TL;DR: Among patients with E coli or K pneumoniae bloodstream infection and ceftriaxone resistance, definitive treatment with piperacillin-tazobactam compared with meropenem did not result in a noninferior 30-day mortality, and findings do not support use of piperACillin- tazobactsam in this setting.

Voriconazole concentrations and outcome of invasive fungal infections

TL;DR: The data suggest that voriconazole TDM is appropriate for all patients as soon as steady state is achieved, and for non-responding patients with low trough concentrations, the association with IFI-related mortality indicates the need for dose adjustments to achieve and sustain voriconsazole concentrations.
