Abstract: Introduction: Uterine rupture is an obstetric catastrophe that is associated with high maternal and perinatal mortality rates. Its incidence is high in developing countries. In Nigeria the incidence of uterine rupture continues to increase due to poverty illiteracy ignorance the lack of quality obstetric care and bad governance. Methods: A retrospective review was conducted of all cases of ruptured uterus seen at the Mater Misericordiae Hospital in Afikpo Nigeria between January 2001 and December 2007. Results: There were a total of 51 ruptured uteri out of 4361 deliveries yielding a ratio of one in 86. A total of 19 (37.3 percent) patients had a scarred uterus while 32 (62.7 percent) had an intact uterus yielding a scarred to unscarred uterus ratio of one in 1.7. The mean maternal age was 32.0 years and the mean parity was 3.4. Patients with a scarred uterus had a lower mean age and parity. 29 (56.9 percent) cases were unbooked out of which seven (24.1 percent) had a scarred uterus. Obstructed labour (88.2 percent) grandmultiparity (27.5 percent) and abnormal lie (9.8 percent) were the main causes of rupture. 39 (76.5 percent) patients had their labour managed at home or by traditional birth attendants before presentation. 45 (88.2 percent) patients underwent uterine repair alone five (9.8 percent) had uterine repair with bilateral tubal ligation and one (two percent) had a hysterectomy. Three maternal deaths occurred yielding a case fatality rate of 5.9 percent. The perinatal mortality rate was 84.6 percent. Conclusion: Health education the provision of quality obstetric care improved governance and monitoring of the activities of traditional birth attendants may help to reduce the menace of uterine rupture.

Abstract: Observation has revealed that women who book at Ebonyi State University Teaching Hospital, Abakaliki, concurrently use multiple antenatal care givers (formal and non-formal). This study was therefore conducted to identify the other sources of antenatal care for the women and to examine them in relation to their socio-demographic characteristics. A total of 200 antenatal women chosen from two randomly selected booking clinics were interviewed using a structured questionnaire. The results show that 25% and 30.5% respectively from the two clinics were concurrently using formal and unorthodox ANC givers. Age and education were significantly related to the practice (p < 0.0002 and p < 0.02 respectively). These findings suggest that free services alone may not be enough to make women attend ANC in the hospital. A larger study, especially investigating patient's perspective of the quality of care, is recommended.

Abstract: In Africa infertility constitutes a major gynaecological complaint and causes enormous sociopsychological stress to the patients. This study examined retrospective data at the University of Benin Teaching Hospital Benin City Nigeria over a 5-year period to determine the factors associated with tubal infertility. Tubal infertility was confirmed in 13.5% of the 1181 new cases of infertility over the study period. The mean age of the patients was 33.2 ± 9.5 years. Over 65% were nulliparous and all socioeconomic classes were affected. Major associated factors included infections such as post-abortal sepsis puerperal sepsis and pelvic inflammatory disease (PID). Abstract: Purpose – The purpose of this paper is to report the results of an empirical study that tested two competing models of relationship quality for superiority and further examined the effect of relationship quality antecedents as well as relationship quality constructs on customer loyalty. Design/methodology/approach – Quantitative cross-sectional data generated from 332 informants who are experienced users of banking services formed the final database. The construct validity and reliability of the data set was confirmed. The two competing models were tested with partial least squares structural equation modelling technique. Findings – Although the two competing models support all the hypothesised relationships as expected, the disaggregated model of relationship quality was found to be a better predictor of customer loyalty than the composite model of the construct.
