Diabetes mellitus is a chronic disease characterized by hyperglycemia and by complications that include microvascular disease of the eye and kidney and a variety of clinical neuropathies.1 Although diabetes is also associated with premature macrovascular disease, this review is limited to a discussion of retinopathy, nephropathy, and neuropathy. The pathophysiology of these complications of diabetes was recently reviewed in the Journal. 1 The specific association of microvascular disease and neuropathy with diabetes and the relation of the two complications to the duration of diabetes suggest that they are linked to hyperglycemia or a concomitant metabolic abnormality. Prevention and Treatment of the Complications of Diabetes Mellitus

OBJECTIVE:To assess the effectiveness of a pharmaceutical care model on the management of non-insulin-dependent diabetes mellitus (NIDDM) in urban African-American patients.DESIGN:Eligible patients were randomized to either a pharmacist intervention or control group and followed over a 4-month period. Patients in the intervention group received diabetes education, medication counseling, instructions on dietary regulation, exercise, and home blood glucose monitoring, and evaluation and adjustment of their hypoglycemic regimen. Patients in the control group continued to receive standard medical care provided by their physicians.SETTING:A university-affiliated internal medicine outpatient clinic.PARTICIPANTS:The study population consisted of urban African-American patients with NIDDM currently attending the clinic.MAIN OUTCOME MEASURES:Primary outcome measures included fasting plasma glucose and glycated hemoglobin concentrations. Evaluation of a Pharmaceutical Care Model on Diabetes Management

The demonstration that microalbuminuria is predictive of overt diabetic nephropathy has created a demand for the routine measurement of urinary albumin in diabetic patients.' The first part of this report provides a brief overview of microalbuminuria in diabetes mellitus. The second defines assay performance criteria and sample handling conditions for the measurement of urinary albumin and gives recommendations for the standardization of assays and includes an assessment of selected immunochemical methods.

Microalbuminuria in diabetes mellitus : review and recommendations for the measurement of albumin in urine

A radioimmunoassay has been developed for the measurement in urine of retinol-binding protein (alpha 2-microglobulin) and used as an index of renal tubular function in adult Type 1 (insulin-dependent) diabetics and to define reference ranges in non-diabetic controls. There was a significantly greater excretion (P less than 0.001) of retinol-binding protein in the diabetic group compared to the controls in both overnight and daytime samples. There was a weak positive correlation with albumin excretion (r = 0.33; P less than 0.01) but no correlation with HbA1, duration of diabetes or arterial blood pressure. The results indicate that retinol-binding protein excretion may be increased in diabetic subjects without increased albumin excretion. The possibility therefore exists that renal tubular damage may occur early in diabetic nephropathy without apparent glomerular dysfunction.

Retinol Binding Protein as a Small Molecular Weight Marker of Renal Tubular Function in Diabetes Mellitus

We have measured ferritin concentrations in healthy women, and ferritin, C-reactive protein, iron and total iron-binding capacity in patients undergoing hysterectomy or major gastrointestinal surgery. Pre-operative serum ferritin concentrations in patients awaiting hysterectomy were significantly lower than those for patients awaiting gastrointestinal surgery and also lower than those for healthy women of similar age. Healthy women aged between 51 and 60 years had significantly higher ferritin levels than women aged 35-50 years. All patients studied showed large increases in serum ferritin and C-reactive protein concentrations after surgery and approximately similar decreases in iron and in total iron-binding capacity.
