When to start antiretroviral therapy: the need for an evidence base during early HIV infection
TL;DR: There is currently no evidence from randomized controlled trials to suggest that a strategy of initiating ART when the CD4 count is above 350 cells/μl results in benefit to the HIV+ person and data from observational studies are inconsistent.
Journal ArticleDOI
Effect of immediate initiation of antiretroviral therapy on risk of severe bacterial infections in HIV-positive people with CD4 cell counts of more than 500 cells per μL: secondary outcome results from a randomised controlled trial.
Jemma L O'Connor,Michael J. Vjecha,Andrew N. Phillips,Brian Angus,David A. Cooper,Beatriz Grinsztejn,Gustavo Lopardo,Satyajit Das,Robin Wood,Aimee M. Wilkin,Hartwig Klinker,Pacharee Kantipong,Karin L. Klingman,David Jilich,Elbushra Ali Mohamed Herieka,Eileen Denning,Ibrahim Abubakar,Fred M. Gordin,Fred M. Gordin,Jens D Lundgren +19 more
TL;DR: Immediate ART reduces the risk of several severe bacterial infections in HIV-positive people with high CD4 cell count, partly explained by ART-induced increases in CD4cell count, but not by increases in neutrophil count.
Journal ArticleDOI
Why START? Reflections that led to the conduct of this large long-term strategic HIV trial.
Jens D Lundgren,Abdel Babiker,Fred M. Gordin,Sean Emery,Gerd Faetkenheuer,Jean-Michel Molina,Robin Wood,Robin Wood,James D. Neaton +8 more
TL;DR: This monograph describes a cohort of 4685 HIV-positive persons, most of whom were recently infected, who volunteered to dedicate several years to being participants in a research study that addresses the question of when antiretroviral therapy (ART) should be initiated.
Journal ArticleDOI
Use of Ciprofloxacin for Successful Eradication of Bacteremia Due to Campylobacter cinaedi in a Human Immunodeficiency Virus-Infected Person
TL;DR: A 36-year-old homosexual man who was infected with human immunodeficiency virus presented with a 2-month history of fever and intermittent diarrhea, and this is the first documented case of symptomatic bacteremia due to C. cinaedi that was successfully treated with ciprofloxacin.
Journal ArticleDOI
Mycobacterium szulgai Infection of the Lung: Case Report and Review of an Unusual Pathogen
TL;DR: The authors reviewed the 25 years of clinical experience with this mycobacteria; particular emphasis is on the presentation and treatment of this very unusual infection, and presented a patient who developed M szulgai pulmonary infection 30 years after an episode of pulmonary tuberculosis.
