Allergic Rhinitis and Its Impact on Asthma

The European Position Paper on Rhinosinusitis and Nasal Polyps 2020 is the update of similar evidence based position papers published in 2005 and 2007 and 2012. The core objective of the EPOS2020 guideline is to provide revised, up-to-date and clear evidence-based recommendations and integrated care pathways in ARS and CRS. EPOS2020 provides an update on the literature published and studies undertaken in the eight years since the EPOS2012 position paper was published and addresses areas not extensively covered in EPOS2012 such as paediatric CRS and sinus surgery. EPOS2020 also involves new stakeholders, including pharmacists and patients, and addresses new target users who have become more involved in the management and treatment of rhinosinusitis since the publication of the last EPOS document, including pharmacists, nurses, specialised care givers and indeed patients themselves, who employ increasing self-management of their condition using over the counter treatments. The document provides suggestions for future research in this area and offers updated guidance for definitions and outcome measurements in research in different settings. EPOS2020 contains chapters on definitions and classification where we have defined a large number of terms and indicated preferred terms. A new classification of CRS into primary and secondary CRS and further division into localized and diffuse disease, based on anatomic distribution is proposed. There are extensive chapters on epidemiology and predisposing factors, inflammatory mechanisms, (differential) diagnosis of facial pain, allergic rhinitis, genetics, cystic fibrosis, aspirin exacerbated respiratory disease, immunodeficiencies, allergic fungal rhinosinusitis and the relationship between upper and lower airways. The chapters on paediatric acute and chronic rhinosinusitis are totally rewritten. All available evidence for the management of acute rhinosinusitis and chronic rhinosinusitis with or without nasal polyps in adults and children is systematically reviewed and integrated care pathways based on the evidence are proposed. Despite considerable increases in the amount of quality publications in recent years, a large number of practical clinical questions remain. It was agreed that the best way to address these was to conduct a Delphi exercise . The results have been integrated into the respective sections. Last but not least, advice for patients and pharmacists and a new list of research needs are included. The objective of this paper is to give a comprehensive introduction to applied cryptography with an engineer or computer scientist in mind. The emphasis is on the knowledge needed to create practical systems which supports integrity, confidentiality, or authenticity. Topics covered includes an introduction to the concepts in cryptography, attacks against cryptographic systems, key use and handling, random bit generation, encryption modes, and message authentication codes. Recommendations on algorithms and further reading is given in the end of the paper. This paper should make the reader able to build, understand and evaluate system descriptions and designs based on the cryptographic components described in the paper.

Traditional remote user authentication methods mainly employ the possession of a token (magnetic cards, cell phones, personal digital assistant (PDA), and notebook computers, etc.) and/or the knowledge of a secret (password, etc.) in order to establish the identity of an individual. In 2006, Khan et al. proposed an efficient and practical chaotic hash-based fingerprint biometric remote user authentication scheme on mobile devices. The current paper, however, demonstrates that Khan et al.’s scheme is vulnerable to a privileged insider’s attacks and impersonation attacks by using lost or stolen mobile devices. Also, we present an improvement to their scheme in order to isolate such problems.

A Secure Chaotic Hash-Based Biometric Remote User Authentication Scheme Using Mobile Devices

In 2006, Shao et al. showed that Wang et al.'s deniable authentication protocol based on ElGamal cryptography is insecure to a person-in-the-middle attack; that is, in the whole process, the receiver cannot be aware of the existence of this adversary as well as the modification of the messages. Furthermore, they also presented a modification of Wang et al.'s protocol to overcome the security flaw. However, the current paper demonstrates that Shao et al.'s improved deniable authentication protocol is susceptible to a malicious receiver's impersonation attack. To mitigate this security breach, we propose an improved deniable authentication protocol based on ElGamal cryptography.

Secure Deniable Authentication Protocol Based on ElGamal Cryptography

In this paper, we present a new password-based authenticated key agreement protocol called PAKA, which provides mutual authentication and key agreement over an insecure channel between two parties knowing only a small password having low entropy. We then extend PAKA to a protocol called PAKA-X, in which the client uses a plaintext version of the password, while the server stores a verifier for the password, and which does not allow an adversary who compromises the server to impersonate a client without actually running a dictionary attack on the password file. The proposed protocols are secure against passive and active attacks and provide perfect forward secrecy.

Efficient Password-Based Authenticated Key Agreement Protocol

Recently, Wu and Chieu proposed an improvement to Sun’s scheme, whereby users could choose and change their passwords freely through a secure channel when using a remote system. However, this improved scheme is still susceptible to impersonation attacks and does not provide mutual authentication. Accordingly, the current paper demonstrates the vulnerability of Wu and Chieu’s scheme to impersonation attacks and presents an enhancement to resolve such problems. As a result, the proposed scheme enables users to update their passwords freely without the help of a remote system, while also providing mutual authentication.

Robust remote user authentication scheme

In 2004, both Ku et al. and Yoon et al. proposed remote user authentication scheme using smart cards, respectively. In 2007, Wang et al., however, showed that both Ku et al.'s scheme and Yoon et al.'s scheme are vulnerable to guessing attacks, forgery attacks and denied service attacks, as well as inefficiency in password authentication. Then, Wang et al. proposed an improvement on them to keep the merits of original schemes by using two-variant hashing operations. Nevertheless, this paper shows that Wang et al.'s scheme still does not provide perfect forward secrecy and is susceptible to a guessing attack and Denning-Sacco attack.
