2009 Focused Update Incorporated Into the ACC/AHA 2005 Guidelines for the Diagnosis and Management of Heart Failure in Adults A Report of the American College of Cardiology Foundation/American Heart Association Task Force on Practice Guidelines: Developed in Collaboration With the International Society for Heart and Lung Transplantation

During the past decade, catheter ablation of atrial fibrillation (AF) has evolved rapidly from an investigational procedure to its current status as a commonly performed ablation procedure in many major hospitals throughout the world. Surgical ablation of AF, using either standard or minimally invasive techniques, is also performed in many major hospitals throughout the world.

In 2007, an initial Consensus Statement on Catheter and Surgical AF Ablation was developed as a joint effort of the Heart Rhythm Society, the European Heart Rhythm Association, and the European Cardiac Arrhythmia Society.1 The 2007 document was also developed in collaboration with the Society of Thoracic Surgeons and the American College of Cardiology. Since the publication of the 2007 document, there has been much learned about AF ablation, and the indications for these procedures have changed. Therefore the purpose of this 2012 Consensus Statement is to provide a state-of-the-art review of the field of catheter and surgical ablation of AF and to report the findings of a Task Force, convened by the Heart Rhythm Society, the European Heart Rhythm Association, and the European Cardiac Arrhythmia Society and charged with defining the indications, techniques, and outcomes of this procedure. Included within this document are recommendations pertinent to the design of clinical trials in the field of AF ablation, including definitions relevant to this topic.

This statement summarizes the opinion of the Task Force members based on an extensive literature review as well as their own experience. It is directed to all health care professionals who are involved in the care of patients with AF, particularly those who are undergoing, or are being considered for, catheter or surgical ablation procedures for AF. 2012 HRS/EHRA/ECAS expert consensus statement on catheter and surgical ablation of atrial fibrillation: Recommendations for patient selection, procedural techniques, patient management and follow-up, definitions, endpoints, and research trial design

Purpose Pediatric heart re-transplantation (re-HTx) has been associated with poorer graft survival when compared to first-time pediatric heart transplants (primary-HTx). Identification of patients who are more likely to benefit from re-HTx could aid clinicians in selecting patients for surgery. Methods The United Network for Organ Sharing (UNOS) database was queried to identify patients who underwent re-HTx from 1987 to 2020 and were less than 18-years-old at the time of their primary-HTx. This cohort was propensity matched with a set of primary-HTx patients based on several prognostic indicators. Results There were a total of 9,993 primary-HTx and 782 re-HTx. After matching, median graft survival [months (95% CI)] was longer for primary-HTx [129 (116 - 149)] than it was for re-transplants [109 (92 - 126)] with a hazard ratio [HR (95% CI)] of 1.21 (1.1 - 1.4, p = 0.0083). The difference in primary-HTx and re-HTx median graft survival was no longer statistically significant if the interval between primary-HTx and re-HTx was greater than 12 months (HR 1.15 [1.0 - 1.3], p = 0.0602), if re-HTx took place between the ages of 3 to 13 (HR 1.07 [0.89 - 1.3], p = 0.474), or if there were no episodes of acute rejection within the first year of primary-HTx (HR 0.99 [0.75 - 1.3], p = 0.962). If re-HTx took place within 12 months of primary-HTx, median survival dropped to 15 months (3 - 41) with a HR of 2.32 (1.7 - 3.2). Conclusion Graft survival is significantly shorter following re-HTx when compared to primary-HTx. However, there are multiple patient cohorts in which re-tx survival is equivalent to primary HTx survival (e.g. patients more than one year out from primary-HTx). Conversely, patients requiring a re-HTx within the first year of their primary-HTx have an abysmal graft survival and re-HTx for these patients may not be prudent. Further study to identify cohorts where re-HTx should and should not be pursued is needed. The most critically ill patients can now be treated with life saving mechanical circulatory support. The development of databases to track the outcomes of patients with critical illness has been ongoing for much of the last two decades. Clinicians who engage in the use of mechanical circulatory support are interested in the analysis of outcomes of patients treated with Extracorporeal Membrane Oxygenation (ECMO) and Ventricular Assist Devices (VAD), with the ultimate aim of improvement in the quality of care.
