Dynamique psychique des patients obèses sévères en demande de chirurgie bariatrique : approche clinique interculturelle

Abstract
Les auteurs s’intéressent à la problématique de l’obésité sévère, notamment au fonctionnement psychique sous-jacent des patients en demande de chirurgie bariatrique. Ils proposent une comparaison interculturelle entre la France et la Turquie en questionnant l’implication de la culture dans la psychogénèse de cette maladie chronique. Ils présentent les résultats d’un premier volet exploratoire qualitatif, par l’analyse comparée de deux cas cliniques, représentatifs de chacun des deux pays. Cette analyse s’appuie sur des entretiens cliniques, des autoquestionnaires et sur le test de Rorschach. Cette comparaison culturelle montre que les deux cas cliniques représentatifs ont des antécédents de vie difficiles et traumatiques, souvent d’origine infantile. Ces événements conduisent à des difficultés de mentalisation des affects, constatés lors des entretiens cliniques, des autoquestionnaires et de la passation du test de Rorschach. Ainsi, l’impossibilité de ce travail de transformation psychique conduit à des Troubles des Conduites Alimentaires (TCA). Ces premiers résultats invitent à poursuivre la construction et la validation d’un modèle transculturel de l’obésité sévère, permettant dans l’avenir de proposer des recommandations internationales pour la prise en charge psychologique de cette maladie chronique, particulièrement lors d’une chirurgie bariatrique. The authors are interested in the problematic of severe obesity, particularly in the underlying psychic function of patients who seek bariatric surgery. They propose an intercultural comparison between France and Turkey by questioning the implication of culture in the psychogenesis of this chronic disease. They represent the results of a first qualitative exploratory section, by the comparative analysis of two clinical cases which are representatives of each of the two countries. This analysis is based on clinical interviews, self-questionnaires and Rorschach Test. This cultural comparison shows that two representative clinical cases have antecedents with difficult and traumatic life experiences, mostly based on infancy background. These incidents lead to difficulties of mentalization of affects as it is occurred during the clinical interviews, the self-questionnaires and the process of Rorschach Test. Therefore, the impossibility of this work of psychic transformation leads to Eating Disorders. These initial results call for further construction and validation of a cross-cultural model of severe obesity, allowing in the future to propose international recommendations for the psychological treatment of this chronic disease, especially during bariatric surgery.
