Chronic obstructive pulmonary disease (COPD) remains a major public health problem. It is the fourth leading cause of chronic morbidity and mortality in the United States, and is projected to rank fifth in 2020 in burden of disease worldwide, according to a study published by the World Bank/World Health Organization. Yet, COPD remains relatively unknown or ignored by the public as well as public health and government officials. In 1998, in an effort to bring more attention to COPD, its management, and its prevention, a committed group of scientists encouraged the U.S. National Heart, Lung, and Blood Institute and the World Health Organization to form the Global Initiative for Chronic Obstructive Lung Disease (GOLD). Among the important objectives of GOLD are to increase awareness of COPD and to help the millions of people who suffer from this disease and die prematurely of it or its complications. The first step in the GOLD program was to prepare a consensus report, Global Strategy for the Diagnosis, Management, and Prevention of COPD, published in 2001. The present, newly revised document follows the same format as the original consensus report, but has been updated to reflect the many publications on COPD that have appeared. GOLD national leaders, a network of international experts, have initiated investigations of the causes and prevalence of COPD in their countries, and developed innovative approaches for the dissemination and implementation of COPD management guidelines. We appreciate the enormous amount of work the GOLD national leaders have done on behalf of their patients with COPD. Despite the achievements in the 5 years since the GOLD report was originally published, considerable additional work is ahead of us if we are to control this major public health problem. The GOLD initiative will continue to bring COPD to the attention of governments, public health officials, health care workers, and the general public, but a concerted effort by all involved in health care will be necessary.

The 11th edition of Harrison's Principles of Internal Medicine welcomes Anthony Fauci to its editorial staff, in addition to more than 85 new contributors. While the organization of the book is similar to previous editions, major emphasis has been placed on disorders that affect multiple organ systems. Important advances in genetics, immunology, and oncology are emphasized. Many chapters of the book have been rewritten and describe major advances in internal medicine. Subjects that received only a paragraph or two of attention in previous editions are now covered in entire chapters. Abstract Invasive pneumococcal disease (IPD) is responsible for substantial clinical and economic burden in adults. Despite the safety and effectiveness of pneumococcal vaccines in protecting against these serious infections, immunization of eligible adults remains largely underused. The Centers for Disease Control and Prevention’s Advisory Committee on Immunization Practices has recently expanded the use of the pneumococcal vaccine to include adults with asthma and all cigarette smokers, in addition to persons previously designated as at-risk. Improving adult pneumococcal immunization rates will require efforts from a broad range of healthcare providers. Clinicians should take a proactive approach in recommending the vaccine to eligible adults and should implement strategies that remove barriers to immunization efforts.

Update on Preventing Pneumococcal Disease in Adults

The elderly are among the groups at highest risk for the serious complications of influenza that can result in hospitalization or death. We conducted a cost benefit and cost effectiveness analysis of influenza vaccination among the elderly. In the economic models we incorporated new information on the benefits of vaccination in reducing hospitalizations for heart disease and stroke, in addition to previously known benefits of vaccination in reducing hospitalizations for pneumonia, influenza and other respiratory conditions. The model took the perspective of the health care payer. Monte Carlo simulation was used to estimate the net costs or savings associated with vaccination. The results of the cost benefit analysis indicated that vaccination would result in net savings of $71 for each elderly person vaccinated (5th to 95th percentiles, $32 to $119 in savings). Vaccination resulted in the prevention of 9 deaths per 1000 persons vaccinated, which translated into net savings of $810 per life-year gained. The findings from this study lend additional support to policies and programs that promote routine influenza vaccination of all persons 65 years and older.
