European Guidelines on Cardiovascular Disease Prevention in Clinical Practice (Version 2012)

Because of new evidence on several diagnostic and therapeutic aspects of hypertension, the present guidelines differ in many respects from the previous ones. Some of the most important differences are listed below: 
1. Epidemiological data on hypertension and BP control in Europe. 
2. Strengthening of the prognostic value of home blood pressure monitoring (HBPM) and of its role for diagnosis and management of hypertension, next to ambulatory blood pressure monitoring (ABPM). 
3. Update of the prognostic significance of night-time BP, white-coat hypertension and masked hypertension. 
4. Re-emphasis on integration of BP, cardiovascular (CV) risk factors, asymptomatic organ damage (OD) and clinical complications for total CV risk assessment.

To the Editor: In the January issue of the Journal, Wernicke et al.1 attempted to determine “the optimum (heart rate) correction factor (of the QT interval) for children and adolescents” by fitting a regression model to the pooled QT/RR data of the resting ECGs of 2,288 children. They found an “ideal” correction factor of 0.38 and proposed a formula QTc = QT/RR0.38 to replace Bazett and Frederica formulae in this age group. As we have pointed out previously,2-4 the range of reported “optimum” values of the exponential coefficient α in the generic formula QTc = QT/RR found in different patient groups is so wide (ranging from 0.255 to 0.604,6 including 0.31 derived by Yoshinaga et al.7 from the resting ECGs of 12,543 children and adolescents) that it should discourage any further attempt to find the “ideal” universally applicable correction factor. Because of the significant interindividual variability of the QT/RR relationship,8 any “optimum” correction coefficient determined from pooled QT/RR data of a particular subject/patient group is very likely (1) to lead to substantial undercorrection or overcorrection when applied to the QT/RR data of many individuals within the same group, and (2) to be far from the “optimum” derived from pooled QT/RR data of a different subject/patient group. In clinical practice, the QTc imprecision of about 20–30 msec due to the differences between different corrections is hardly of any practical concern since before clinical conclusions on prolonged QT interval are made, ECG recordings are repeated and excesses in their heart rate avoided. Thus, precise QTc correction is very important and needed only in clinical studies, such as investigations of drug-induced QT/QTc interval changes. In this field, it has been already shown beyond any reasonable doubt that the use of any “ad hoc” selected correction formula based on ECG data independent of the study in hand may (and frequently does) lead to unfounded conclusions.9 A study-specific correction formula derived from the pooled ECGs in baseline drug-free state of a particular patient group is relatively useful only when applied to the (on-drug) data of the same patient group. This is also clear, among others, from the study of Dmitrienko and Smith10 cited but not correctly interpreted by Wernicke et al. While offering substantial improvement over the use of some “universal” predetermined correction formulae, such study-specific formulae are still inferior to the individually derived, subject-specific corrections.4,11 In detailed studies of drug-induced QTc prolongation, individual assessment of QT/RR relationship and
