Eosinophilic esophagitis (EoE) is a clinicopathologic condition of increasing recognition and prevalence. In 2007, a consensus recommendation provided clinical and histopathologic guidance for the diagnosis and treatment of EoE; however, only a minority of physicians use the 2007 guidelines, which require fulfillment of both histologic and clinical features. Since 2007, the number of EoE publications has doubled, providing new disease insight. Accordingly, a panel of 33 physicians with expertise in pediatric and adult allergy/immunology, gastroenterology, and pathology conducted a systematic review of the EoE literature (since September 2006) using electronic databases. Based on the literature review and expertise of the panel, information and recommendations were provided in each of the following areas of EoE: diagnostics, genetics, allergy testing, therapeutics, and disease complications. Because accumulating animal and human data have provided evidence that EoE appears to be an antigen-driven immunologic process that involves multiple pathogenic pathways, a new conceptual definition is proposed highlighting that EoE represents a chronic, immune/antigen-mediated disease characterized clinically by symptoms related to esophageal dysfunction and histologically by eosinophil-predominant inflammation. The diagnostic guidelines continue to define EoE as an isolated chronic disorder of the esophagus diagnosed by the need of both clinical and pathologic features. Patients commonly have high rates of concurrent allergic diatheses, especially food sensitization, compared with the general population. Proved therapeutic options include chronic dietary elimination, topical corticosteroids, and esophageal dilation. Important additions since 2007 include genetic underpinnings that implicate EoE susceptibility caused by polymorphisms in the thymic stromal lymphopoietin protein gene and the description of a new potential disease phenotype, proton pump inhibitor-responsive esophageal eosinophila. Further advances and controversies regarding diagnostic methods, surrogate disease markers, allergy testing, and treatment approaches are discussed.

The perspective of celiac disease patients on emerging treatment options and non-celiac gluten sensitivity

Die vorliegende Ubersicht der IBD-Dach-Gruppe legt eine Ubersicht zu Wirkmechanismus, klinischer Entwicklung, Zulassung, Wirksamkeit und Sicherheitsaspekten des neuen Anti-p40-Antikorpers Ustekinumab vor, der fur die Therapie des moderaten bis schweren Morbus Crohn zugelassen ist. Die Arbeit fasst die aktuelle Datenlage zusammen und enthalt somit sowohl die Daten der Zulassungsstudien als auch die Daten aus kleineren und auch fachubergreifenden Arbeiten, mit denen die Aspekte zu besonderen Situationen diskutiert werden konnen. Alle Arbeiten zusammen zeigen, dass mit Ustekinumab eine wirksame und sichere Option fur den Morbus Crohn zur Verfugung steht.

Patienten mit Morbus Crohn und Colitis ulcerosa leiden oftmals wahrend mehrerer Monate, bevor die Diagnose gestellt wird. Die Messung von fakalem Calprotectin hat das Potential, diese diagnostische Verzogerung zu verkurzen. Therapieziel sollte nicht nur die Beschwerdebesserung sein, sondern die mukosale Abheilung. Mehrere neue Medikamente haben in den letzten Jahren fur die Behandlung von Morbus Crohn und Colitis ulcerosa die Marktzulassung erhalten.

Das „severe acute respiratory syndrome coronavirus 2“ (SARS-CoV-2) verursacht die Coronaviruserkrankung (COVID-19), die aktuell pandemisch ist und sich mit einer infektiosen Pneumonie manifestiert. Neben den typischen Symptomen, wie Fieber, Husten und Dyspnoe, entwickeln einige Patienten auch gastrointestinale und hepatische Manifestationen, die sich meist als Diarrho, Nausea, Erbrechen und Abdominalschmerzen bemerkbar machen. Von hepatischer Seite konnen erhohte Leberenzyme beobachtet werden. SARS-CoV‑2 kann den Gastrointestinaltrakt via den Rezeptor „angiotensin converting enzyme 2“ infizieren, der auf den Enterozyten des Ileums und Kolons exprimiert wird. Virale RNA wurde im Stuhl vom COVID-19-Patienten isoliert, was die Moglichkeit offenlasst, dass SARS-CoV‑2 neben der Tropfcheninfektion auch mittels fakal-oraler Transmission ubertragen werden kann. Der Einfluss von SARS-CoV‑2 auf zugrunde liegende Erkrankungen des Gastrointestinaltrakts und des hepatobiliaren Systems wird aktuell ausfuhrlich untersucht. Dieser Artikel soll einen kurzen Uberblick verschaffen bezuglich gastroenterologischer und hepatologischer Manifestationen von COVID-19 sowie des Einflusses von COVID-19 auf zugrunde liegende, chronische gastroenterologische Erkrankungen.
