes were found to be associated. Interestingly, the significant association was also found between atopy and the AGAG haplotype among adult asthmatic patients ( P = .008; odds ratio, 1.66; 95% CI, 1.14-2.44). Conclusions The present study demonstrated that genetic variations of VEGFR2 are significantly associated with atopy in the Korean population.

Abstract: It has been demonstrated that spider mites such as the two-spotted spider mite (Tetranychus urticae) are important allergens for fruit farmers. A total of 2,467 adults (795 metropolitan urban, 788 non-metropolitan urban, and 884 rural subjects) were enrolled. They responded to the questionnaire, and underwent methacholine bron- chial provocation tests as well as skin prick tests to locally common aeroallergens including the two-spotted spider mite. The prevalences of asthma and rhinitis as reported on the questionnaire were 7.8% and 16.4% of adults aged 20-35, 9.4% and 24.7% of those 36-50, and 17.7% and 21.7% of those older than 50, respec- tively. Among the older group, the two-spotted spider mite was the most common sensitizing allergen, although it was second of that of house dust mites among the other two age groups. Sensitization to the two-spotted spider mite was significantly associated with the prevalence of asthma and rhinitis among the younger age group, and associated with the prevalence of rhinitis among the older age group. The two- spotted spider mite might be a common sensitizing allergen in the general popula- tion of adults, and sensitization to this mite may play a role in the manifestation of asthma and rhinitis symptoms during adulthood.

Abstract: 이러한 부작용 중에서 아나필락시스를 포함하는 IgE 매개성 즉시형 과민 반응은 1% 미만으로 드문 것으로 알려져 있으 나 2,3) 저혈압성 쇼크 등 생명을 위협할 정도로 중증도가 높 으면서도 빠른 경과를 보여 임상적인 중요성을 지닌다. 국소 마취제에 아나필락시스의 병력이 있는 환자에게 다 시 국소 마취제를 투여해야 할 때에 대체 약물이 있는 경우 에는 피부단자 검사, 피내 검사 등을 통하여 약제 간에 교차 반응이 없음을 확인 후 그 약물을 투여해야 한다. 반면에 다른 대체 약물이 없는 경우에는 탈감작 요법(desensitization) 을 시행 후 해당 약물을 사용해야 한다. 4,5) 그러나 실제적으 로 약제 간의 교차 반응이 드물지 않게 일어나기 때문에 대체 약물 선정에 어려움이 있다. 저자들은 lidocaine에 대해서 아나필락시스 반응을 보인 환자에게 lidocaine 및 다른 국소 마취제에 대한 피부단자 검 사 및 피내 검사를 시행하여 약물 알레르기로 인한 부작용 이 적을 것으로 예상되는 국소 마취제를 선정하여 안전하 게 투여한 바 있어서 이를 증례 보고하고자 한다. 이 논문은 국내에서 처음으로 리도카인 아나필락시스 증례를 보고했 다는 점에서도 의미가 있다.
