2013 ACC/AHA Guideline on the Assessment of Cardiovascular Risk

Guidelines and Expert Consensus Documents summarize and evaluate all available evidence with the aim of assisting physicians in selecting the best management strategy for an individual patient suffering from a given condition, taking into account the impact on outcome and the risk–benefit ratio of diagnostic or therapeutic means. Guidelines are no substitutes for textbooks and their legal implications have been discussed previously. Guidelines and recommendations should help physicians to make decisions in their daily practice. However, the ultimate judgement regarding the care of an individual patient must be made by his/her responsible physician(s).

The recommendations for formulating and issuing ESC Guidelines and Expert Consensus Documents can be found on the ESC website (http://www.escardio.org/knowledge/guidelines/rules).

Members of this Task Force were selected by the European Society of Cardiology (ESC) and the European Association for Cardio-Thoracic Surgery (EACTS) to represent all physicians involved with the medical and surgical care of patients with coronary artery disease (CAD). A critical evaluation of diagnostic and therapeutic procedures is performed including assessment of the risk–benefit ratio. Estimates of expected health outcomes for society are included, where data exist. The level of evidence and the strength of recommendation of particular treatment options are weighed and graded according to predefined scales, as outlined in Tables 1 and 2 .

The members of the Task Force have provided disclosure statements of all relationships that might be perceived as real or potential sources of conflicts of interest. ACC/AHA 2005 Practice Guidelines for the Management of Patients With Peripheral Arterial Disease (Lower Extremity, Renal, Mesenteric, and Abdominal Aortic)

L'angiographie par resonance magnetique permet une evaluation non invasive de la bifurcation carotidienne. La sensibilite et la specificite sont excellentes par rapport a l'angiographie classique. En revanche, la precision de l'angiographie par resonance magnetique est limitee comparee au « gold standard » de l'angiographie classique. Nous presentons les principes de l'angiographie par resonance magnetique et les resultats d'une etude ou le degre de stenose, determine sur la piece d'endarteriectomie, est le temoin reel de la maladie carotidienne. Le bilan preoperatoire chez les patients programmes pour une endarteriectomie a comporte une angiographie par resonance magnetique, une angiographie classique et une echo-Doppler. Le degre de stenose determine par chaque methode a ete compare a celui mesure sur la piece operatoire. Les resultats de cette analyse montrent la plus grande precision de l'angiographie par resonance magnetique et l'echo-Doppler. Nous presentons une discussion sur l'utilisation de la resonance magnetique et les methodes ultrasonographiques pour l'evaluation des plaques de la bifurcation carotidienne. Ces differentes methodes permettent une differenciation des composants de la plaque. Ces resultats suggerent que l'angiographie par resonance magnetique, l'echo-Doppler, ou une combinaison des deux permet une evaluation precise des lesions de la bifurcation carotidienne.

Evaluation of disease of the carotid bifurcation using magnetic resonance imaging

We have used a multimodality imaging approach to assess the inflammatory component of a growing iliac artery aneurysm in a 53-year-old man who presented with related ureteral obstruction. Research suggests that episodic and heterogeneous inflammatory processes are important for the progression of aneurysms. The combined PET and MRI evaluation of inflammation that we present here is a novel approach to vascular imaging that is well suited for emerging hybrid PET/MRI systems.
