STRATEGIES FOR IMPROVING DIABETES CARE D iabetes is a chronic illness that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications. Diabetes care is complex and requires that many issues, beyond glycemic control, be addressed. A large body of evidence exists that supports a range of interventions to improve diabetes outcomes. These standards of care are intended to provide clinicians, patients, researchers, payors, and other interested individuals with the components of diabetes care, treatment goals, and tools to evaluate the quality of care. While individual preferences, comorbidities, and other patient factors may require modification of goals, targets that are desirable for most patients with diabetes are provided. These standards are not intended to preclude more extensive evaluation and management of the patient by other specialists as needed. For more detailed information, refer to Bode (Ed.): Medical Management of Type 1 Diabetes (1), Burant (Ed): Medical Management of Type 2 Diabetes (2), and Klingensmith (Ed): Intensive Diabetes Management (3). The recommendations included are diagnostic and therapeutic actions that are known or believed to favorably affect health outcomes of patients with diabetes. A grading system (Table 1), developed by the American Diabetes Association (ADA) and modeled after existing methods, was utilized to clarify and codify the evidence that forms the basis for the recommendations. The level of evidence that supports each recommendation is listed after each recommendation using the letters A, B, C, or E.

EASL–EORTC Clinical Practice Guidelines (CPG) on the management of hepatocellular carcinoma (HCC) define the use of surveillance, diagnosis, and therapeutic strategies recommended for patients with this type of cancer. This is the first European joint effort by the European Association for the Study of the Liver (EASL) and the European Organization for Research and Treatment of Cancer (EORTC) to provide common guidelines for the management of hepatocellular carcinoma. These guidelines update the recommendations reported by the EASL panel of experts in HCC published in 2001 [1]. Several clinical and scientific advances have occurred during the past decade and, thus, a modern version of the document is urgently needed. The purpose of this document is to assist physicians, patients, health-care providers, and health-policy makers from Europe and worldwide in the decision-making process according to evidencebased data. Users of these guidelines should be aware that the recommendations are intended to guide clinical practice in circumstances where all possible resources and therapies are available. Thus, they should adapt the recommendations to their local regulations and/or team capacities, infrastructure, and cost– benefit strategies. Finally, this document sets out some recommendations that should be instrumental in advancing the research and knowledge of this disease and ultimately contribute to improve patient care. The EASL–EORTC CPG on the management of hepatocellular carcinoma provide recommendations based on the level of evidence and the strength of the data (the classification of evidence is adapted from National Cancer Institute [2]) (Table 1A) and the strength of recommendations following previously reported systems (GRADE systems) (Table 1B).

In recent years, copyright violations from illegal copying and distribution of e-comic contents have become an important issue. Fingerprinting techniques have been emerged to provide a fast and reliable identification method of identifying e-comic contents. When illegally scanned or camera captured comic contents are distributed, they suffer from distortions. So the fingerprint differs from the original version. This paper presents a restoration framework for correcting geometric distortions in distorted comics to improve the comic content identification.

Epidemiologic and Etiological Features of Korean Patients With Behçet's Disease

The electricity distribution system in Korea is adopting a multi-grounding system. Protection of this distribution system against lightning is performed by installing overhead ground wires over the high voltage wires, and connecting the overhead ground wires to the ground every 200 m. The ground resistance in this system is limited not to exceed and overhead ground wire and neutral wire are multiple parallel lines. Although overhead ground wire and neutral wire are installed in different locations on the same pole, this circuit configuration has duplicated functions of providing a return path for unbalanced currents and protecting the distribution system against induced lightning. Therefore, the purpose of this study is to analyze the induced lightning shielding effect according to the neutral wire installation structure of a 22.9kV distribution line in order to present a new 22.9kV distribution line structure model and characteristics. This study calculated induced lightning voltage by performing numerical analysis when an overhead ground wire is present in the multi-grounding type 22.9kV distribution line structure, and calculated the induced lightning shielding effect based on this calculated induced lightning voltage. In addition, this study proposed and analyzed an improved distribution line model allowing the use of both overhead wire and neutral wire to be installed in the current distribution lines. The result of MATLAB simulation using the conditions applied by Yokoyama showed almost no difference between the induced lightning voltage developed in the current line and that developed in the proposed line. This signifies that shielding the induced lightning voltage through overhead wire makes no difference between current and proposed distribution line structures. That is, this study found that the ground resistance of the overhead wire had an effect on the induced lightning voltage, and that the induced lightning shielding effect of overhead wire is small.

Analysis on the Induced Lightning Shielding Effect According to the Neutral Wire Installation Structure of a 22.9kV Distribution Line

Dimerization of hypoxia-inducible factor-1 beta (HIF-1β) [aryl hydrocarbon receptor nuclear translocator (ARNT)] with HIF-1α is involved in various aspects of cancer biology, including proliferation and survival under hypoxic conditions. We investigated the in vitro mechanism by which silencing of HIF-1β leads to the suppression of tumor cell growth and cellular functions. Various hepatocellular carcinoma (HCC) cell lines (Huh-7, Hep3B, and HepG2) were transfected with small interfering RNA (siRNA) against HIF-1β (siHIF-1β) and cultured under hypoxic conditions (1% O2 for 24 h). The expression levels of HIF-1β, HIF-1α, and growth factors were examined by immunoblotting. Tumor growth was measured using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay, and tumor activity was measured by terminal deoxynucleotidyl transferase dUTP nick end labeling, tumor cell invasion, and migration assays. Under hypoxic conditions, silencing of HIF-1β expression suppressed tumor cell growth and regulated the expression of tumor growth-related factors, such as vascular endothelial growth factor, epidermal growth factor, and hepatocyte growth factor. Suppression of tumor cell invasion and migration was also demonstrated in HIF-1β-silenced HCC cell lines. Silencing of HIF-1β expression may induce anti-tumor effects under hypoxic conditions in HCC cell lines.

Various leaching tests were applied to the soil affected by accidental leakage of HF in an industrial area in Korea. Three different leaching methods including pH-stat, continuous batch leaching, and column tests were adopted to assess leaching characteristics and mobility of fluorine(F) in soil and the potential risks to ecosystem. Both natural and spiked samples were used for the leaching tests. F concentrations in the batch tests increased by leaching rapidly in the early stage of leaching and then maintained rather constant levels. Column leaching test also show similar result to that of the batch test. pH also controlled the leaching behavior of the soil. With increasing pH, more F was released in the pH-stat test. This is mainly due to the competition and exchange with hydroxyl ions, as pH increase to the alkaline range. Most of the F released by the accident seem to have removed in the very early stage of leaching, whereas some natural proportion from soil minerals are thought to have been released very slowly. Therefore, little F released during the accident remained, based on the results of this study on the samples after two years of the accident. We could conclude that soil contaminated by external effects such as chemical accidents should be managed immediately, especially with F.
