XI. STRATEGIES FOR IMPROVING DIABETES CARE D iabetes is a chronic illness that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications. Diabetes care is complex and requires that many issues, beyond glycemic control, be addressed. A large body of evidence exists that supports a range of interventions to improve diabetes outcomes. These standards of care are intended to provide clinicians, patients, researchers, payors, and other interested individuals with the components of diabetes care, treatment goals, and tools to evaluate the quality of care. While individual preferences, comorbidities, and other patient factors may require modification of goals, targets that are desirable for most patients with diabetes are provided. These standards are not intended to preclude more extensive evaluation and management of the patient by other specialists as needed. For more detailed information, refer to Bode (Ed.): Medical Management of Type 1 Diabetes (1), Burant (Ed): Medical Management of Type 2 Diabetes (2), and Klingensmith (Ed): Intensive Diabetes Management (3). The recommendations included are diagnostic and therapeutic actions that are known or believed to favorably affect health outcomes of patients with diabetes. A grading system (Table 1), developed by the American Diabetes Association (ADA) and modeled after existing methods, was utilized to clarify and codify the evidence that forms the basis for the recommendations. The level of evidence that supports each recommendation is listed after each recommendation using the letters A, B, C, or E.

European Guidelines on Cardiovascular Disease Prevention in Clinical Practice (Version 2012)

Hypertension is the most common condition seen in primary care and leads to myocardial infarction, stroke, renal failure, and death if not detected early and treated appropriately. Patients want to be assured that blood pressure (BP) treatment will reduce their disease burden, while clinicians want guidance on hypertension management using the best scientific evidence. This report takes a rigorous, evidence-based approach to recommend treatment thresholds, goals, and medications in the management of hypertension in adults. Evidence was drawn from randomized controlled trials, which represent the gold standard for determining efficacy and effectiveness. Evidence quality and recommendations were graded based on their effect on important outcomes. There is strong evidence to support treating hypertensive persons aged 60 years or older to a BP goal of less than 150/90 mm Hg and hypertensive persons 30 through 59 years of age to a diastolic goal of less than 90 mm Hg; however, there is insufficient evidence in hypertensive persons younger than 60 years for a systolic goal, or in those younger than 30 years for a diastolic goal, so the panel recommends a BP of less than 140/90 mm Hg for those groups based on expert opinion. The same thresholds and goals are recommended for hypertensive adults with diabetes or nondiabetic chronic kidney disease (CKD) as for the general hypertensive population younger than 60 years. There is moderate evidence to support initiating drug treatment with an angiotensin-converting enzyme inhibitor, angiotensin receptor blocker, calcium channel blocker, or thiazide-type diuretic in the nonblack hypertensive population, including those with diabetes. In the black hypertensive population, including those with diabetes, a calcium channel blocker or thiazide-type diuretic is recommended as initial therapy. There is moderate evidence to support initial or add-on antihypertensive therapy with an angiotensin-converting enzyme inhibitor or angiotensin receptor blocker in persons with CKD to improve kidney outcomes. Although this guideline provides evidence-based recommendations for the management of high BP and should meet the clinical needs of most patients, these recommendations are not a substitute for clinical judgment, and decisions about care must carefully consider and incorporate the clinical characteristics and circumstances of each individual patient.

6. Reconsideration of the risk of overweight and target body mass index (BMI) in hypertension.

9. Target BP for treatment. More evidence-based criteria and unified target systolic blood pressure (SBP) (<140 mmHg) in both higher and lower CV risk patients.

Opis przypadku dotyczy chorego z ostrym zawalem serca, leczonego trombolitycznie, u ktorego w badaniu angiograficznym rozpoznano zagrazające pekniecie mieśnia sercowego. Z tego powodu chorego poddano pilnej operacji kardiochirurgicznej. Zabieg operacyjny i okres pooperacyjny przebiegly bez powiklan. Chory po ponad rocznej obserwacji jest w dobrym stanie klinicznym, bez dolegliwości wiencowych.

Uklad renina-angiotensyna-aldosteron wywiera znaczny wplyw na regulacje ciśnienia tetniczego poprzez dzialanie na gospodarke wodno-elektrolitową, funkcje środblonka i kurczliwośc blony mieśniowej naczyn. Wiekszośc poznanych dzialan angiotensyna II wywiera poprzez receptor AT1. Celem naszej pracy byla analiza wspolwystepowania polimorfizmow I/D genu ACE i A1166C genu receptora AT1 angiotensyny w zalezności od wysokości ciśnienia tetniczego.
 Material i metody Badaniem objeto 539 mezczyzn 
w wieku 44 ± 9 lat, u ktorych nie rozpoznawano wcześniej jawnych schorzen miazdzycopochodnych. U kazdego badanego określono na czczo poziom glikemii i frakcji lipidow. Ponadto zmierzono ciśnienie tetnicze, mase ciala, wzrost, obwod talii i bioder. Analize polimorfizmow I/D genu ACE i A1166C genu receptora AT1 angiotensyny II przeprowadzono za pomocą lancuchowej reakcji polimerazy DNA oraz techniki dlugości fragmentow restrykcyjnych i elektroforezy uzyskanych produktow na zelu agarozowym.
 Wyniki Wykonana analiza znaczenia interakcji wariantow polimorficznych genow AT1R i ACE dla nadciśnienia tetniczego nie wykazala istotnych zalezności. Iloraz szans dla nadciśnienia tetniczego związany z genotypem AC+CC genu receptora AT1 wynosil 0,59 (95% CI 0,28–1,26) wśrod homozygot DD genu ACE, p = 0,36 i 1,26 (95% CI 0,76–2,07) wśrod heterozygot ID, p = 0,36 oraz 0,76 (95% CI 0,36–1,61) wśrod homozygot II, p = 0,61. Wspolwystepowanie badanych polimorfizmow bylo takze nieistotne statystycznie po uwzglednieniu wplywu innych czynnikow ryzyka miazdzycy.
 Wnioski Nie stwierdzono istotnego związku wspolwystepowania wariantow polimorficznych I/D genu ACE i A1166C genu receptora angiotensyny AT1 
z nadciśnieniem tetniczym w badanej przez autorow grupie mezczyzn.

Association of the I/D Polymorphism of ACE and A1166C Polymorphism of the Angiotensin II AT1 Receptor Gene and Blood Preassure in Men without Clinical Manifestations of Atherocslerotic Diseases

Myo-inositol is the most popular inositol in living organisms, where it is present as a sugar alcohol, in a free form, and can also be described as a lipid. The aim of this study was to check the concentration change of a myo-inositol solution from the time of oral administration and over a 48 h period in Wistar-type rats. All rats received 2 g/kg of inositol as a solution in distilled water by oral gavage. Estimated parameters were based on the serum concentration of myo-inositol observed in nine individual rats with regard to time. Observations were described as a one-compartment pharmacokinetic model with first-order absorption and zero-order endogenous input of checked inositol. The highest myo-inositol concentration was observed in the first hour after oral administration in the serum of all tested rats. Then, the concentration began decreasing immediately after the maximal peak. The inositol concentration continued to decrease, but after 24 h its level was still higher than before the administration. The plasma profile of the myo-inositol concentration showed a rapid decline over time, possibly due to the metabolism of this compound.
