Till today, thrombocytopenia is not included in severe malaria criterion described by WHO, but when platelet counts <20 × 103/mm3, we advocate it to include as one of the severe malaria criteria.
It is recommended that platelet count be determined as it could assist in the diagnosis of malaria infection.
Further, platelet turnover rates and recovery provide a useful prognostic parameter during malaria infection.
Finally, we have established that platelet-mediated clumping is strongly associated with severe malaria.
An inverse relationship between the platelet count and platelet antibody levels in serum supported the view that thrombocytopenia in malaria may be partly immune-mediated.
The above finding can have therapeutic implications in context of avoiding unnecessary platelet infusion in malaria patients.
