Allergic Rhinitis and Its Impact on Asthma

The European Position Paper on Rhinosinusitis and Nasal Polyps 2020 is the update of similar evidence based position papers published in 2005 and 2007 and 2012. The core objective of the EPOS2020 guideline is to provide revised, up-to-date and clear evidence-based recommendations and integrated care pathways in ARS and CRS. EPOS2020 provides an update on the literature published and studies undertaken in the eight years since the EPOS2012 position paper was published and addresses areas not extensively covered in EPOS2012 such as paediatric CRS and sinus surgery. EPOS2020 also involves new stakeholders, including pharmacists and patients, and addresses new target users who have become more involved in the management and treatment of rhinosinusitis since the publication of the last EPOS document, including pharmacists, nurses, specialised care givers and indeed patients themselves, who employ increasing self-management of their condition using over the counter treatments. The document provides suggestions for future research in this area and offers updated guidance for definitions and outcome measurements in research in different settings. EPOS2020 contains chapters on definitions and classification where we have defined a large number of terms and indicated preferred terms. A new classification of CRS into primary and secondary CRS and further division into localized and diffuse disease, based on anatomic distribution is proposed. There are extensive chapters on epidemiology and predisposing factors, inflammatory mechanisms, (differential) diagnosis of facial pain, allergic rhinitis, genetics, cystic fibrosis, aspirin exacerbated respiratory disease, immunodeficiencies, allergic fungal rhinosinusitis and the relationship between upper and lower airways. The chapters on paediatric acute and chronic rhinosinusitis are totally rewritten. All available evidence for the management of acute rhinosinusitis and chronic rhinosinusitis with or without nasal polyps in adults and children is systematically reviewed and integrated care pathways based on the evidence are proposed. Despite considerable increases in the amount of quality publications in recent years, a large number of practical clinical questions remain. It was agreed that the best way to address these was to conduct a Delphi exercise . The results have been integrated into the respective sections. Last but not least, advice for patients and pharmacists and a new list of research needs are included. The objective of this paper is to give a comprehensive introduction to applied cryptography with an engineer or computer scientist in mind. The emphasis is on the knowledge needed to create practical systems which supports integrity, confidentiality, or authenticity. Topics covered includes an introduction to the concepts in cryptography, attacks against cryptographic systems, key use and handling, random bit generation, encryption modes, and message authentication codes. Recommendations on algorithms and further reading is given in the end of the paper. This paper should make the reader able to build, understand and evaluate system descriptions and designs based on the cryptographic components described in the paper.

In 2006, Liao et al. proposed a new password authentication scheme over insecure networks. The primary merit of their scheme is its simplicity and practicality for implementation under insecure communication links. The current paper, however, demonstrates that Liao et al.?s scheme is vulnerable to masquerading server attacks, password guessing attacks using lost smart cards, and insider attacks. In addition, we point out that Liao et al.?s password change scheme is insecure, because an unauthorized user can easily change a new password for a smart card.

Drawbacks of Liao et al.'s Password Authentication Scheme

Authentication protocols for multi-server architectures have gained momentum in recent times due to advancements in computing technologies and associated constraints. Lu et al. recently proposed a biometrics and smartcards-based authentication scheme for multi-server environment. The careful analysis of this paper demonstrates Lu et al.’s protocol is susceptible to user impersonation attacks and comprises insufficient data. In addition, this paper proposes an improved authentication with key-agreement protocol for multi-server architecture based on biometrics and smartcards. The formal security of the proposed protocol is verified using the widely accepted AVISPA (Automated Validation of Internet Security Protocols and Applications) tool to ensure that our protocol can withstand active and passive attacks. The formal and informal security analysis, and performance analysis sections determines that our protocol is robust and efficient compared to Lu et al.’s protocol and existing similar protocols.

User anonymity is very important security technique in distributed computing environments that an illegal entity cannot determine any information concerning the user's identity. In 2006, KumarRajendra proposed a Secure Identification and Key agreement protocol with user Anonymity (SIKA). This paper demonstrates the vulnerability of the SIKA protocol and then presents an improvement to repair the security flaws of the SIKA protocol.

An Improvement of the User Identification and Key Agreement Protocol with User Anonymity

Recently, secure group-oriented applications have grown very rapidly. In order to provide secure group communication, WMNs (Wireless Mesh Network) are required to establish a secret key which is shared with all participants. Recently, Tseng proposed an authenticated group key agreement for regarding mobile networks. This mobile network is an asymmetric wireless network which includes many low-power nodes and several powerful nodes. However, this protocol does not satisfy some important security properties such a simplicit key authentication. In this paper, we conducted ananalysis of Tseng's protocol and propose a secure and efficient group key agreement protocol regarding WMNs which improved upon it. The proposed protocol not only reduces the computational cost, but also meets strong security requirements.
