TL;DR: A spinal stabilizing system as mentioned in this paper includes a collar having a tubular sidewall with an interior surface and an exterior surface, the interior surface at least partially bounding a longitudinal passage extending therethrough.
Abstract: A spinal stabilizing system includes a collar having a tubular sidewall with an interior surface and an exterior surface, the interior surface at least partially bounding a longitudinal passage extending therethrough. A shoulder radially inwardly projects from a second end of the sidewall so as to at last partially encircle the longitudinal passage. A pair of spaced apart channels transversely extend through the sidewall at the first end thereof. The system further includes a screw having a threaded portion and an enlarged head mounted on the end thereof, the head of the screw resting against the shoulder of the collar so that the head can pivot on the shoulder, a locking slot being formed on the head of the screw. A pin is secured to the collar and projects into the locking slot on the head of the screw such rotation of the collar facilitates rotation of the screw.

TL;DR: In this paper, a heart valve prosthesis includes an annular member implantable within a biological annulus, a collar extending upwardly from the annular members, and a sewing ring extending from the anular member.
Abstract: A heart valve prostheses includes an annular member implantable within a biological annulus, a collar extending upwardly from the annular member, and a sewing ring extending from the annular member. A spring structure couples the collar to the annular member and biases the collar to align with the annular member at a predetermined distance above the annular member. During use, the prosthesis is introduced into a biological annulus, and fasteners are directed through the sewing ring into surrounding tissue to secure the prosthesis with the annular member within the biological annulus. A mechanical or bioprosthetic valve is introduced and coupled to the collar, e.g., using tabs or other connectors on the collar. The spring structure allows the collar to be directed towards the annular member and/or folded inwardly, e.g., to facilitate accessing the sewing ring to deliver fasteners.

The collar has a circular opening (210) in the bottom, with a relief (212) extending from the circular opening. The second cross-sectional shape of the bone fastener is keyed to the opening to permit insertion of the bone fastener into the collar assembly, from the bottom. After insertion, the bone fastener is rotated to prohibit removal of the bone fastener from the collar. The collar can then be rotated and/or angulated relative to the bone fastener. An elongated member can be positioned in the collar and a closure member is then used to secure the elongated member to the collar.
