TL;DR: The modified sizing technique simplifies the "David-procedure" and allows to achieve a good intraoperative and mid-term results, however, these results have to be confirmed in a larger cohort with a long-term follow-up.

TL;DR: A 43-year-old patient with acute ST-segment elevation anterior myocardial infarction who underwent off-pump coronary artery bypass grafting and acute reperfusion of the infarcted coronary artery was achieved using an aortocoronary shunt thereby perfusing the occluded left anterior descending artery.
