The rapid technological developments of the past decade and the changes in echocardiographic practice brought about by these developments have resulted in the need for updated recommendations to the previously published guidelines for cardiac chamber quantification, which was the goal of the joint writing group assembled by the American Society of Echocardiography and the European Association of Cardiovascular Imaging. This document provides updated normal values for all four cardiac chambers, including three-dimensional echocardiography and myocardial deformation, when possible, on the basis of considerably larger numbers of normal subjects, compiled from multiple databases. In addition, this document attempts to eliminate several minor discrepancies that existed between previously published guidelines.

Recommendations for cardiac chamber quantification by echocardiography in adults: an update from the American Society of Echocardiography and the European Association of Cardiovascular Imaging.

Nach der Etablierung der chirurgischen Revaskularisation ohne Verwendung der Herz-Lungen-Maschine (HLM) zur Behandlung der koronaren Mehrgefaserkrankung konnte in vielen prospektiv-randomisierten Studien sowie in Metaanalysen zumeist kein signifikanter Uberlebensvorteil der Off-Pump-Chirurgie („off-pump coronary artery bypass“ [OPCAB]) gegenuber der konventionellen Bypasschirurgie (CCAB) mit Einsatz der HLM und kardioplegischem Herzstillstand belegt werden. Daneben waren insbesondere die perioperative Myokardinfarkt- und Schlaganfallrate fur beide Verfahren vergleichbar. Diesen Untersuchungen gemeinsam war, dass sie nahezu ausnahmslos an Patienten mit vergleichsweise geringem perioperativen Risiko oder an Mischpopulationen vorgenommen wurden. In den letzten Jahren wird nun auch zunehmend auf den Stellenwert der Beating-Heart-Chirurgie bei unterschiedlichen Risikopopulationen wie Patienten mit schlechter kardialer Pumpfunktion, hohem Alter, vorbestehender Niereninsuffizienz, akutem Myokardinfarkt oder Hauptstammstenose fokussiert. Fur die einzelnen Subpopulationen mit spezifischen extrakardialen und kardialen Risiken liegt heute eine Vielzahl von mono- und multizentrischen Studien sowie Metaanalysen vor, die allerdings aufgrund ihres meist nichtrandomisierten Designs die Kriterien einer Level-AEvidenz nicht erreichen. Dennoch erlauben sie es, an Patienten mit spezifischen Operationsrisiken eine Zwischenbilanz vorzunehmen. So zeigte sich, dass insbesondere Multirisikopatienten und Patienten mit praoperativ eingeschrankter kardialer Pumpfunktion bei Anwendung der OPCAB-Chirurgie eine signifikant geringere perioperative Mortalitat und Morbiditat aufwiesen. Zudem war die Inzidenz perioperativer Schlaganfalle fur die meisten Subpopulationen mit extrakardialen Risikofaktoren reduziert. Fur Patienten mit praoperativ bestehender pulmonaler oder renaler Funktionsstorung war die Haufigkeit einer entsprechenden postoperativen Organkomplikation signifikant verringert. Bei nahezu allen Risikogruppen war die Transfusionspflichtigkeit reduziert. Fur keine der Risikopopulationen zeigte die Beating-Heart-Strategie ein im Vergleich zur CCAB-Chirurgie erhohtes Risiko. Dies belegt den hohen Stellenwert der OPCAB-Chirurgie insbesondere fur Patienten mit einem uberdurchschnittlich erhohten perioperativen Risiko.

Management of increased systemic flow requirements in patients with left ventricular assist devices.

Because the first generation of pulsatile-flow devices was primarily used to bridge the sickest patients to transplantation (bridge-to-transplant therapy), the current generation of continuous-flow ventricular assist devices qualifies for destination therapy for patients with advanced heart failure who are ineligible for transplantation. The first-generation devices were associated with frequent adverse events, limited mechanical durability, and patient discomfort due device size. In contrast, second-generation continuous-flow devices are smaller, more quiet, and durable, thus resulting in less complications and significantly improved survival rates. Heart transplantation remains an option for a limited number of patients only, and this fact has also triggered the discussion about the optimal timing for device implantation. The increasing use of continuous-flow devices has resulted in new challenges, such as adverse events during long-term support, and high hospital readmission rates. In addition, there are a number of device-related complications including mechanical problems such as device thrombosis, percutaneous driveline damage, as well as conditions such as hemolysis, infection, and cerebrovascular accidents. This review provides an overview of the evolution of mechanical circulatory support systems from bridge to transplantation to destination therapy including technological advances and clinical improvements in long-term patient survival and quality of life. In addition, recent changes in device implant strategies and current trials are reviewed and discussed. A brief glimpse into the future of mechanical circulatory support therapy will summarize the innovations that may soon enter clinical practice.

Transatlantic editorial: The use of multiple arterial grafts for coronary revascularization in Europe and North America

Circulatory efficiency reflects the ratio between total left ventricular work and the work required for maintaining cardiovascular circulation. The effect of severe aortic valve stenosis (AS) and aortic valve replacement (AVR) on left ventricular/circulatory mechanical power and efficiency is not yet fully understood. We aimed to quantify left ventricular (LV) efficiency in patients with severe AS before and after surgical AVR. Circulatory efficiency was computed from cardiovascular magnetic resonance (CMR) imaging derived volumetric data, echocardiographic and clinical data in patients with severe AS (n = 41) before and 4 months after AVR and in age and sex-matched healthy subjects (n = 10). In patients with AS circulatory efficiency was significantly decreased compared to healthy subjects (9 ± 3% vs 12 ± 2%; p = 0.004). There were significant negative correlations between circulatory efficiency and LV myocardial mass (r = − 0.591, p < 0.001), myocardial fibrosis volume (r = − 0.427, p = 0.015), end systolic volume (r = − 0.609, p < 0.001) and NT-proBNP (r = − 0.444, p = 0.009) and significant positive correlation between circulatory efficiency and LV ejection fraction (r = 0.704, p < 0.001). After AVR, circulatory efficiency increased significantly in the total cohort (9 ± 3 vs 13 ± 5%; p < 0.001). However, in 10/41 (24%) patients, circulatory efficiency remained below 10% after AVR and, thus, did not restore to normal values. These patients also showed less reduction in myocardial fibrosis volume compared to patients with restored circulatory efficiency after AVR. In our cohort, circulatory efficiency is reduced in patients with severe AS. In 76% of cases, AVR leads to normalization of circulatory efficiency. However, in 24% of patients, circulatory efficiency remained below normal values even after successful AVR. In these patients also less regression of myocardial fibrosis volume was seen.
