2013 ESH/ESC Guidelines for the management of arterial hypertension: The Task Force for the management of arterial hypertension of the European Society of Hypertension (ESH) and of the European Society of Cardiology (ESC).

Because of new evidence on several diagnostic and therapeutic aspects of hypertension, the present guidelines differ in many respects from the previous ones. Some of the most important differences are listed below: 
1. Epidemiological data on hypertension and BP control in Europe. 
2. Strengthening of the prognostic value of home blood pressure monitoring (HBPM) and of its role for diagnosis and management of hypertension, next to ambulatory blood pressure monitoring (ABPM).

The NOX Family of ROS-Generating NADPH Oxidases: Physiology and Pathophysiology

The discovery that mammalian cells have the ability to synthesize the free radical nitric oxide (NO) has stimulated an extraordinary impetus for scientific research in all the fields of biology and medicine. Since its early description as an endothelial-derived relaxing factor, NO has emerged as a fundamental signaling device regulating virtually every critical cellular function, as well as a potent mediator of cellular damage in a wide range of conditions. Recent evidence indicates that most of the cytotoxicity attributed to NO is rather due to peroxynitrite, produced from the diffusion-controlled reaction between NO and another free radical, the superoxide anion. Peroxynitrite interacts with lipids, DNA, and proteins via direct oxidative reactions or via indirect, radical-mediated mechanisms. These reactions trigger cellular responses ranging from subtle modulations of cell signaling to overwhelming oxidative injury, committing cells to necrosis or apoptosis. In vivo, peroxynitrite generation represents a crucial pathogenic mechanism in conditions such as stroke, myocardial infarction, chronic heart failure, diabetes, circulatory shock, chronic inflammatory diseases, cancer, and neurodegenerative disorders. Hence, novel pharmacological strategies aimed at removing peroxynitrite might represent powerful therapeutic tools in the future. Evidence supporting these novel roles of NO and peroxynitrite is presented in detail in this review.

WSTĘP Leczenie chirurgiczne migotania przedsionkow (AF) jest rzadko polecaną alternatywą dla leczenia zachowawczego, co jest uzasadnione [1]. Ablacja chirurgiczna, choc jest metodą bardzo skuteczną, ma jednak bardzo inwazyjny charakter i wiąze sie ze stosunkowo wysokim ryzykiem środoperacyjnym [2]. Wiekszośc stosowanych obecnie metod chirurgicznej ablacji lewego przedsionka (LA) wymaga otwarcia klatki piersiowej poprzez sternotomie pośrodkową lub niewielką torakotomie 1lub obustronną [3]. W zaleceniach Europejskiego Towarzystwa Kardiologicznego kardiochirurgiczne procedury ablacji LA mają klase zalecen 2B [4]. W ostatnich kliku latach rozwinely sie kardiochirurgiczne techniki maloinwazyjne, pozwalające na zmniejszenie ryzyka powiklan operacyjnych i oferujące akceptowalne wyniki, rowniez kosmetyczne [5]. Najbardziej zaawansowaną obecnie metodą chirurgicznej ablacji LA jest metoda torakoskopowa. Wykorzystuje ona dostep typu port access [6]. Jej odmianą jest ablacja zarowno lewego, jak i prawego przedsionka. Krakowska kardiochirurgia od 2006 r. stosuje z powodzeniem te wlaśnie metode w wybranych przypadkach, w ktorych zarowno leczenie zachowawcze, jak i ablacja endokardialna nie są wystarczająco skuteczne. Niedoskonalością stosowania metody epikardialnej jest brak mozliwości wykonania szczelnej linii aplikacyjnej wokol zyl plucnych, co wiąze sie z aspektami technicznymi i trudnym dostepem elektrody ablacyjnej do tkanki serca. Dojście epikardialne nie gwarantuje skutecznych aplikacji ponad zylą plucną prawą i lewą gorną oraz ponizej zyly plucnej dolnej prawej. Konsekwencją wykonania niepelnej linii okrązającej zyly plucne mogą byc nowe formy arytmii wymagające dalszych ablacji. Wiadomo rowniez, ze skutecznośc ablacji przezskornych u pacjentow z przetrwalym AF jest ograniczona [7]. W tej grupie chorych ze wzgledu na zaawansowany remodeling przedsionkow sama izolacja zyl plucnych [8] nie jest wystarczająca i wymaga uzupelnienia wykonaniem dodatkowych linii ablacyjnych w przedsionkach. Dlatego tez powstala koncepcja polączenia uzupelniających sie typow procedur, czyli ablacji endokardialnej i epikardialnej. Wychodząc z tego zalozenia, w ośrodku w ktorym pracują autorzy niniejszej pracy, podjeto inicjatywe ablacji hybrydowych przeprowadzanych sekwencyjnie obiema metodami. Okazalo sie, ze do tego typu procedur konieczne jest wykorzystanie sali hybrydowej, przeznaczonej do wykonywania procedur z pogranicza kardiologii i kardiochirurgii. Pierwsze wspolne procedury ablacji epikardialnej i endokardialnej przeprowadzono 15 stycznia 2009 r. Zespol kardiochirurgow wykonal epikardialną ablacje torakoskopową, natomiast zespol kardiologow ablacje endokardialną.

[Hybrid ablation of the left atrium on beating-heart - initial experience].

This report describes a 52 year-old women with an inferior myocardial infarction due to a coronary artery embolus. Coronary angiography revealed occlusion of right coronary artery and echocardiographic findings showed a large left intraatrial tumour. The tumour was removed surgically and pathological findings confirmed the diagnosis of myxoma. Aetiology of the occlusion was closely related to the left atrial tumour. Myocardial infarction was the first manifestation of left atrial myxoma.

Acute myocardial infarction due to coronary embolisation as the first manifestation of left atrial myxoma

Evaluation of carotid intima-media thickness in patients with aortic valve calcifications.

The patient is a 28 year old woman who received a heart transplant in 1992 secondary to hypertrophic cardiomyopathy with unremarkable post-operative course. In the period immediately post transplantation the patient was on a four-drug immunosuppressive regimen which was subsequently changed to standard three-agent therapy. This therapy was continued until the patient became pregnant. In the first trimester only Cyclosporine (CsA) was used, and thereafter, the patient was continued on the previous three agent regimen. Toward the end of pregnancy a rise in systolic pressure was observed, but the child was delivered by spontaneous vaginal delivery without complications in the 38th week of pregnancy. The newborn weighed 3320 g and was in good health. A sharp fall in the newborn CsA blood levels was observed post delivery reaching zero level on the third day of life. At the present time, both mother and baby are in good health, 6 weeks after delivery.

Introduction Renalase may degrade catecholamines and regulate sympathetic tone and blood pressure. The aim of this study was to assess dopamine, norepinephrine, and renalase in 80 heart transplant recipients and 22 healthy volunteers and their correlations with heart rate, blood pressure control, type of hypotensive therapy, and renal function. Patients and Methods Renalase, dopamine, and norepinephrine were studied by using commercially available assays. Results Renalase levels were higher in heart transplant recipients compared with healthy volunteers, and noradrenaline levels were lower in the studied cohort patients than in the healthy volunteers. Noradrenaline was correlated with white blood cell count ( r  = −0.21, P r  = 0.41, P r  = −0.29, P r  = −0.22, P r  = 0.58, P r  = −0.31, P 90 mm Hg) compared with those with normal diastolic blood pressure ( P Conclusions Elevated renalase levels in heart transplant patients were related to kidney function but not linked to the sympathetic nervous system activity in this study population. In heart transplant recipients, these findings might suggest that sympathetic denervation and the modulation of β-receptors persist.
