Abstract: The twentieth century has seen a sea change in the concentration of ownership of U.S. corporations. Early in the century Berle and Means identified the divorce of ownership from control as the central corporate governance problem, but since the 1970’s ownership has been re’rating into the hands of fiduciary institutions – most notably pension funds and mutual funds. By the 1990s fiduciaries collectively owned over 50% of the outstanding equity of the 1,000 largest corporations. This new pattern of ownership, fiduciary capitalism, has begun to raise important policy questions including: How can agents (fiduciaries) effectively monitor other agents (boards of directors)? What are the social implications of universal ownership where fiduciaries own substantial stakes in virtually all of the corporations in a country, and, finally, What does it mean to maximize shareholder wealth when fiduciaries are universal owners?

Abstract: Malignant eccrine spiradenoma is an exceedingly rare tumor. A case of a 72-year-old women with this highly aggressive malignancy arising from a long-standing lower leg lesion is reported. Management during the course of disease included surgery, radiation therapy (RT), hyperthermic limb perfusion chemotherapy, and chemotherapy. The patient died of her disease, with widespread metastatic disease 20 months after the diagnosis. A review of the literature is presented, and treatment considerations are summarized.

Abstract: In order to examine whether saliva and breast-milk are mediators of the vertical transmission of hepatitis C virus (HCV) from an HCV carrier mother, serum, saliva, and breast-milk samples from 11 HCV carrier mothers were collected at the time of delivery, and at approximately 1- to 3-months intervals for as long as 30 months postpartum. Serum was also sampled from their children. All samples were analysed for the presence of HCV RNA, using the nested polymerase chain reaction method. No HCV RNA was detected in any breast-milk samples. In saliva, HCV RNA was detected in four of the 11 mothers (36%). These four mothers also had liver function abnormalities. Hepatitis C virus RNA was not detected in any serum samples of the children, and all children had normal liver function. The children were monitored for periods from 2 to 44 months. During this period, there was no evidence of virus transmission. Breast-milk is not likely to be a source of mother-to-child transmission of HCV. Maternal saliva may harbour HCV, but it may not result in infant infection.

Abstract: Minor Pliocene dacites from Crommyonia mark the western end of the South Aegean volcanic arc. They form small lava domes and flows generally associated with extensional faults. An older group (3.6–4 Ma) occurs in the west and a younger group (2.3–2.8 Ma) in the east. Volcanic rocks of similar age are found at Aegina, Poros and Milos in the western part of the South Aegean arc, whereas volcanism in the eastern part of the arc is of Quaternary age. The two groups of rocks at Crommyonia are chemically distinct. Both groups contain multiple generations of plagioclase. Both have eNd (−8.0 to −10.6) that is much more negative than any other rocks in the South Aegean arc and model ages that are similar to those for many Miocene extensional granites of the Cyclades. The model ages are interpreted to reflect a mid-Proterozoic mantle event recognized elsewhere in the Hellenides. The Crommyonia dacitic magmas represent the first stages of melting of deep lithosphere as a result of both subduction-related hydrous fluids and extensional decompression. Plagioclase compositions suggest important magma evolution in a base-of-crust magma chamber, where the strong crustal Nd isotope signature was acquired. With time, asthenospheric sources that upwelled as a result of extension played an increasingly important role in determining the isotopic characteristics of the arc volcanism.

Abstract: A 51-yr-old woman was transferred to the emergency department with nonspecific interscapular pain and a progressive right-sided hemiparesis. Physical examination and laboratory examination revealed moderate right-sided hemiparesis, with no other fecal neurologic deficits. A computed axial tomography scan of the brain was negative. Emergency arteriography revealed bilateral highgrade fibromuscular dysplasia involving the origins of the left and right common carotid arteries. The patient was admitted with the presumptive diagnosis of embolic cerebrovascular accident. Subsequently, the patient developed a left-sided sensory deficit, and magnetic resonance imaging of the cervical spine revealed a C5-T2 epidural hemorrhage. The patient underwent emergent surgical evacuation of the clot and recovered without incident. Spinal epidural hematomas are rare and typically present as cord compressions with or without pain rather than as unilateral hemiplegia.

Abstract: Mortality and morbidity are high in elderly stroke patients. Early mobilization and prevention of stroke-related complications improve their ability to participate in a more intense and comprehensive rehabilitation program. An interdisciplinary approach to stroke rehabilitation restores functional loss, improves quality of life, and decreases long-term economic costs. Important parts of stroke rehabilitation include patient and family education, treatment of stroke-related complications, and prevention of recurrent stroke. A healthy and caring spouse, continence of bladder and bowel, and ability to feed oneself are the most positive predictors of stroke outcome.

Abstract: Objective To audit the provision of urodynamic services in the UK, with particular emphasis on the personnel involved and the training they received. Methods A questionnaire was sent to 163 centres carrying out urodynamics in the UK asking for information on the department providing the service, the frequency of use, the number of patients investigated, professional training of the personnel and whether the personnel thought that the training was adequate. Results Most urodynamic facilities in the UK function on a part-time basis. Urodynamics are carried out by a variety of healthcare professionals but most are performed by doctors and nurses. Half the respondents considered the training to be inadequate. Conclusion Consideration should be given to requiring a minimum standard of training for personnel carrying out urodynamics in the UK to ensure that a quality service is provided.

Abstract: The Mayo Medical Center, Rochester, Minnesota, has developed a unique collaborative method to educate both the experienced and the recent graduate nurse about critical care. These programs are 12 to 16 weeks and include a classroom, a skills lab, computer-assisted learning, and preceptor instruction. The curriculum model design incorporates Benner's novice-to-expert professional advancement model. Professional growth of the critical care nurse occurs through a combination of educational strategies that build on general theory to the application of critical thinking skills. The result of this program is a competent, practicing, critical care nurse.

Abstract: In the quenched approximation we use the abelian and monopole fields from abelian projection in SU(2) lattice gauge theory to numerically compute the value of the chiral condensate. The condensate calculated using abelian projection is observed to vanish at the same critical temperature as the full SU(2) theory predicts.
