An adequate supply of saliva is critical to the preservation and maintenance of oral tissue. Clinicians often do not value the many benefits of saliva until quantities are decreased. Much is written on the subject of salivary hypofunction, but little attention is paid to normal salivary flow and function. This article is a brief, up-to-date overview of the literature on the basics of normal salivary composition, flow, and function. A review of the literature was conducted using MEDLINE and Healthstar (1944 through 1999); articles were selected for inclusion on the basis of relevance and significance to the clinician.

Ductal carcinoma in situ (DCIS) of the breast represents a heterogeneous group of neoplastic lesions in the breast ducts. The goal for management of DCIS is to prevent the development of invasive breast cancer. This manuscript focuses on the NCCN Guidelines Panel recommendations for the workup, primary treatment, risk reduction strategies, and surveillance specific to DCIS.

Clinical practice guidelines in oncology

Platelet-rich plasma (PRP) has been utilized in surgery for 2 decades; there has been a recent interest in the use of PRP for the treatment of sports-related injuries. PRP contains growth factors and bioactive proteins that influence the healing of tendon, ligament, muscle, and bone. This article examines the basic science of PRP, and it describes the current clinical applications in sports medicine. This study reviews and evaluates the human studies that have been published in the orthopaedic surgery and sports medicine literature. The use of PRP in amateur and professional sports is reviewed, and the regulation of PRP by antidoping agencies is discussed.

Background Abatacept, a selective co-stimulation modulator of T lymphocytes, could be a new treatment option for primary Sjogren’s syndrome (pSS). Objectives To assess short-term efficacy and safety of abatacept treatment in early pSS patients. Methods 15 pSS patients (12 female, 3 male) were included in the open-label ASAP (Active Sjogren Abatacept Pilot) study. All patients met the revised AECG criteria. Disease duration was Results Median age and disease duration were 43 years and 11 months. Significant improvements during treatment were found for ESSPRI and ESSDAI (Fig.1). Furthermore, RF levels decreased significantly from 43 kIU/l at baseline to 28 kIU/l at week 24, and IgG levels dropped from 20.2 g/l to 16.5 g/l. The study aimed to quantify the lubricating properties of chewing stimulated whole saliva from healthy controls (n = 22), from patients suffering from primary Sjogren’s syndrome (n = 37) and from patients undergoing head-and-neck radiotherapy (n = 34). All participants had to complete the Xerostomia Inventory questionnaire to score dry mouth sensation. Lubrication was measured using an ex vivo tongue-enamel friction system in terms of Relief and Relief period. MUC5b and total protein concentrations of the saliva samples were measured by an enzyme-linked immunosorbent assay and a bicinchoninic acid assay, respectively. Relief of Sjogren’s patients’ saliva and post-irradiation patients’ saliva was similar compared with healthy controls, but saliva from post-irradiation patients lubricated significantly better than saliva from Sjogren’s patients. The Relief period was similar between the three groups. The Relief and Relief period were higher for saliva samples post-irradiation compared to pre-irradiation. MUC5b and total protein concentrations were comparable in all groups. MUC5b and total protein output were significantly lower in patients subjected to radiotherapy compared to saliva from healthy controls and pre-irradiation patients. MUC5b concentrations positively correlated with lubricating properties of post-irradiation patient saliva. The lubricating properties of patient saliva were not any worse than healthy controls. Lower flow rate leads to lower availability of saliva in the oral cavity and decreases the overall output of protein and MUC5b, which might result in an insufficient replenishing of the mucosal salivary film. An insufficient replenishing might underlie the sensation of a dry mouth and loss of oral function.
