A cognitive theory of post traumatic stress disorder (PTSD) is proposed that assumes traumas experienced after early childhood give rise to 2 sorts of memory, 1 verbally accessible and 1 automatically accessible through appropriate situational cues. These different types of memory are used to explain the complex phenomenology of PTSD, including the experiences of reliving the traumatic event and of emotionally processing the trauma. The theory considers 3 possible outcomes of the emotional processing of trauma, successful completion, chronic processing, and premature inhibition of processing. We discuss the implications of the theory for research design, clinical practice, and resolving contradictions in the empirical data.

A dual representation theory of posttraumatic stress disorder.

This review examines the diagnostic application of saliva for systemic diseases. As a diagnostic fluid, saliva offers distinctive advantages over serum because it can be collected non-invasively by individuals with modest training. Furthermore, saliva may provide a cost-effective approach for the screening of large populations. Gland-specific saliva can be used for diagnosis of pathology specific to one of the major salivary glands. Whole saliva, however, is most frequently used for diagnosis of systemic diseases, since it is readily collected and contains serum constituents. These constituents are derived from the local vasculature of the salivary glands and also reach the oral cavity via the flow of gingival fluid. Analysis of saliva may be useful for the diagnosis of hereditary disorders, autoimmune diseases, malignant and infectious diseases, and endocrine disorders, as well as in the assessment of therapeutic levels of drugs and the monitoring of illicit drug use.

The Diagnostic Applications of Saliva— A Review:

Studies during the last 20 years have indicated that enamel-related proteins are involved in the formation of cementum. In the present article, this relation is further explored. Attention is called to the fact that coronal acellular extrinsic fiber cementum is formed on the enamel surface in a number of species. The composition of the enamel matrix proteins and the expression of these proteins during root formation are briefly reviewed. The dominating constituent of the enamel matrix, amelogenin, is shown by means of immunohistochemistry to be expressed in human teeth during root formation. Amelogenin was also found to be present in Tomes' granular layer of human teeth. When mesenchymal cells of the dental follicle were exposed to the enamel matrix a non-cellular hard tissue matrix was formed at the enamel surface. Application of porcine enamel matrix in experimental cavities in the roots of incisors of monkeys induced formation of acellular cementum that was well attached to the dentin. In control cavities without enamel matrix, a cellular, poorly attached hard tissue was formed. The present studies provide additional support to the idea that enamel matrix proteins are involved in the formation of acellular cementum and also that they have the potential to induce regeneration of the same type of cementum.

BACKGROUND: Over the past 10 years, the use of saliva as a diagnostic fluid has gained attention and has become a translational research success story. Some of the current nanotechnologies have been demonstrated to have the analytical sensitivity required for the use of saliva as a diagnostic medium to detect and predict disease progression. However, these technologies have not yet been integrated into current clinical practice and work flow. CONTENT: As a diagnostic fluid, saliva offers advantages over serum because it can be collected noninvasively by individuals with modest training, and it offers a cost-effective approach for the screening of large populations. Gland-specific saliva can also be used for diagnosis of pathology specific to one of the major salivary glands. There is minimal risk of contracting infections during saliva collection, and saliva can be used in clinically challenging situations, such as obtaining samples from children or handicapped or anxious patients, in whom blood sampling could be a difficult act to perform. In this review we highlight the production of and secretion of saliva, the salivary proteome, transportation of biomolecules from blood capillaries to salivary glands, and the diagnostic potential of saliva for use in detection of cardiovascular disease and oral and breast cancers. We also highlight the barriers to application of saliva testing and its advancement in clinical settings. SUMMARY: Saliva has the potential to become a first-line diagnostic sample of choice owing to the advancements in detection technologies coupled with combinations of biomolecules with clinical relevance.

Diagnostic Potential of Saliva: Current State and Future Applications

Fibro-osseous lesions of the jaws.

Myoepithelial cells were present between the basal lamina and the acinar secretory cells of human labial salivary glands. In form and disposition, they resembled myoepithelial cells in the major salivary glands. Many of these cells possessed single cilia on their upper surfaces. Such cilia occasionally extended into invaginations of the overlying secretory cell.



The intercalated ducts were variable in occurrence. Their epithelium ranged from columnar to squamous, and showed few signs of secretory activity.



Few intralobular ducts possessed basal striations. While mitochondria were abundant in non-striated cells, they were randomly disposed in both basal and apical cytoplasm, and the basal plasmalemma showed only occasional infoldings. The paucity of true striated ducts in labial salivary glands may be responsible for the high concentration of sodium and chloride in unstimulated labial gland salivary secretions.

A COMPREHENSIVE examination of the composition of the separately collected parotid and submaxillary secretions affords a convenient means of studying exocrine gland function in cystic fibrosis of the pancreas. Deviations in composition could arise from alterations in gland structure (hypertrophy, hyperplasia), function (metabolism, protein synthesis, ion transport, cell permeability), neural regulation, or combinations thereof. Elucidation of the aberrant mechanisms responsible for salivary gland abnormality could provide meaningful clues to the basic pathogenesis of cystic fibrosis. Studies on parotid saliva have yielded conflicting results. Several investigators have reported elevations in sodium, chloride, and flow rate in cystic fibrosis patients.1-5Previous reports from this laboratory on parotid saliva indicated that although "there was a tendency for electrolyte values to be higher in a group of patients with cystic fibrosis of the pancreas, only in the case of inorganic phosphorus, however, was this elevation statistically significant."6Parotid glycoproteins were not found

Salivary studies in cystic fibrosis.

The structure of human labial salivary gland acini was studied by light and electron microscopy. Contrary to previous reports, these glands were pure mucous in nature; no serous elements were present. The acinar cells were found in all stages of maturation. Immature cells were characterized by an extensive and highly organized rough-surfaced endoplasmic reticulum. The Golgi complex was extremely prominent, consisting of stacks of flattened cisternae and swarms of small vesicles. Mucous droplets were almost completely absent. As secretory activity progressed, the endoplasmic reticulum involuted, while the Golgi cisternae became distended and formed many vacuoles. In mature mucous cells, the apical cytoplasm was filled with membrane-bounded mucous droplets, and the nucleus was displaced basally. The droplets frequently showed great variation in density from cell to cell, and even within the same cell they sometimes were quite heterogeneous. They were liberated from the acinar cells by an apocrine process, so that droplets with intact limiting membranes were often observed in the acinar lumen. These droplets soon lysed, their contents fusing into streams of mucus. Occasionally during apocrine secretion a mucous cell failed to reconstitute its apical surface, and its entire contents spilled into the acinar lumen.



Unusual cytoplasmic inclusions were present in many of the acinar cells. These inclusions, which were surrounded by a single membrane, consisted of lipid droplets closely associated with bundles of fine filaments.
