Abstract: Objective To assess the association of HLA DRB1 genes with pulmonary tuberculosis (PTB) and the correlation between HLA DRB1 alleles and clinical manifestations of patients with PTB. Method Alleles at the HLA DRB1 typing were analyzed by polymerase chain reaction with sequence specific primers (PCR SSP) in 74 patients and 90 healthy controls. Results Compared with controls,a significant increased frequency of DRB1*15 was found (34 3% vs 17 0%, Pc 0 05,RR=2 91) The frequency of DRB1*12 was also increased,but the difference was not statistically significant(15 4% vs 7 5%, Pc 0 05) The frequency of DRB1*11 was significantly decreased (1 4% vs 9 9%, Pc 0 05,RR=0 12) More retreated and drug resistant cases were found in DRB1*15 positive patients than in DRB1*15 negative patients. Conclusions These results show that HLA DRB1*15 allele is susceptible gene in PTB,DRB1*11 may be a protective allele,HLA DRB1*15 allele may be related to patients with refractory PTB

Abstract: OBJECTIVE To investigate the expression of the lung resistance-related protein (LRP) in non-small cell lung carcinomas (NSCLC) and evaluate its clinical significance. METHODS Using immunohistochemistry, LRP was examined in 69 NSCLC specimens obtained by either transbronchial biopsy via fibrobronchoscopy or transcutaneous needle biopsy or surgical resection. Of the 69 patients from whom tumor specimens were obtained, 52 were male and 17 were female. RESULTS LRP expression was detected in 39/69 (57%) tumor specimens. There was no correlation between LRP expression and NSCLC histological classification (P > 0.05). No significant differences were found in gender and among age groups. The positive rates of LRP were 67% (18/27), 55% (17/31), 52% (23/44), 64% (16/25), 83% (5/6), 57% (36/63) and 50% (3/6) in adenocarcinoma, squamous, staging T1-2, T3-4, N3, M0 and M1 respectively. It was shown that LRP expression was not associated with primary tumor size and metastasis status (N,M). NSCLC patients with positive LRP expression responded more poorly to chemotherapy than those with negative LRP expression (P < 0.05). CONCLUSIONS The frequencies of multidrug resistance caused by LRP are similar between adenocarcinoma and squamous. Expression of LRP is related to multidrug resistance of NSCLC, which in turn related to the efficacy and prognosis of chemotherapy. The examination of LRP expression may be valuable for choice of chemotherapy regimen.

Abstract: BACKGROUND To obtain human papillomavirus type 16(HPV 16) major capsid protein L1 with baculovirus expression system. METHODS Using pFb1 as a vector,a recombinant baculovirus expressing plasmid,which contains HPV16L1 gene sequence,was generated. The constructed virus was infected into an insect cell line Sf9. RESULTS After incubating at 27 degrees for 72 hours, the infected cells were collected and total cellular poteins were extracted. SDS-PAGE assay revealed a roughly 56 000 expressed protein and Western blot confirmed that the expressed protein arose from HPV16L1. CONCLUSIONS HPV16 later protein L1 could be efficiently expressed with baculovirus expression system,and the expressed L1 protein remains to have good immunoreactivity. This study may supply a basic work for preparing virus-like particle and prophylactic HPV subunit vaccines.

Abstract: Objective To investigate the distribution of diffuse and local retinal-nerve-fiber-layer defects (RNFLD) and their clinical features in early primary open-angle glaucoma (POAG) and normal-tension glaucoma (NTG). Method The types of RNFLD in 81 cases with POAG and 70 cases with NTG were observed, the distribution of diffuse and local RNFLD were analyzed, and the maximum untreated intraocular pressure (IOP) and the history of optic disc hemorrhage between the two types of RNFLD in the two glaucomatous groups were compared. Results RNFLD appeared in 78 of 81 cases with POAG, 50 of them were diffuse and 28 were local; 27 of diffuse and 43 of local RNFLD were found in 70 cases with NTG. The IOP was (28.78 +/- 5.41) mm Hg in the diffuse and (26.2 +/- 5.2) mm Hg in the local type in POAG (t = 2.04, P 0.05), and neither were in the local type (chi(2) = 1.31, P > 0.05). Conclusion The diffuse RNFLD tends to be distributed in POAG, while the local RNFLD tends to be distributed in NTG. The maximum IOP in the eye with diffuse RNFTD is higher than that in the eye with local RNFLD, either in POAG or NTG group. The occurrence of disc hemorrhage is higher in the eye with local RNFLD than that in the eye with diffuse RNFLD in the two groups respectively. There are differences in clinical features between the two types of RNFLD in early POAG and NTG.

Abstract: OBJECTIVE To bring forward a suggestion about clinical diagnostic standard and clinical typing for chronic and severe hepatitis METHODS To make a comprehensive study on clinical features and pathology of 895 cases of severe and chronic hepatitis, liver cirrhosis after hepatitis based on the viral prevention and control plan laid down in 1995 RESULTS The chronic hepatitis can still be divided into mild, moderate and severe types clinically, but the PTA should be changed for normol-71, 70-61, 60-51, the A/G value for normal, 15-13, 12-10 respectively ALT, BIL, alpha-globulin are kept unchanged The albumin value can be cut out from the reference indexes of clinical typing for chronic hepatitis Acute severe hepatitis can be divided into early stage (taking edema as the main type) and late stage (taking necrosis as the main type); subacute severe hepatitis can be divided into ascite type, coma type and mixed type; if those lacking of coma and ascite with PTA about 60% 50% can be treated as earlier stage Subacute and chronic severe hepatitis still can be divided into early, middle and late stages The disease course of subacute severe hepatitis may prolong to six months Chronic severe hepatitis can be divided into type B (typical chronic hepatitis type) type C (liver cirrhosis type) and type c (acute liver failure type developed from chronic hepatitis and viral carriers) CONCLUSIONS The original procedure of 1995 are feasible on the whole

Abstract: OBJECTIVE To evaluate the values of pulmonary function assay and dynamic pulmonary perfusion imaging with technetium labeled macroaggregated albumin ((99m)TcMAA) in the early diagnosis of hepatopulmonary syndrome (HPS). METHODS The pulmonary function assay and (99m)TcMAA scans were performed in 28 patients with HPS, 30 cirrhotic patients (CP) without HPS, and 21 healthy controls (HC). RESULTS In the patients with HPS, PaO(2), SaO(2) and diffusion capacity for carbon monoxide of lungs (DLco) was significantly lower than that in CP (P<0.01) and HC (P<0.01), and alveolar-arterial gradient [P((A-a))O(2)] was significantly increased (P<0.001). Results from (99m)TcMAA scans showed that the radionuclides were distributed over the spleen, kidney, liver and brain, and the ratios of arterivenous shunt were significantly higher than that in CP (P<0.001) and NC (P<0.001). In cirrhotic patients, DLco significantly decreased (P<0.05), P((A-a))O(2) and shunt ratios increased (P<0.01 and 0.001). CONCLUSIONS Pulmonary function assay and dynamic pulmonary perfusion imaging with (99m)TcMAA are sensitive methods for diagnosis of the early HPS.

Abstract: Objective Cerebral infarction, a common acute cereovascular disease, is one of the main complications of type 2 diabetes mellitus (DM). This paper is to explore the risk factors of cerebral infarction among type 2 DM patients. Methods A pair matched case control study was carried out. Data was analyzed with conditional logistic regression. Results One hundred and ten type 2 DM patients with cerebral infarction and controls were selected respectively. Results showed that higher educational level and physical exercises lowered the risk of cerebral infarction among type 2 DM patients. Higher HDL C and blood sugar under control could also lower the risk of developing cerebral infarction among type 2 DM patients. The above factors seemed to have protective functions. Excessive salt intake and snoring increased the risks of cerebral infarction development among type 2 DM patients. Hypertension served as an independent risk factor while atrial fibrillation and hyperinsulinemia were important risk factors. High LDL C, Apo A , Lp(a), Fg increased the risk of cerebral infarction among type 2 DM patients. The study did not discover the relationship of smoking, drinking, oral contraception, menstrual and obstetrical history, TC, TG, Apo A and cerebral infarction among type 2 DM patients. Conclusion To prevent cerebral infarction among type 2 DM, measures need to be taken as control of salt intake; eating more vegetables; regular physical exercises; control of blood sugar, blood pressure and Hyperinsulinemia.

Abstract: OBJECTIVE To observe and evaluate the effect and safety of pasinizid on treatment of the senile pulmonary tuberculosis with bacteriological positive. METHODS 115 cases of the senile pulmonary tuberculosis with bacteriological positive were divided into therapy group(group A) and control group(group B1 and B2) randomly. Chemotherapy regimen: group A were treated by 3DZL2E/5DL2E mainly composed of pasinizid; group B1 were treated by 3HPZL2/5HL2E(abbreviated HP regimen); group B2 were treated by 3DZL2E/5DL2E(abbreviated SH regimen). RESULTS When the chemotherapy was over, the sputum negative conversion rates were 92%, 72%, 71% in group A, group B1 and group B2 respectively. The conversion rate in group A was significantly higher than that in group B1 and group B2 (P < 0.05); focal absorption rates were 75%, 50%, 50% respectively, while the vanishing rates of cavity were 85%, 46%, 50% respectively. The rate in group A was significantly higher than that in group B1 and B2(P < 0.05); Rates of side effects were 17%, 58%, 18% respectively. The rates in group A and group B2 were apparently lower than that in group B1(P < 0.01). Bacteriological relapse rate of 2 years were 5%, 19%, 18%, respectively. The rate in group A was lower than that in group B1 and B2 (P < 0.01). CONCLUSIONS The effect of pasinizid on senile retreated sputum positive pulmonary tuberculosis was better than HP and SH regimen. Due to its lower rates of side effects, it is recommended for clinical usage.

Abstract: Objective To investigate the changes of telomerase activity and its related genes expressi on in leukemia cells and their roles in leukemogenesis. Methods T he telomerase activity in bone marrow cells of 47 leukemia patients were detecte d by a semi-quantitative telomeric repeat amplification protocol (TRAP)-silver staining, and the expression of telomerase ralated genes hEST2, TP1 and hTR by semi-quantitative reverse transcriptase polymerase chain reaction (RT-PCR). [ WTHZ Results The telomerase activity and expression of telomerase rela ted genes were significantly higher in 42 leukemia patients in progress stage th an that in complete remission/chronic stage and in normal/anemia cases (P[ WTBZ]<0.01).The telomerase activity and expression of TP1,hTR gene in 14 cases of acute leuk emia complete remission (AL-CR)/chronic myelogenous (CML)-chronic stage were h igher than that in normal/anemia cases (P<0.05). Low levels of telom erase activi ty and telomerase related genes expression could be detected in part of normal b one marrow cells. The levels of telomerase activity and telomerase related genes expression in 3 cases of anemia increased after short-term culture. Conc lusions High level of telomerase activity and its related genes express ion were specific markers of malignant hematopoietic cells and their changes mig ht be useful for observation of treatment effectiveness in leukemia. Low level of telomerase activity was associated with hematopoietic stem cells with the abi lity of proliferation. hEST2 gene might be a up-regulation gene of telomerase; hTR might be related to feedback regulation of telomerase.
