TL;DR: Xpert testing for the diagnosis of TB should be performed in patients for whom the probability of TB was lower than the test-treat threshold, because even a negative Xpert result will not lower the post-test probability below the treatment threshold.

TL;DR: The large majority of national HIV clinical practice guidelines had methodological weaknesses and content inaccuracies and countries require assistance with the adaptation process to ensure that guidelines are valid and up to date and accurately reflect WHO global clinical care recommendations for patients with HIV.
