Allergic Rhinitis and Its Impact on Asthma

The European Position Paper on Rhinosinusitis and Nasal Polyps 2020 is the update of similar evidence based position papers published in 2005 and 2007 and 2012. The core objective of the EPOS2020 guideline is to provide revised, up-to-date and clear evidence-based recommendations and integrated care pathways in ARS and CRS. EPOS2020 provides an update on the literature published and studies undertaken in the eight years since the EPOS2012 position paper was published and addresses areas not extensively covered in EPOS2012 such as paediatric CRS and sinus surgery. EPOS2020 also involves new stakeholders, including pharmacists and patients, and addresses new target users who have become more involved in the management and treatment of rhinosinusitis since the publication of the last EPOS document, including pharmacists, nurses, specialised care givers and indeed patients themselves, who employ increasing self-management of their condition using over the counter treatments. The document provides suggestions for future research in this area and offers updated guidance for definitions and outcome measurements in research in different settings. EPOS2020 contains chapters on definitions and classification where we have defined a large number of terms and indicated preferred terms. A new classification of CRS into primary and secondary CRS and further division into localized and diffuse disease, based on anatomic distribution is proposed. There are extensive chapters on epidemiology and predisposing factors, inflammatory mechanisms, (differential) diagnosis of facial pain, allergic rhinitis, genetics, cystic fibrosis, aspirin exacerbated respiratory disease, immunodeficiencies, allergic fungal rhinosinusitis and the relationship between upper and lower airways. The chapters on paediatric acute and chronic rhinosinusitis are totally rewritten. All available evidence for the management of acute rhinosinusitis and chronic rhinosinusitis with or without nasal polyps in adults and children is systematically reviewed and integrated care pathways based on the evidence are proposed. Despite considerable increases in the amount of quality publications in recent years, a large number of practical clinical questions remain. It was agreed that the best way to address these was to conduct a Delphi exercise . The results have been integrated into the respective sections. Last but not least, advice for patients and pharmacists and a new list of research needs are included. The objective of this paper is to give a comprehensive introduction to applied cryptography with an engineer or computer scientist in mind. The emphasis is on the knowledge needed to create practical systems which supports integrity, confidentiality, or authenticity. Topics covered includes an introduction to the concepts in cryptography, attacks against cryptographic systems, key use and handling, random bit generation, encryption modes, and message authentication codes. Recommendations on algorithms and further reading is given in the end of the paper. This paper should make the reader able to build, understand and evaluate system descriptions and designs based on the cryptographic components described in the paper.

User authentication is the most fundamental decision in designing secure systems and to organize majority of the attacks. Though alpha-numeric password and biometric based authentication methods are the most popular methods till date, they have been exposed to numerous attacks. As an alternative solution, various graphical-based password authentication schemes have been proposed. The efficiency of a graphical password is evaluated by its intensity of security and usability. Regardless of being many existing methods most were unsuccessful to accomplish both features concurrently. In this paper, we propose recognition based mutual authentication method with mirror-image encryption technique, based on images and text. The goal of this paper is to emphasize the enhancements on security and usability of existing graphical password schemes. The proposed method is resistant to most of existing attacks such as shoulder-surfing, secret camera etc.

Implicit graphical password mutual authentication using mirror-image encryption

In 2005, Lee and Chen proposed an improved one-time password authentication scheme that can prevent a stolen verifier attack and that is as efficient as the scheme of Yeh-Shen-Hwang. The current paper, however, demonstrates that Lee-Chen's scheme is still vulnerable to Denial-of-Service attacks and a simple solution is presented in order to isolate such a problem. Furthermore, we propose an efficiently optimized one-time password authentication scheme that can provide user anonymity. In addition the computational costs are lower than those of Lee-Chen's scheme.

In 2008, Zhu-Wu showed that the redefined Liaw’s broadcasting cryptosystem is still insecure in that an unauthorized user is able to obtain the shared secret, which is only intended for certain privileged users. However, Zhu-Wu did not provide an improvement of the redefined Liaw’s broadcasting cryptosystem. In addition, we found that the redefined Liaw’s broadcasting cryptosystem has another security problems. Therefore, this paper proposes a new broadcasting cryptosystem that can withstand Zhu-Wu’s attack and our proposed attacks. The proposed cryptosystem not only has still advantages of the redefined Liaw’s broadcasting cryptosystem, but also is more secure compare with previous related broadcasting cryptosystems.

Lee-Chen claimed that their proposed protocol not only is secure to a forgery attack, but also provides perfect forward secrecy. However, this paper points out that Lee-Chen's protocol is still insecure to a forgery attack and cannot provide perfect forward secrecy.
