The importance of selenium to human health

The essential trace mineral, selenium, is of fundamental importance to human health. As a constituent of selenoproteins, selenium has structural and enzymic roles, in the latter context being best-known as an antioxidant and catalyst for the production of active thyroid hormone. Selenium is needed for the proper functioning of the immune system, and appears to be a key nutrient in counteracting the development of virulence and inhibiting HIV progression to AIDS. It is required for sperm motility and may reduce the risk of miscarriage. Deficiency has been linked to adverse mood states. Findings have been equivocal in linking selenium to cardiovascular disease risk although other conditions involving oxidative stress and inflammation have shown benefits of a higher selenium status. An elevated selenium intake may be associated with reduced cancer risk. Large clinical trials are now planned to confirm or refute this hypothesis. In the context of these health effects, low or diminishing selenium status in some parts of the world, notably in some European countries, is giving cause for concern.

The importance of selenium to human health.

Because depression and painful symptoms commonly occur together, we conducted a literature review to determine the prevalence of both conditions and the effects of comorbidity on diagnosis, clinical outcomes, and treatment. The prevalences of pain in depressed cohorts and depression in pain cohorts are higher than when these conditions are individually examined. The presence of pain negatively affects the recognition and treatment of depression. When pain is moderate to severe, impairs function, and/or is refractory to treatment, it is associated with more depressive symptoms and worse depression outcomes (eg, lower quality of life, decreased work function, and increased health care utilization). Similarly, depression in patients with pain is associated with more pain complaints and greater impairment. Depression and pain share biological pathways and neurotransmitters, which has implications for the treatment of both concurrently. A model that incorporates assessment and treatment of depression and pain simultaneously is necessary for improved outcomes.

Depression and pain comorbidity: a literature review.

We investigated cardiovascular autonomic functions in 23 patients with Alzheimer's disease (AD) and 23 age-matched control subjects. The vagal parasympathetic function was assessed with R-R interval variation (RRIV), and sympathetic function with sympathetic skin response (SSR) and orthostatic cardiovascular reflexes. Compared with controls, the AD patients had significantly depressed RRIV during rest (6.4 +/- 1.3% vs. 7.5 +/- 1.6%, p < 0.02) and deep breathing (12.6 +/- 4.6% vs. 17.0 +/- 5.1%, p < 0.006), but all AD patients had normal SSR. After standing for 3 minutes from supine, the changes of blood pressure and heart rate from baseline in AD patients did not differ from those in controls. The AD patients had a lower systolic blood pressure than controls in both supine (130.7 +/- 17.4 vs. 145.4 +/- 20.7 mmHg, p < 0.02) and standing (129.2 +/- 20.1 vs. 146.6 +/- 21.1 mmHg, p < 0.008) positions, especially in those patients with more severe dementia. We conclude that AD patients have mildly impaired autonomic functions, mainly in vagal parasympathetic functions. The pathogenesis and clinical significance of low blood pressure in AD needs further study.

Cardiovascular Autonomic Functions in Alzheimer's Disease

The mechanisms responsible for cyclosporin-induced encephalopathy remain controversial. Herein we present a case of cyclosporin-induced encephalopathy with unusually prolonged vasospasm, which might have contributed to the slow recovery of the patient.

Prolonged Reversible Vasospasm in Cyclosporin A-Induced Encephalopathy

Objective: To investigate the prevalence of headaches in a Chinese elderly population. Background: There are few headache surveys in the elderly. Previous studies have shown a low headache prevalence in Chinese. Methods: Target population: eligible registered residents ≥65 years old (N = 2,003) in two townships of Kinmen Island on August 1, 1993. All participants completed a headache questionnaire and underwent clinical evaluation and examination by a neurologist. Headache diagnoses were made according to the International Headache Society, 1988. Results: 1,533 persons (77%) participated in the study, of whom 584 (38%) had at least one episode of headache in the previous year. One-year prevalence of migraine was 3.0%, and tension-type headache, 35%. The prevalence of migraine, but not tension-type headaches, continued to decline with age in the elderly. Life-time prevalence of "incapacitating headache" was 10%, and that of migraine, 5.2%. Forty-two percent of migraineurs stopped having migraine before this survey. In comparison with "10 years ago," 8% participants felt their current headaches were worse, 25% better, and 67%, no change, with a net improvement of 17%. Conclusions: We have reported the highest headache prevalence among different Chinese elderly populations, but these were still lower than those reported from Western series. More than half of the elderly life-time migraineurs still had attacks of migraine. Severe headaches, including migraine but not tension-type headaches, declined with age.

Does Alzheimer’s Disease Protect against Cancers? A Nationwide Population-Based Study

We investigated autonomic function in patients with idiopathic Parkinson's disease (PD) by measuring sympathetic skin response (SSR) and R-R interval variation (RRIV). Sixty-two PD patients and 62 age-matched normal subjects were recruited. Abnormal SSR was noted in nine (14.5%) PD patients, including three in Stage II, three in Stage III, and three in Stage IV, but not in Stage I patients or normal subjects. Four of these nine patients had postural hypotension. Abnormal SSR was correlated with duration of illness and impotence. In PD patients, abnormal SSR may be due to intermediolateral column dysfunction. After logarithmic transformation and age adjustment, 19 (31.6%) of 60 PD patients had abnormal RRIV during rest and deep breathing. Abnormal RRIV was not related to staging or duration of illness. Patients with constipation had significantly lower RRIV, indicating parasympathetic dysfunction. RRIV was not affected by acute or chronic L-dopa treatment. The agreement between RRIV and SSR in PD patients was poor (kappa = -0.07). It appears that abnormal SSR, but not RRIV, may be associated with more autonomic disturbances in PD patients.
