Chronic obstructive pulmonary disease (COPD) remains a major public health problem. It is the fourth leading cause of chronic morbidity and mortality in the United States, and is projected to rank fifth in 2020 in burden of disease worldwide, according to a study published by the World Bank/World Health Organization. Yet, COPD remains relatively unknown or ignored by the public as well as public health and government officials. In 1998, in an effort to bring more attention to COPD, its management, and its prevention, a committed group of scientists encouraged the U.S. National Heart, Lung, and Blood Institute and the World Health Organization to form the Global Initiative for Chronic Obstructive Lung Disease (GOLD). Among the important objectives of GOLD are to increase awareness of COPD and to help the millions of people who suffer from this disease and die prematurely of it or its complications. The first step in the GOLD program was to prepare a consensus report, Global Strategy for the Diagnosis, Management, and Prevention of COPD, published in 2001. The present, newly revised document follows the same format as the original consensus report, but has been updated to reflect the many publications on COPD that have appeared. GOLD national leaders, a network of international experts, have initiated investigations of the causes and prevalence of COPD in their countries, and developed innovative approaches for the dissemination and implementation of COPD management guidelines. We appreciate the enormous amount of work the GOLD national leaders have done on behalf of their patients with COPD. Despite the achievements in the 5 years since the GOLD report was originally published, considerable additional work is ahead of us if we are to control this major public health problem. The GOLD initiative will continue to bring COPD to the attention of governments, public health officials, health care workers, and the general public, but a concerted effort by all involved in health care will be necessary.

Rehabilitation for people with cardiac conditions: an overview of systematic reviews

The objectives were to assess the short- and long-term effect of the patient education strategy 'Learning and Coping' (LC) in cardiac rehabilitation (CR) on health-related quality of life, patient education impact, cardiac risk factors and lifestyle. In total, 825 patients hospitalized with ischaemic heart disease or heart failure were randomized to either LC-CR or standard CR at three Danish hospitals. Teaching approach in LC-CR was situational, inductive and reflective, with experienced patients as co-educators and supplemental interviews. Teaching approach in standard CR was structured and deductive. Outcomes were assessed immediately after CR, and after 3 months (short term), and after 3 years (long term). Between-arm differences in favour of LC-CR were SF-12 'role emotional' (3.7, 95% CI: 0.6-6.8) and MDI depression score (0.9, 0.1-1.8) immediately after CR, exercise capacity (4 W, 1-9) at 3 months and SF-12 'role physical' (4.6, 0.1-9.0) (long term). Between-arm differences in favour of controls were waist circumference (-1.7 cm, -2.3 to -1.0) immediately after CR and HeiQ domain 'Constructive attitudes and approaches' (0.11, 0.04-0.18), triglycerides (-0.12 mmol/l, -0.21 to -0.02), systolic blood pressure (-3.12 mmHg, -5.66 to -0.58) at 3 months. Adding LC strategies to CR provides inconsistent short-term results but improves 'role physical' long term.

Short- and long-term effects of the patient education strategy-learning and coping-in cardiac rehabilitation: a randomized controlled trial (LC-REHAB)

The CACTUS Study is a randomised pragmatic trial with a nested qualitative study comparing traditional acupuncture with usual care in people who consult frequently with medically unexplained symptoms.1 2 Margaret McCartney has based her Observations article about it on several inaccuracies (quotations from her article in italics below).3

(1) The “wellbeing score” was better in the control group than in the acupuncture group . This statement is untrue: an adjusted mean difference in favour of acupuncture was seen with wellbeing (wellbeing questionnaire, W-BQ12: 4.4 (1.6 to 7.2); P=0.002). these five studies comprised lifestyle advice, education, support and the monitoring and titration of medicines. Importantly, the monitoring and titration of medicines was achieved by the use of locally agreed protocols, treatment algorithms and guidelines. Although Clark et al. refer to these activities as prescribing, they are not. Furthermore, nurses in the UK have only been able to prescribe medicines independently for hypertension since 2006 [2]. Four of the 11 studies reviewed by Clark et al. were undertaken in the UK prior to 2006. Given the above, it is impossible to see how Clark et al. have reached the conclusion that there is evidence of improved outcomes with nurse prescribers. Nurses in the UK have the most extended prescribing rights in the world. Importantly, misunderstanding about the role and nature of nurse prescribing has been reported amongst some healthcare professionals [3–6]. Many countries, for example the Netherlands, are looking to follow our example and have used our experiences to inform the development of this role for nurses. It is therefore essential that the term prescribing is used appropriately and correctly. The following, provided by the Department of Health [2], is a useful definition of prescribing.
