Abstract: The high survival rates of more than 80% achieved in childhood cancer treatment have been based on intensive therapy or maximum tolerated dose. With the refinement in risk stratification, groups of patients that can achieve acceptably high survival with less intensive therapy and therefore less treatment related toxicity are being identified.[1] Consequently, it becomes logical to assume that smaller doses of anticancer agents are all that is needed in some patients. This approach is quite tempting in resource‐constrained lower‐middle income countries (LMCs) which carry the huge burden of cancer cases, but limited by the fact that long‐term survival rarely exceeds 20% in some of these countries.[2] The reasons for the dismal outcome in the LMC are many. A significant number of patients are diagnosed at an advanced stage and therefore are likely to relapse or qualify for outright palliative care. The limited supportive care available in the LMCs means that treatment toxicities cannot be managed efficiently. In addition, most families do not have enough resources to absorb the huge costs of treatment.[3] Parents/patients’ lack of faith in the health‐care system due to high mortality rate may fuel delay in diagnosis and treatment abandonment, creating a vicious circle.[4]

Abstract: Ethnopharmacological Relevance. Development of resistance to antimalarial drugs by Plasmodium falciparum is still rampant, and there is an urgent need for novel drugs to either standalone or to partner artemisinin for treatment of malaria. Traditionally, plants have, over the years, been a good source of antimalarial drugs. Efficacy and safety of such plants need to be scientifically authenticated. Aims, Materials, and Method. This study investigated the in vitro antiplasmodial activity, cytotoxicity, and genotoxicity of aqueous extracts of Acanthospermum hispidum DC, Alstonia boone (De Wild), Cocos nucifera L, Cymbopogon citratus (DC.) Stapf, Morinda lucida Benth, Psidium guajava, Phyllanthus niruri L, and Senna siamea Lam. Results. Five out of the eight plants, A. boonei stem bark, S; siamea Lam root, M. lucida Benth leaves, P. niruri, and A. hispidum DC whole plants, showed varying degrees of antiplasmodial activity against the asexual stage of the parasite. The most active extract against chloroquine-sensitive (3D7) and chloroquine-resistant (Dd2) P. falciparum strains is the A. hispidum extract which yielded a mean inhibitory concentration at 50% (IC50) of 3.66 µg/ml and 3.71 µg/ml for 3D7 and Dd2, respectively. This was followed by S. siamea Lam with 3.95 µg/ml for 3D7 and 4.47 µg/ml for Dd2. The IC50 values of the A. boonei extract against 3D7 and Dd2 P. falciparum parasites were 5.13 µg/ml and 3.62 µg/ml, respectively. For the M. lucida Benth extract, the least IC50 value was 6.46 µg/ml. All five extracts exhibited dose-dependent antiplasmodial activity. Assessment of the genotoxic effects the A. hispidum extract by the comet assay revealed substantial damage to P. falciparum DNA. Conclusion. This study demonstrates that the crude extract of A. hispidum DC, one of the plants used traditionally to treat malaria, inhibits the growth of P. falciparum in vitro and could be a potential source of antimalarial drug. The report has highlighted genotoxic and cytotoxic effects of the selected plant extracts on human leukocytes as well.

Abstract: Background The testing capacity for SARS-CoV-2 in Africa is rather limited. Antigen-detection rapid diagnostic tests (Ag-RDTs) are a cheap and rapid alternative to reverse transcriptase-polymerase chain reaction (RT-PCR) tests, but there is little data about their performance under real life conditions in tropical countries. Objective To evaluate the performance of a standard Ag-RDT in a population of a major hospital in northern Ghana. Methods Prospective, cross-sectional, blinded verification of the performance of the SD Biosensor Standard Q SARS-CoV-2 Ag-RDT under real life conditions in 135 symptomatic patients and 58 contacts of RT-PCR positives at Tamale Teaching Hospital in February 2021. Nasopharyngeal samples were taken under standard conditions and tested against RT-PCR in the hospital laboratory. Results 193 participants (median age 35 years, 109 male) were included into the study for which both RT-PCR test and Ag-RDT results were available. A total of 42 (22%) were RT-PCR-positive. Of the 42 RT-PCR-positives, 27 were Ag-RDT positive, resulting in a sensitivity of 64% (95% CI 49-79). Sensitivity among symptomatic patients was 58% (95% CI 38-78). 123 were identified Ag-RDT negatives of the 151 RT-PCR negatives, resulting in a specificity of 81% (95% CI 75-87). Conclusions SARS-CoV-2 Ag-RDTs appear to have a rather low sensitivity and particularly a low specificity under real life conditions in Africa. The role of existing Ag-RDTs in countries with high-temperature climates and limited resources still needs more data and discussion.

Abstract: Currently, blood donors in Ghana are not screened for malaria parasites. Therefore, this study assessed platelet thrombogenicity in blood donors infected asymptomatically with Plasmodium falciparum and the relationship between tumour necrosis factor alpha (TNF-α), 8-iso-prostaglandin F2α oxidative stress biomarker (8-iso-PG2α), C-reactive protein (hs-CRP) and D-dimer, and platelet thrombogenes levels. Haematology analyser was used to enumerate platelet count and platelet indices in 80 P. falciparum infected blood donors and 160 matched non-infected controls. Replicate serum levels of von Willebrand Factor (vWF), platelet factor 4 (PF4), P-selectin thrombogenic factors as well as TNF-α and 8-iso-PG2α were determined using enzyme immuno-assay while high sensitive hs-CRP and D-dimer concentrations were determined by fluorescent immunoassay. The geometric mean of parasite density in malaria infected donors was 1784 parasites/µL (505-2478 parasites/µL). This led to significant increase in the mean levels of 8-iso-PG2α, hs-CRP, TNF-α and D-dimer. However, PF4, P-selectin were significantly lower in infected donors while vWF levels did not differ significantly among the groups even though lower levels were observed in the infected donors. Significant direct relationship existed between both P-selectin and PF4 and platelet count, and plateletcrit and platelet large cell ratio whereas these thrombogenic factors varied inversely to 8-iso-PG2α, TNF-α and hs-CRP. Relative thrombocytopaenia was associated with significant reduction in P-selectin and platelet factor 4 levels together with increased 8-iso-PG2α, hs-CRP, TNF-α and D-dimer levels. Taken together, it is recommended that all P. falciparum infected blood donors should be deferred.
