Wearable healthcare: Lessons from the past and a peek into the future

Physician chart documentation can facilitate patient care decisions, reduce treatment errors, and inform health system planning and resource allocation activities. Although accurate and complete patient chart data supports quality and continuity of patient care, physician documentation often varies in terms of timeliness, legibility, clarity and completeness. While many educational and other approaches have been implemented in hospital settings, the extent to which these interventions can improve the quality of documentation in emergency departments (EDs) is unknown. We conducted a systematic review to assess the effectiveness of approaches to improve ED physician documentation. Peer reviewed electronic databases, grey literature sources, and reference lists of included studies were searched to March 2015. Studies were included if they reported on outcomes associated with interventions designed to enhance the quality of physician documentation. Nineteen studies were identified that report on the effectiveness of interventions to improve physician documentation in EDs. Interventions included audit/feedback, dictation, education, facilitation, reminders, templates, and multi-interventions. While ten studies found that audit/feedback, dictation, pharmacist facilitation, reminders, templates, and multi-pronged approaches did improve the quality of physician documentation across multiple outcome measures, the remaining nine studies reported mixed results. Promising approaches to improving physician documentation in emergency department settings include audit/feedback, reminders, templates, and multi-pronged education interventions. Future research should focus on exploring the impact of implementing these interventions in EDs with and without emergency medical record systems (EMRs), and investigating the potential of emerging technologies, including EMR-based machine-learning, to promote improvements in the quality of ED documentation.

Strategies for improving physician documentation in the emergency department: a systematic review

Improving Performance in Diabetes Care: Benefits of Information Technology Enabled Diabetes Management.

The prevalence of diabetes is rising globally. Diabetes patients need continuous monitoring, and to achieve this objective, they have to be engaged in their healthcare management process. Mobile health (MH) is an information and communications technology trend to empower chronically ill patients in a smart environment. Discussing the current state of MH technologies is required in order to address their limitations. Existing review articles have evaluated the MH literature based on applicability and level of adoption by patients and healthcare providers. Most of these reviews asserted that MH apps and research have not reached a stable level yet. To the best of our knowledge, there is no clear description of solutions to these problems. In addition, no one has investigated and analyzed MH in its contextual environment in a detailed way. We conducted a comprehensive survey of MH research on diabetes management articles published between 2011 and September 27, 2017. In this survey, we discuss current challenges in MH, along with research gaps, opportunities, and trends. Our literature review searched three academic databases (ScienceDirect, IEEE Xplore, and SpringerLink). A total of 60 articles were analyzed, with 30% from ScienceDirect, 38% from IEEE Xplore, and 32% from SpringerLink. MH was analyzed in the context of the electronic health record (EHR) ecosystem. We consider dimensions such as clinical decision support systems, EHRs, cloud computing, semantic interoperability, wireless body area networks, and big data analytics. We propose specific metrics to analyze and evaluate MH from each of these dimensions. A comprehensive analysis of the literature from this viewpoint is valuable for both theoretical and developmental progress. This paper provides a critical analysis of challenges that have not been fully met and highlights directions for future research that could improve MH applicability.

With the Internet permeating every aspect of daily life, organizations of all types are increasingly concerned about the degree to which their employees are cyberloafing by shirking their work responsibilities to surf the Internet, check e-mail, or send text messages. Although technological interventions against cyberloafing have been shown to be effective, they might be perceived by employees as an invasion to their privacy, and are expected to have repercussions on employee behavior and loyalty. The main objectives of this study are to (1) examine how the introduction of such technological interventions might affect employees' emotions and fairness perceptions, and (2) understand the effect of the interventions on behavioral outcomes, i.e., employees' intentions to cyberloaf and their loyalty to the company. We developed a justice-based framework that we empirically test using a field experiment composed of field surveys complemented with hypothetical scenarios describing new organizational initiatives to curb employees' cyberloafing. Our findings suggest that technological interventions, although associated with perceptions of unfairness, are effective at controlling cyberloafing, albeit at the expense of employee loyalty. On the other hand, contrary to prior findings, we find that fairness perceptions of technological interventions, although reinforcing employee loyalty, are ineffective at curbing cyberloafing. These findings are especially enlightening in that they contradict a common belief that perceived fairness encourages employees, as a sign of their appreciation for this fairness, to curb their misuse of IT. The findings also help managers fine-tune their cyberloafing policies to achieve a long-lasting remedy to their employees' cyberloafing while maintaining a necessary level of employee loyalty.

Cyberloafing in the workplace: mitigation tactics and their impact on individuals' behavior

Objectives Electronic Medical Record (EMR) systems have become an integral part of patient care, in both inpatient and outpatient settings. The objective of this paper is to propose a set of recommendations on how the Epic EMR system can be used to improve patient care. To this end, we present findings on the use of the Epic EMR system in the University of Virginia (UVa)׳s Health System. Target audience Healthcare organizations implementing electronic medical record systems and health technology managers. Methods Face-to-face interviews with 30 of UVa׳s hospital personnel and others in the Epic department at UVa. Results and conclusions Three key areas are discussed to determine the feasibility of improvement including a decrease in medical errors and the resulting parallel improvement in patient safety, inter-disciplinary collaboration, and a decrease in the overall cost of healthcare. We identified many discrepancies between the Epic EMR system’s intended use, and the workaround system that clinicians have used to document patient care. In addition, we discuss a dichotomy in perspectives amongst the Health System and Technology Services department at UVa, and healthcare staff end users, with regard to the intended functionality and the usability of the Epic EMR system. Evaluating the epic electronic medical record system: A dichotomy in perspectives and solution recommendations

People with chronic illnesses are engaged in the lifelong management of their disease and with this management comes the need for emotional support, informational support, and companionship from others, particularly those who have the same disease and can relate to their situation. More often people are seeking this social support through online health communities. The concern is that online health communities may not have a positive impact on the health condition since anyone can be providing information and the information provided may not be valid. We utilize an objective measure of the health condition to determine the impact social support, given and received, in an online health community has on the user’s health condition over time.
