Abstract: Cytomegalovirus (CMV), similar to other members of the Herpesviridae family, can establish both persistent and latent infections. Each of the CMVs that are found in many animal species replicates in the salivary gland, and oral secretion represents a source of horizontal transmission. Locally restricted replication characterizes the immunocompetent individual, whereas in the immunocompromised host, protean disease manifestations occur due to virus dissemination. The virus is cleared by immune surveillance, and CD8+ T lymphocytes play a major role. Remarkably, certain cell types of salivary gland tissues are exempt from CD8+ T-lymphocyte control of murine CMV infection and require the activity of CD4+ T lymphocytes. The results presented here suggest that this activity is a function of Th1 cells. Neutralization of endogenous gamma interferon abrogated the antiviral activity of Th1 cells but not that of CD8+ T lymphocytes in other tissues. Neutralization of endogenous gamma interferon did not interfere with the induction of the cellular and humoral immune response but acted during the effector phase. Recombinant gamma interferon could not replace the function of Th1 cells in vivo and had limited direct antiviral activity in vitro. The results therefore suggest that gamma interferon represents one, but not the only, essential factor involved in salivary gland clearance, establishment of CMV latency, and, eventually, the control of horizontal transmission.

Abstract: Deviation of the major apical foramen from the anatomical root apex is a recognized phenomenon. To determine the frequency, position and mean distance of the major apical foramen from the anatomical root apex, 230 roots of permanent teeth were examined stereomicroscopically and radiographically. Radiographic analysis was used to establish how accurate the conventional radiograph was in displaying such a deviation. The frequency of deviation of the major foramen, determined stereomicroscopically, was 76%, and depended on the type of teeth examined. Radiographic analysis of the same sample revealed 57% of root canals had asymmetry of the root canal foramen. Agreement of stereomicroscopic and radiographic findings was found to be 61%. The most frequent deviation of the major foramen was on the distal root surface (29%), but this was not statistically significant. The mean distance between the deviation of the major foramen and the anatomical root apex was 0.99 mm. The study indicates that the clinician should consider deviation of the foramen during root canal treatment, as the deviation could not easily be detected radiographically.

Abstract: A sixty-eight-year-old woman with a clinical diagnosis of pulmonary embolism and large right-heart embolus on echocardiography is presented. Because of the critical clinical condition of the patient and the inability to perform surgical intervention, thrombolytic treatment (streptokinase) was administered. The patient's condition improved, the right-heart embolus disappeared, and on the perfusion lung scan there was only one small perfusion defect in the right lung. The authors consider thrombolysis to be appropriate treatment for right-heart thromboembolism whenever surgical embolectomy is not possible.

Abstract: A patient, young fisherman, with a locked-in syndrome is reported, in whom intact consciousness, quadriplegia of spastic type, voluntary eye blinking, (de)sursumvergence and anarthria were observed. Thrombosis of the basilar artery and slightly disturbed bioelectrogenesis of the cerebral cortex were proved by clinical examination. The patient died after 41 days. At the autopsy thrombosis a. basilaris and ventrobasal pontine infarction were confirmed. Differential diagnosis of this and similar syndromes has been discussed.
