Infantile Anorexia (IA), defined by the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood Revised (DC: 0-3R, Zero-To-Three, 2005), occurs when the child (a) refuses to eat adequate amounts of food for at least 1 month, and shows growth deficiency, (b) does not communicate hunger and lacks interest in food, and (c) the child’s food refusal does not follow a traumatic event and is not due to an underlying medical illness. IA usually emerges during the transition to self-feeding, when the child issues of autonomy are played out daily in the feeding situation. Studies evidence that the feeding interactions between children with IA and their mothers are characterized by low reciprocity, greater interactional conflict and negative affects (Ammaniti et al., 2010, 2012; Chatoor et al., 2000). Moreover, these studies pointed out that maternal depression and eating disorders are frequently associated with IA (Ammaniti et al., 2010; Cooper et al., 2004; Lucarelli et al., 2013). To date, research has focused almost exclusively on the mother–child dyad, while fathers’ involvement, co-parental and family interactions are poorly studied. The current study is a pilot research that investigated mother–father-child triadic interactions, during feeding and play, in families with children diagnosed with IA, in comparison to families with normally developing children. Until now, at the study participated N = 10 families (5 with a child with IA diagnosis and 5 with lack of child’s IA diagnosis, matched for child’s age and gender). The parents-child triadic interactions were assessed in feeding and play contexts using the Lausanne Trilogue Play (Fivaz-Depeursinge and Corboz-Warnery, 1999), adapted to observe father-mother-infant primary triangle in the feeding context, compared to the play context (Lucarelli, Simonelli, and Ammaniti., 2012). Families of the IA-group showed difficulties in expressing and sharing pleasure and positive affects, and in structuring a predictable and flexible context. Children showed little autonomy and difficulty in being actively engaged and tune with parents. Dysfunctional family interactions are a critical issue for IA that affects co-parental and family subsystems, stressing the importance of an articulated diagnostic assessment in order to target effective treatment approaches.

The key concept of Ignacio Matte Blanco’s bi-logic is the unavoidable but variable presence of primary process (symmetric thought) in the secondary one (asymmetric thought) ruling consciousness, for every human being. This variable and dynamic presence allows us, by therapeutic intervention, to convert suitably the symmetric thought into the asymmetric one. The former erupts into the latter by means of affectivity which, accordingly, should be suitably treated to be rightly modulated, regulated and symbolically represented to accomplish the aims of secondary process. This transition has been termed affective mentalization by Peter Fonagy et al. Accordingly, any therapeutic intervention should take into account these facts, aiming to treat the impetuous affective charge to accomplish this, through analytical means or the psychotherapeutic setting.

Objectives : In this work we tried to infer some important information in psychotherapeutic process from different measures. The primary aim was to obtain a better assessment of strengths and weaknesses of two patients during the first evaluation sessions and generally in psychotherapy. Methods : We used two main measures, the Adult Attachment Interview and the SWAP-200. The first assessed adult mental representations regarding relationships, while the latter assessed personality disorders and traits. We also measured reflective functioning through the Reflective Functioning scale applied on the AAI. As for the SWAP-200, the Personality Health Index and the RADIO were calculated. Results : We tried to explain in descriptive terms some unusual results from the measurements, comparing the data. Discordances in the different levels of reflective functioning of the two patients were explicated through the in-depth analysis of the SWAP indices, showing different ways to deal with personal and relational difficulties. Conclusions : Comparing two different patients that share similar scores in the assessment can be confusing. In order to better envision strengths and weaknesses of the patients it is sometimes necessary to go in depth of the different indices. Also, we tried to stress some of these results in order to better orient the therapeutic program.

Reflective and High Functioning in preliminary assessment

L’argomento che intendo affrontare e legato a problematiche che vengono da lontano e risalgono agli albori della psicoanalisi. Vorrei prima introdurre un antefatto che permetta di chiarire meglio il discorso. Nel settembre del 1932 lo psicoanalista ungherese Sandor Ferenczi, oggi considerato da molti il piu importante della sua generazione, quella immediatamente successiva e in parte contemporanea a Freud, presento al XII° Congresso Internazionale della Associazione Psicoanalitica, che si tenne a Wiesbaden, una relazione dal titolo poco comune per un testo scientifico: “Confusione delle lingue tra adulti e bambini (Il linguaggio della tenerezza e il linguaggio della passione)”. Da tempo le opinioni di Ferenczi riguardo alla tecnica terapeutica sollevavano un certo stupore e anche irritazione nell’ambiente psicoanalitico. L’ostilita nei suoi confronti era evidente nonostante fosse considerato il piu importante tra gli amici di Freud, l’allievo con il quale aveva avuto i rapporti piu stretti, il piu amato e forse quello dal quale e stato amato piu sinceramente. Il fondatore, assieme allo stesso Freud, della Associazione Internazionale di Psicoanalisi, della quale, pero, non fu mai presidente. Negli ultimi anni, pero, il rapporto tra i due amici era diventato difficile, in quanto il lavoro clinico e di ricerca di Ferenczi lo avevano spinto a tentare nuove strade terapeutiche (come la tecnica “attiva”, volta ad abbreviare il periodo di cura) e a sviluppare idee che erano state accolte con diffidenza dal maestro. In particolare Freud aveva avuto notizia che, in occasione del convegno, Ferenczi intendeva affrontare argomenti, come quello dei traumi sessuali infantili, che avevano caratterizzato le prime ipotesi teoriche di Freud, ora abbandonate, e che preferiva fossero evitati. Nonostante questo, quattro giorni prima del congresso, al quale Freud non avrebbe potuto partecipare perche troppo malato, Ferenczi si senti in dovere di andare da Budapest a Vienna, in Berggasse 19, a casa del proprio maestro, per leggergli in anteprima la relazione e avere un suo giudizio (Jones, 1953, vol. 3; Fromm, 1959; Masson, 1984; Gay, 1988; Antonelli, 1997). La tensione era alta. Ferenczi entro nello studio di Freud e, senza salutarlo, in piedi, gli chiese di leggere la relazione, senza peraltro provocare in lui una particolare reazione se non il consiglio di rinunciare a pubblicarla sotto forma di articolo. Nella stanza in quel momento era presente anche Abraham A. Brill, un personaggio molto influente e conservatore (era contrario a permettere ai non medici di diventare psicoanalisti) che era considerato il rappresentante ufficiale della Associazione Psicoanalitica Americana, ma nei cui confronti Ferenczi e Freud non nutrivano alcuna simpatia. Era stato Freud a insistere che Brill fosse presente, forse per dimostrare al collega americano una certa difesa dell’ortodossia. Quando Ferenczi se ne ando, offeso per l’atteggiamento distaccato del proprio maestro, Brill chiese a Freud come aveva potuto accettare che quella relazione venisse presentata al congresso di psicoanalisi.
