ダビガトランエテキシラートメタンスルホン酸塩(以下,ダビガトランエテキシラート,プラザキサ®)は「非弁膜症性心房細動患者における虚血性脳卒中及び全身性塞栓症の発症抑制」を適応として,2011年1月に,日本で初めての経口直接トロンビン阻害薬として承認された.ダビガトランエテキシラートはダビガトランのプロドラッグであり,生体内でエステラーゼによって活性代謝物であるダビガトランに変換され,強力で選択的な直接トロンビン阻害薬となる.ダビガトランはヒト血漿および他の動物種を用いた内因系,外因系または共通の血液凝固系経路の検査で高い抗凝固活性を示すとともに,フィブリンに結合した固相トロンビンと液相中の遊離トロンビンの両方を同程度に阻害する.さらにダビガトランのin vitroならびにダビガトランエテキシラートのex vivoにおける抗凝固作用ならびに血栓モデルにおける抗血栓作用は,速やかに発現し,用量依存性が認められる.また,ダビガトランの抗血栓作用は,納豆などのビタミンK含有食物の影響を受けないことが動物実験で明らかとなっている.脳卒中危険因子を有する心房細動患者に対する血栓塞栓症の発症予防療法としては,ビタミンK拮抗薬(ワルファリンなど)による抗凝固療法が各種のガイドラインで推奨されているが,ワルファリンは,定期的な血液凝固パラメータのモニタリングによる用量調節が必要であることや,薬物相互作用が多いおよびビタミンKを含む食物の摂取制限が必要であるなどの問題を有している.臨床試験において,ダビガトランは,日本人を含む非弁膜症性心房細動患者を対象とした国際共同第III相試験で,脳卒中および全身性塞栓症の発症における抑制効果においてワルファリン投与に比べ優越性が認められた.また,頭蓋内出血などの重篤な出血のリスクもワルファリンに比べて低いことが確認された.ワルファリンで問題となっている治療方法の改善も期待され,心房細動患者に対する血栓塞栓症の発症予防療法として標準薬あるいは第一選択薬の1つとなりえる薬剤であると考えられる.read more

Clinically important drug interactions with anticoagulants. An update
TL;DR: The complex response of coumarins to concomitant drug therapy makes it difficult to predict the occurrence and degree of a deterioration of anticoagulant control in individual patients and it seems advisable that one should monitor for changes in prothrombin time when adding or deleting any newly approved drug or any drug suspected to cause an interaction.
