Barriers to Goals of Care Discussions With Seriously Ill Hospitalized Patients and Their Families: A Multicenter Survey of Clinicians
John J. You,James Downar,Robert A. Fowler,Francois Lamontagne,Irene W. Y. Ma,Dev Jayaraman,Jennifer Kryworuchko,Patricia H. Strachan,Roy Ilan,Aman P.K. Nijjar,John Neary,John Shik,Kevin Brazil,Amen Patel,Kim Wiebe,Martin Albert,Anita Palepu,Elysée Nouvet,Amanda Roze des Ordons,Nishan Sharma,Amane Abdul-Razzak,Xuran Jiang,Andrew G. Day,Daren K. Heyland,Daren K. Heyland +24 more
TL;DR: Hospital-based clinicians perceive family member-related and patient-related factors as the most important barriers to goals of care discussions, and this findings can inform the design of future interventions to improve communication and decision making about goals of Care.
Journal ArticleDOI
Patient-ventilator interaction during pressure support ventilation and neurally adjusted ventilatory assist.
Jadranka Spahija,Michel de Marchie,Martin Albert,Patrick Bellemare,Stéphane Delisle,Jennifer Beck,Christer Sinderby +6 more
TL;DR: Neurally adjusted ventilatory assist can improve patient-ventilator synchrony by reducing the triggering and cycling delays, especially at higher levels of assist, at the same time preserving breathing and maintaining blood gases.
Journal ArticleDOI
Recovery after critical illness in patients aged 80 years or older: a multi-center prospective observational cohort study.
Daren K. Heyland,Allan Garland,Sean M. Bagshaw,Deborah J. Cook,Kenneth Rockwood,Henry T. Stelfox,Peter Dodek,Robert A. Fowler,Alexis F. Turgeon,Karen E. A. Burns,John Muscedere,Jim Kutsogiannis,Martin Albert,Sangeeta Mehta,Xuran Jiang,Andrew G. Day +15 more
TL;DR: One-quarter of patients aged 80 years or older who are admitted to ICU survived and returned to baseline levels of physical function at 1 year and routine assessment of baseline physical function and frailty status could aid in prognostication and informed decision-making for very old critically ill patients.
Journal ArticleDOI
Thrombocytopenia in critically ill patients receiving thromboprophylaxis: frequency, risk factors, and outcomes.
David Williamson,Martin Albert,Diane Heels-Ansdell,Donald M. Arnold,Donald M. Arnold,François Lauzier,Ryan Zarychanski,Mark Crowther,Theodore E. Warkentin,Peter Dodek,John F. Cade,Olivier Lesur,Wendy Lim,Robert A. Fowler,Robert A. Fowler,Francois Lamontagne,Stephan Langevin,Andreas Freitag,John Muscedere,Jan O. Friedrich,Jan O. Friedrich,William Geerts,William Geerts,Lisa Burry,Jamal A. Alhashemi,Deborah J. Cook +25 more
TL;DR: A high severity of illness, prior surgery, use of inotropes or vasopressors, renal replacement therapy, and liver dysfunction are associated with a higher risk of thrombocytopenia developing in the ICU, whereas LMWHThromboprophylaxis isassociated with a lower risk.
Journal ArticleDOI
Prophylaxis against deep vein thrombosis in critically ill patients with severe renal insufficiency with the low-molecular-weight heparin dalteparin: an assessment of safety and pharmacodynamics: the DIRECT study.
James D. Douketis,Deborah J. Cook,Maureen O. Meade,Gordon H. Guyatt,William Geerts,Yoanna Skrobik,Martin Albert,John Granton,Paul C. Hébert,G. Pagliarello,John Marshall,Robert A. Fowler,Andreas Freitag,Christian Rabbat,David C. Anderson,Nicole Zytaruk,Diane Heels-Ansdell,Mark Crowther +17 more
TL;DR: In critically ill patients with severe renal insufficiency, DVT prophylaxis with dalteparin sodium, 5000 IU once daily, is not associated with an excessive anticoagulant effect due to drug bioaccumulation and is unlikely to contribute to bleeding.
