Abstract: Bangladesh is an endemic region of dengue fever and experienced an unprecedented large outbreak with more than 100,000 confirmed cases in 2019. To understand the prevalence of dengue antibody in patients and molecular epidemiological characteristics of dengue virus (DENV) in this outbreak, a total of 179 blood samples were collected from patients in 10 districts (seven divisions) covering nearly the whole country from August to December 2019. DENV NS-1 was detected in 162 samples, among which DENV-specific IgM was positive in 119 samples (73.5%), including 60.5% samples also positive for DENV-specific IgG. Sequencing of the partial C-prM gene and its phylogenetic analysis revealed predominance of DENV type 3 genotype I, accounting for 93% of samples examined. DENV-3 genotype III was identified in two samples from separate districts, and only one DENV-2 cosmopolitan genotype was found in the capital city, Dhaka. These findings suggest the predominance of DENV-3 genotype I and occurrence of DENV-3 genotype III, associated with increased incidence of recent secondary infection in Bangladesh in 2019.

Abstract: Despite the fact that Pneumomediastinum owing to a foreign body is uncommon, it might present a difficult clinical scenario in the absence of any suggestive history. Foreign bodies that are radiolucent are more challenging to diagnose. We present a case report of an eight-year-old boy from Bangladesh who developed pneumomediastinum and subcutaneous emphysema due to organic foreign body inhalation which was initially misdiagnosed in immediate clinical setting. We discussed how a thorough history and a high level of suspicion aided us in diagnosing and treating such a case.

Abstract: Introduction Medical students are vulnerable group to migraine, one of the most common type of headache worldwide. The aim of the present study was to determine the prevalence of migraine and related disability among medical students of Bangladesh. Methods This cross-sectional study was conducted among 1327 students from six medical colleges Bangladesh during March 2021 through a self-administered online survey. ID Migraine™ scale and MIDAS scale were used to screen migraine and migraine related disability respectively. Frequency distribution, and Chi-square test, t-test along with multiple logistic regressions model were used to determine the prevalence and associated factors of migraine respectively. Results The overall prevalence of migraine among the participants was 19%. The prevalence was higher among females (27%) than males (8%). Female sex (aOR 4.11, 95% CI 2.79-6.03) and poor sleep quality (aOR 2.07, 95% CI 1.48-2.91) were identified as independent risk factors of migraine. More than 90% migrainures reported to suffer from moderate to severe headache. Nausea was most commonly reported associated symptom (83.5%) followed by photophobia (72%) and vomiting (53%). Self-reported mental stress (55%), irregular sleep (49%), noise (30.5%), and usage of electronic device (30.5%) were most commonly reported triggering factor of migraine attack. More than half of the sufferers reported severe migraine related disability (MIDAS score ≥ 21). Conclusions The prevalence of migraine among medical students of Bangladesh is alarmingly high. Frequent migraine attacks and severe intensity of headache cause a substantial level of disability among the sufferers. Cautious avoidance of the triggering factors through appropriate interventions and prophylactic medication can mitigate the negative impact of migraine as well as improve the quality of life.

Abstract: Background Gene alterations are required for the development of gastric cancer, which are influenced by environmental and host factors. We conducted the present study to find the status of Helicobacter pylori (H. pylori) infection and its association with altered genes P53, hMLH1, and HER2 in gastric cancer patients and to analyze their correlation with clinical, pathological, and environmental factors.Method This was a cross-sectional study. For genetic (P53 and hMLH1) study of the gastrectomized tissue DNA extraction and optimization, we performed PCR amplification and DNA sequencing. HER2 was studied by immunochemical technique. The results were matched with tumor status, age and sex, smoking, and H. pylori antibody status of the patients to find their association.Results The mean age of the patients was 52.91 (±13.94) years. Among the 45 patients selected for genetic tests, 12 aged 40 or more and 33 aged over 40. Among the genes, 33(73.3%) in P53 and 17(37.7%) in hMLH1 were mutated and 11(24.2%) in HER2 were found to be overexpressed. Chi square and regression analysis showed that they all had associations with H. pylori positivity (P 1). hMLH1 was associated with the location of the tumor, smoking, sex, blood group, and age, and P53 was found to be affected by extra salt intake, sex, blood group, and age of the patients (P ≤ 0.05 ).Conclusion Genetic mutation was found in nearly all the patients with gastric cancer, which was significantly associated with H. pylori infection. Mass eradication of this organism might play a role in reducing cancer incidence in Bangladesh.

Abstract: Though prepubertal vaginal bleeding is rare, it can create a dilemma for both physicians and parents. Prepubertal vaginal bleeding due to leech bite is even rarer and very difficult to diagnose without proper history. We report a case of a six-year-old girl presenting with vaginal bleeding and ultimately diagnosed as a case of vaginal bleeding due to leech bite. We described how focused history helped us in diagnosis and management of such case in rural areas with limited healthcare resources. A detailed history with a high index of suspicion and a thorough examination is essential to diagnose leech bite in the vagina.

Abstract: Background: Effective management of postoperative pain provides a significant successful outcome of any surgical cases. Objectives: This study was designed to address the evaluation of using analgesics in relieving postoperative pain. Methods: After taking proper approval from hospital administration a prospective observational study was conducted in 250 surgically treated patients from January 2019 to May 2019 in a tertiary care hospital where patient of all ages and both sexes were included and prescriptions which did not contain analgesics were excluded from the study. Data was collected from day of operation to 5th post- operative day (POD) of each individual patient. Analysis of data was done by SPSS of 18.0 version. Results: Among 250 patients recruited 165 (66%) patients were female and highest numbers of patients were in the age group 18- 37 years 180 (72%). In the day of operation opioid tramadol and non-opioid diclofenac was mostly prescribed analgesic. Associated analgesicswere pethidine and or ketorolac. Among different combinations of analgesics, ketorolac & diclofenac combination 33% is used in highest cases. Average number of analgesic was 2.1 in the day of operation which became 1 in 5th POD. Percentage of patients prescribed analgesics from national essential drug list was 22%. The most common route of administration was intravenous (91.6%). Oral form was prescribed in 4th & 5th postoperative day. Conclusion: These results emphasize that a further study is needed to improve analgesic use by following standard treatment guideline to achieve effective pain management.

Abstract: Chronic Kidney disease is becoming a global public health problem throughout the world. The aim of the study was to assess management status of the hemodialysis unit in a specialized hospital. It was a hospital based descriptive cross sectional study. A total of 17 participants were selected by purposive sampling method on the basis of defined selection criteria from January 2015 to December 2016. The place of study was National Institute of Kidney Diseases and Urology (NIKDU) hospital, Dhaka. The research instruments were a semi structured questionnaire and a checklist. The socio-demographic status of the respondents showed that 11.8 %( n=02) were doctor, 58.8 %( n=10) nurses and 29.4 %( n=05) were technician. Mean age of care providers was 38.82±8.79 years. Majority of respondents, were satisfied regarding the unit including physical facilities 76.5% (n=13) cleanliness of the unit 58.8% (n=10) and medical record facility 52.9% (n=9). Need to strengthen infection control measure and BCC activities among the service providers to improve the management status of the hemodialysis unit of NIKDU hospital. Asian J. Med. Biol. Res.
