Effectiveness of Probiotics in the Prophylaxis of Necrotizing Enterocolitis in Preterm Neonates: A Systematic Review and Meta-analysis

Abstract: OBJECTIVE: To elucidate the benefits of using probiotics in the prevention of necrotizing enterocolitis (NEC) and its complications in preterm newborns. METHOD: This was a systematic review of randomized controlled trials, which included studies retrieved from three databases (MEDLINE, Embase, and LILACS), using a combination of the terms (necrotizing enterocolitis) AND (probiotics). RESULTS: 11 randomized trials were included, totaling 2,887 patients, 1,431 in the probiotic group and 1,456 in the control group. There was a reduction in the incidence of NEC (NNT = 25), overall death (NNT = 34), and neonatal sepsis (NNT = 34) in the probiotic group compared to the control group. Patients that received probiotic supplementation had lower food reintroduction time (p < 0.001) and hospitalization time (p < 0.001) when compared to those not receiving probiotics. There was no difference in mortality caused by NEC. CONCLUSION: In premature newborns, the use of probiotics is effective as a prophylaxis for NEC and its complications.

A influência da Reforma Sanitária na construção das bioéticas brasileiras

Abstract: This revision article remakes in general guidelines the path to the Brazilian Bioethics in the planning of research and education autochthones lines from the relations between these lines and the social and historical process that consolidated the Brazilian Sanitary Reform. The works and theoretical perspectives selected have the proposal to act in the some scope, the social dimension; that relate to groups or particularly vulnerable segments that question the power relationship from the identification of the social inequalities as well as to the ones focused in the same orienting parameters, the Human Rights treaties; or the ones that included in its themathical rol the discussion about quality of life. The presentation of these distinct points of views of the Brazilian Bioethics are circumscribed at the tangential specifically relationed above and chronologically orientated from the creation of the "Bioethics" term in the North American academic environment.

Bioética de intervenção: considerações sobre a economia de mercado

Abstract: Este trabalho apresenta os marcos teoricos da bioetica de intervencao a partir de seu documento referencial – Intervention bioethics: a proposal for peripheral countries in a context of power and injustice – apresentado na sessao de abertura do Sexto Congresso Mundial de Bioetica. A bioetica de intervencao caracteriza-se por ser uma discussao aberta e plural, voltada a complexidade das relacoes de producao e inter-relacoes humanas nas sociedades contemporâneas. Para atingir tal objetivo, faz-se necessario romper os limites cartesianos que circunscrevem os saberes a campos especificos, gerando conhecimentos transdisciplinares capazes de responder dinamicamente as transformacoes da realidade; o que, basicamente, constitui o campo da etica aplicada.

Primary neural leprosy: systematic review

Abstract: The authors proposed a systematic review on the current concepts of primary neural leprosy by consulting the following online databases: MEDLINE, Lilacs/SciELO, and Embase. Selected studies were classified based on the degree of recommendation and levels of scientific evidence according to the "Oxford Centre for Evidence-based Medicine". The following aspects were reviewed: cutaneous clinical and laboratorial investigations, i.e. skin clinical exam, smears, and biopsy, and Mitsuda's reaction; neurological investigation (anamnesis, electromyography and nerve biopsy); serological investigation and molecular testing, i.e. serological testing for the detection of the phenolic glycolipid 1 (PGL-I) and the polymerase chain reaction (PCR); and treatment (classification criteria for the definition of specific treatment, steroid treatment, and cure criteria).

A importancia da dimensao espiritual na pratica dos cuidados paliativos

Abstract: Este estudio enfoca la importancia de los valores religiosos y espirituales, asi como la fe de las personas en lo enfrentamiento y en el relacionamiento con los grandes eventos de la vida humana: nacimiento, dolor, sufrimiento, y el mas alla de la vida, entre otros. Nuestro itinerario reflexivo abre la puerta del mundo de las grandes religiones, presentando algunos de sus valores fundamentales, entre otros; buscase en las raices de las religiones sus puntos de convergencia y sus distinciones sin separar religion, espiritualidad y mistica. En seguida, delimitamos nuestro enfoque en el campo de la medicina resaltando algunos documentos internacionales que valoran la dimension de la espiritualidad en el ambito de los cuidados de asistencia a la salud y de los cuidados paliativos. Hacemos tambien referencia a la carta brasilena de los derechos de los usuarios de la salud, que reconoce el derecho de ser cuidados en terminos espirituales. Llegamos a la conclusion de que, de manera fundamental, la espiritualidad se relaciona con la busqueda transcendente de un sentido mayor en lo aparente absurdo de nuestras experiencias del dolor, sufrimiento, perdida, angustia y hasta mismo del miedo a la muerte
