Safety, ethical considerations, and application guidelines for the use of transcranial magnetic stimulation in clinical practice and research

OBJECTIVE — To systematically review the incidence of posttransplantation diabetes (PTD), risk factors for its development, prognostic implications, and optimal management. RESEARCH DESIGN AND METHODS — We searched databases (MEDLINE, EMBASE, the Cochrane Library, and others) from inception to September 2000, reviewed bibliographies in reports retrieved, contacted transplantation experts, and reviewed specialty journals. Two reviewers independently determined report inclusion (original studies, in all languages, of PTD in adults with no history of diabetes before transplantation), assessed study methods, and extracted data using a standardized form. Meta-regression was used to explain between-study differences in incidence. RESULTS — Nineteen studies with 3,611 patients were included. The 12-month cumulative incidence of PTD is lower (10% in most studies) than it was 3 decades ago. The type of immunosuppression explained 74% of the variability in incidence (P 0.0004). Risk factors were patient age, nonwhite ethnicity, glucocorticoid treatment for rejection, and immunosuppression with high-dose cyclosporine and tacrolimus. PTD was associated with decreased graft and patient survival in earlier studies; later studies showed improved outcomes. Randomized trials of treatment regimens have not been conducted. CONCLUSIONS — Physicians should consider modification of immunosuppressive regimens to decrease the risk of PTD in high-risk transplant recipients. Randomized trials are needed to evaluate the use of oral glucose-lowering agents in transplant recipients, paying particular attention to interactions with immunosuppressive drugs. Three patients with definite multiple sclerosis (MS) and acute loss of visual acuity on one side but without side-to-side differences in visual evoked potential (VEP) latencies revealed prolongation of visual event-related potentials (ERP) on the impaired side. These patients represent 5.1 % of a cohort with clinical optic mononeuritis and 21.4% of a cohort with clinical optic mononeuritis but no side-to-side differences in VEP. The data suggest, that in some cases, side different measurement of ERP (rather than VEP) is an appropriate method to prove the sensory or cognitive impairment in patients with MS and clinical optic mononeuritis.

Der prozentuale Anteil von Patienten mit Herdbefunden im EEG ging von 5,1% auf 1,7% zuruck. Die CD4-Zellen fielen im Untersuchungszeitraum von 294,2±209,5/μl auf 188,7±208,3/μl ab (p<0,01). Die Ergebnisse dieser Langsschnittuntersuchung belegen eine progrediente zerebrale Dysfunktion in Korrelation zur Verschlechterung des Immunstatus.

Kopfschmerzklassifikationen existieren seit der Spatantike und haben zuerst in einfacher Weise Kopfschmerzen semiologisch beschrieben. Durch die humoralpathologische Medizin kam es im Mittelalter aber auch zu atiologisch orientierten Klassifikationen, die jedoch immer von der vorherrschenden Lehrmeinung ausgingen und sich immer wieder als nicht haltbar erwiesen. Im 20. Jahrhundert wurde als erstes 1962 eine atiologische Klassifikation vorgelegt, die aber durch moderne Forschungsergebnisse uberholt wurde. Die aktuelle Kopfschmerzklassifikation ist zwar phanomenologisch orientiert, wird sich aber durch moderne Feldforschung hin zu einer atiologisch orientierten entwickeln.

Es wird der Fall eines 34 Jahre alten Patienten berichtet, der nach einer Epstein-Barr-Virus-Infektion eine symptomatische Hypersomnie entwickelt hat. Diese Hypersomnie konnte mit konventionellen Substanzen nicht befriedigend gebessert werden. Unter Solriamfetol kam es zu einer signifikanten Besserung der exzessiven Tagesschläfrigkeit.
