Abstract
Background The early course of illness in first-episode schizophrenia was examined with special emphasis on the duration of untreated psychosis and pathways to care. Method The consecutively admitted individuals (n = 34) were assessed on premorbid functioning, duration of untreated psychosis, global functioning, symptoms and social network. To clarify the obstacles for receiving earlier treatment, 17 case histories with long duration of untreated psychosis were intensively studied. Results The duration of untreated psychosis was on average very long (130 weeks), the median value was 54 weeks. The long duration of untreated psychosis group (> 54 weeks) had greater deterioration in the premorbid phase, a weaker social network and were more withdrawn than the short duration of untreated psychosis group ( Conclusions In order to identify people earlier, a system of detection must be mobile, easily accessible and attentive to early symptoms of psychosis. It seems to be important to educate the social network related to the individual about the importance of early treatment.

Relationship Between Duration of Untreated Psychosis and Outcome in First-Episode Schizophrenia: A Critical Review and Meta-Analysis

TL;DR: Shorter duration of untreated psychosis was associated with greater response to antipsychotic treatment, as measured by severity of global psychopathology, positive symptoms, negative symptoms, and functional outcomes.

TL;DR: Evidence thus far suggests that DUP may be related to ease of reducing psychotic symptoms once treatment begins for first episode patients, but there is no evidence of a relationship to likelihood of relapse.

Early Intervention for Psychosis

TL;DR: Early detection and phase-specific treatment may both be offered as supplements to standard care or may be provided through a specialized early intervention team.

The Positive and Negative Syndrome Scale (PANSS) for Schizophrenia

TL;DR: Review of five studies involving the PANSS provided evidence of its criterion-related validity with antecedent, genealogical, and concurrent measures, its predictive validity, its drug sensitivity, and its utility for both typological and dimensional assessment.

Measurement of Premorbid Adjustment in Chronic Schizophrenia

TL;DR: The Premorbid Adjustment Scale has been found to be useful in identifying patients likely to become chronically hospitalized or at high risk for readmission, and may also serve as a possible predictor of patients with brain abnormalities on a computerized tomography scan.

Duration of psychosis and outcome in first-episode schizophrenia.

TL;DR: Duration of psychosis before treatment may be an important predictor of outcome in first-episode schizophrenia, and is significantly associated with time to remission as well as with level of remission.

Awareness of Illness in Schizophrenia and Schizoaffective and Mood Disorders

TL;DR: It is suggested that severe self-awareness deficits are a prevalent feature of schizophrenia, perhaps stemming from the neuropsychological dysfunction associated with the disorder, and are more common in schizophrenia than in other psychotic disorders.
