Motivational Interviewing: Preparing People for Change

The widely disseminated clinical method of motivational interviewing (MI) arose through a convergence of science and practice. Beyond a large base of clinical trials, advances have been made toward "looking under the hood" of MI to understand the underlying mechanisms by which it affects behavior change. Such specification of outcome-relevant aspects of practice is vital to theory development and can inform both treatment delivery and clinical training. An emergent theory of MI is proposed that emphasizes two specific active components: a relational component focused on empathy and the interpersonal spirit of MI, and a technical component involving the differential evocation and reinforcement of client change talk. A resulting causal chain model links therapist training, therapist and client responses during treatment sessions, and posttreatment outcomes.

Motivational Interviewing in Health Care: Helping Patients Change Behavior

The Evaluating Methods for Motivational Enhancement Education trial evaluated methods for learning motivational interviewing (MI). Licensed substance abuse professionals (N = 140) were randomized to 5 training conditions: (a) clinical workshop only; (b) workshop plus practice feedback; (c) workshop plus individual coaching sessions; (d) workshop, feedback, and coaching; or (e) a waiting list control group of self-guided training. Audiotaped practice samples were analyzed at baseline, posttraining, and 4, 8, and 12 months later. Relative to controls, the 4 trained groups showed larger gains in proficiency. Coaching and/or feedback also increased posttraining proficiency. After delayed training, the waiting list group showed modest gains in proficiency. Posttraining proficiency was generally well maintained throughout follow-up. Clinician self-reports of MI skillfulness were unrelated to proficiency levels in observed practice.

Fur die Untersuchungen der Wirkungsweise von MI bedarf es reliabler Instrumente. Das MI-SCOPE ermoglicht die Kodierung transkribierter MI-Interaktionen. Die Interraterreliabilitat liegt bei к=0,67. Ziel dieser Studie ist die Uberprufung der Reliabilitat der deutschen Ubersetzung des MI-SCOPE. Die Kodierung erfolgt im ersten Schritt (Gliedern) uber die Identifizierung sprachlicher Auserungen, denen im zweiten Schritte eine von 49 Kategorien MI-relevanten Verhaltens zugeordnet werden. Datengrundlage bildet eine Zufallsauswahl von jeweils 20% aus 78 Sitzungen mit jungen Muttern, die vor oder wahrend der Schwangerschaft geraucht hatten. 3 Kodierer kategorisierten paarweise unabhangig voneinander die Gesprache. Die Ubereinstimmungen beim Gliedern lagen zwischen к=0,813 und к=0,951 und unterschieden sich zwischen den 3 Kodiererkombinationen nicht (Kruskal-Wallis χ 2 =0,704, df=2, p=0,703). Die Ubereinstimmungen beim Kodieren lagen zwischen к=0,590 und к=0,788 und unterschieden sich zwischen den 3 Kodiererkombinationen nicht (Kruskal-Wallis χ 2 =3,594, df=2, p=0,166). Mit dem MI-SCOPE;D liegt ein reliables Kodiersystem zur Untersuchung von MI-Interventionen vor.

This article seeks to articulate how neoliberal thinking perceives—and conflates—the homeless and mad subject in opposition to the middle class “productive” citizen. I engage in a narrative analysis of two cultural artifacts: James Burns’s documentary film, Out of Sight, Out of Mind: Mental Healthcare in Rural America and CTV’s coverage of the Housing First approach as a viable response to homelessness. Using an analysis of neoliberal rationality coupled with a Critical Mad Studies approach, I examine the ways documentary filmmaking engenders particular ways of witnessing the housing crisis. The housing crisis is a direct result of the rise of neoliberal restructuring, yet homelessness is often presented as an individualized failure. Documentary filmmaking can exacerbate these (mis)understandings, even while particular voices offer powerful cultural critique in the form of counternarratives. Counterhegemonic representations of dehousing and madness must emerge from within the consumer/survivor/ex- patient community if they are to offer a reflexive, nuanced, and decisive departure from middle class orientations of health and recovery. I provide recommendations for producers and consumers of media representations of the housing crisis. Film based representation must be grounded in authentic personal encounters, attention to structural violence, and the desire to precipitate critical dialogue. By doing so filmmakers can elicit a response to homelessness that enhances political solidarity, relies on peer-led research, and confronts neoliberal thinking in the contemporary media landscape.
