01 Apr 2019TL;DR: This review will focus on how RDW was associated with mortality and severity of illness in the recent literature, as an independent prognosis factor and as a component part in different predictive and severity scores.
Abstract: Abstract Red blood cell distribution width (RDW) is a hematological parameter usually measured with every complete blood count. Its place in daily practice is mainly in the differential diagnosis of anemia, but nowadays, researchers are focused on different approaches for the erythrocyte’s changes in function and morphology. Sepsis and its most advanced form, septic shock, induces profound disturbances into organ system’s function and morphology. The red blood cells physiology and structure are directly and indirectly altered by these im balances produced in sepsis. RDW was studied in many diseases, like acute heart failure, acute stroke, inflammatory bowel diseases, chronic lung diseases and cancer, but also in sepsis. Its changes are seen to be mainly associated with prognosis. Higher values of RDW are correlated with mortality and severity of illnes in septic and all-cause critically ill patients. RDW was studied also as an independent variable in different predictive scores and some studies suggest it should be introduced in the scores use on a daily basis in critical care settings and emergency departments. In this review we will focus on how RDW was associated with mortality and severity of illness in the recent literature, as an independent prognosis factor and as a component part in different predictive and severity scores.

19 Jan 2020TL;DR: Although both radial head fractures were comminuted, with different treatment options, for one of them open reduction and internal fixation was performed, while for the other one the option was replacement of radial head with a prosthesis.
Abstract: Considered to be a rare and very disabling condition, bilateral terrible triad of the elbow is a fracture-dislocation, which consists in a fracture of the radial head and also of the coronoid process of the ulna, all of this associated with a posterior displacement of the elbow and high instability. It was named "terrible triad" because of the poor outcome of bone healing and range of motion, both medium and long term. What we present is a special case, first because it was not a unilateral terrible triad injury but bilateral, and because the treatment was different for each elbow, which was interesting for comparing the outcome. Although both radial head fractures were comminuted, with different treatment options, for one of them open reduction and internal fixation was performed, while for the other one our option was replacement of radial head with a prosthesis. For both elbows we obtained good functional outcome at 1-year follow up.

TL;DR: Capsule endoscopy small bowel evaluation is a reliable, non-invasive and safe with many advantages and minimum risks, with a proper selection of patients, and can be used as first line investigation.
Abstract: Despite the extensive recent development of different techniques for endoscopic evaluation for both diagnostic and therapeutic reasons, the small bowel remains quite difficult to visualize Capsule endoscopy and device assisted enteroscopy are presently considered the best diagnostic tools for examination of small bowel disorders, assessing diverse pathologies such as obscure gastrointestinal bleeding, iron deficiency anemia, Crohn disease, small bowel tumors and polyposis syndromes Like any other imagistic method, it has specific indications, and contraindications, and possibly it is more important to consider limitations In order to obtain a better result, it is necessary to respect the procedural quality indicators Among them the use of prokinetics - diverse pharmacological substances increasing the success rate of capsule endoscopy have raised debates Capsule endoscopy small bowel evaluation is a reliable, non-invasive and safe with many advantages and minimum risks, with a proper selection of patients, and can be used as first line investigation

Abstract: We present a case of a 14-year-old male athlete to demonstrate the potential usefulness of three-dimensional echocardiography in supplementing magnetic resonance angiography in the assessment of actual anatomy in patients with anomalous origin of right coronary artery.
Anomalous origin of the right coronary artery (RCA) from the left Valsalva sinus with an inter-arterial course between the great arteries is a …

TL;DR: Right upper abdominal resections seem to be feasible and effective in order to maximize the debulking effort with acceptable risks arising from perioperative complications.
Abstract: Background/aim The right upper abdominal involvement is frequently encountered in patients with advanced stage ovarian cancer. The aim of this paper is to study the safety and efficacy of extended resections at this level as well as to determine the sites of residual disease. Patients and methods Between January 2016 and December 2019, 26 patients submitted to right upper abdominal resections were identified. Results Peritoneal stripping and full thickness resections were the most commonly performed resections (in 57% and 19% of cases, respectively), followed by capsular liver resection and atypical liver resection (in 30% and 23% of cases, respectively) while the most common sites where resection was incomplete were the liver pedicle and porta hepatis. Exceptionally, one case necessitated performing a pancreatoduodenectomy as part of debulking surgery. Postoperatively, two cases developed serious complications and required reintervention; however, the overall mortality was null. Conclusion Right upper abdominal resections seem to be feasible and effective in order to maximize the debulking effort with acceptable risks arising from perioperative complications.
