Abstract: A case of coexisting Klinefelter's Syndrome and primary hypothyroidism is presented. Enlargement of the pituitary fossa was noted and the probable reason for such enlargement is discussed. We believe this to be the first case of Klinefelter's Syndrome with primary hypothyroidism in which benign pituitary enlargement has been recorded.

Abstract: MONITORING the perfusing pressure, with constant flow perfusion, and isometric recording of longitudinal tension, the responses of the circular and longitudinal muscle in the wall of the dog anterior mesenteric vein to transmural electrical stimulation were recorded simultaneously. It was concluded from the results with tetrodotoxin and the local anaesthetic agents, cocaine and lignocaine, that the contractions were due to the excitation of nerves. The findings with alpha blocking agents, reserpine, guanethidine, hexamethonium and atropine, suggest that these nerves are post-ganglionic sympathetic noradrenergic fibres. This preparation may be a useful model for further studies on the innervation of venous smooth muscle.

Abstract: BECAUSE of the known effect of RNA inhibitors in Paget's disease of bone, a study was made of the RNA end-product, pseudouridine, in urine of such patients. Measurements were made before and after treatment with actinomycin D and mithramycin. Urinary levels were above normal in all patients studied and returned towards normal levels after treatment. The change in levels of pseudouridine followed closely those of urinary hydroxyproline excretion. There appears to be a basic RNA abnormality in Paget's disease of bone which can be reversed by the use of RNA inhibitors.

Abstract: THE stress of myocardial infarction or surgery has been studied with sequential intravenous glucose tolerance tests for growth hormone, insulin, glucose and with 24-hour urinary epinephrine and norepinephrine assays. Elevated catecholamines were observed in all patients but were not specifically related to the observed growth hormone and insulin changes. Insulin was elevated initially in 60 per cent, and sluggish insulin responses were found initially in 40 per cent. Sixty-six per cent had elevation of growth hormone initially; sixteen per cent had a paradoxical growth hormone response initially, and two patients failed to respond to intravenous glucose. Plasma cortisols were more readily suppressed than plasma growth hormone.

Abstract: MESIAL temporal sclerosis, or Ammon's horn sclerosis, as it used to be called, is the commonest single lesion to be found at autopsy in the brains of epileptic patients who die a natural death. Whenever it does occur, it is unilateral in 80 per cent of instances. Recent evidence suggests that, although there may be some genetic factor, it is really an acquired lesion, and is commonly the result of some hypoxic episode occurring in infancy, such as a severe febrile convulsion. Post-natal and birth injuries play but little part. The case histories of two teen-age girls are described both of whom had this lesion as the substrate of their habitual epilepsy. Both have been completely relieved of epilepsy by resection of the affected temporal lobe. The medical and social problems which they and other children like them encountered are discussed. Temporal lobe epilepsy probably accounts for from 10 to 15 per cent of all epileptic children referred to hospitals. A small proportion of these may likewise be benefited by surgery.

Abstract: SummaryThe thromboplastic behaviour of isolated pure phospholipids has been investigated under defined conditions. In conventional citrated platelet-free non-contact human plasma, phosphatidyl-serine (PS) developed thromboplastic activity equivalent to or better than that of whole platelet extract only in the presence of phosphatidyl choline (PC) in equimolecular amounts. Phosphatidyl ethanolamine (PE) and phosphatidyl inositol (PI) showed insignificant activity; phosphatidyl choline alone and sphingomyelin (SPH) were inert. In recalcified dialysed citrate-free plasma, PE is more active than PS, but PC has no influence upon PS or PE. Free long-chain fatty acids induce contact-activation (procoagulant) in plasma as well as functioning as antithromboplastins (anticoagulant). In citrated plasma, some FA enhance the activity of PE but not of PS. Since, apart from free FA, no other platelet lipid is active in thromboplastin generation, it is proposed that PS and PE may be the individual phospholipid species required at two independent stages of the thromboplastic sequence.

Abstract: Twenty-two consecutive below-knee amputations using a long posterior flap myoplastic technique were carried out on 20 patients. There was primary healing in 20 (91 per cent). There was delayed healing in two stumps. Two patients died. In the survivors 18 out of 20 stumps (90 per cent) were in good condition at follow-up. This report endorses the view that the long posterior flap operation permits successful below-knee amputation in many patients with obliterative vascular disease of the lower limb.

Abstract: A case of cranial eosinophilic granuloma complicated by diabetes insipidus is presented. Vinblastine sulphate successfully healed the eosinophilic granuloma. Chlorpropamide achieved control of the diabetes insipidus, in that daily urinary output was halved, and urinary concentrating ability increased. Hypoglycaemia did not occur.

Abstract: SIX patients with chronic renal disease treated by renal transplantation 26–50 months previously have been followed clinically and electro-diagnostically. Patients who had parathaesiae before the operation no longer complained of this symptom after transplantation, and one also became free of neurological signs. Various types of nervous system manifestations were present in combination in four patients: peripheral neuropathy in one patient, symptoms of restless legs in two, limb girdle weakness in three and two patients had evidence of cerebral vascular disease. Two patients developed impotence.

Abstract: PATIENTS who came to autopsy with atrophy, fibrosis of the pancreas, chronic pancreatitis and cystic fibrosis, were examined for hepatic haemosiderosis. In adult patients dying from chronic pancreatic disease, there was no statistically significant increase in hepatic haemosiderosis. The liver haemosiderosis in young children below the age of two years, with cystic fibrosis and extrapancreatic disease, was due to residual fetal iron stores. No conclusive evidence was found for increased liver haemosiderosis in chronic pancreatic disease.

Abstract: The intestinal mucosa was investigated by light and electron microscopic techniques in a group of 7 patients with thyrotoxicosis. Steatorrhoea was found in two patients and can be accounted for by other medical states. The intestinal mucosa was essentially normal in all seven cases including those with biochemical evidence of malabsorption. Urinary excretion of D-xylose and serum carotene have not been found to be reliable screening tests for malabsorption in thyrotoxicosis and possible explanations are discussed. It is concluded that thyrotoxicosisper se is not associated with structural alterations in the intestinal mucosa.

Abstract: Using a single radial diffusion technique the serum levels of IgA, IgG and IgM were estimated in sixty healthy members of the hospital staff, seventy-two patients suffering from bronchiectasis and forty-two patients suffering from chronic bronchitis and/or recurrent pneumonia. Two patients, both with bronchiectasis, were found to have hypogamma-globulinaemia. Two patients had low levels of IgA, and six patients had subnormal levels of IgM. The fact that deficiency of IgM was found in chest disease sufferers more frequently than in healthy individuals suggests that this form of dysglobulinaemia may be a relevant factor in the occurrence of repeated episodes of chest infection. It may thus be possible to identify a group of patients who are likely to suffer respiratory disablement and to take measures to prevent or delay this complication.
