Chronic obstructive pulmonary disease (COPD) remains a major public health problem. It is the fourth leading cause of chronic morbidity and mortality in the United States, and is projected to rank fifth in 2020 in burden of disease worldwide, according to a study published by the World Bank/World Health Organization. Yet, COPD remains relatively unknown or ignored by the public as well as public health and government officials. In 1998, in an effort to bring more attention to COPD, its management, and its prevention, a committed group of scientists encouraged the U.S. National Heart, Lung, and Blood Institute and the World Health Organization to form the Global Initiative for Chronic Obstructive Lung Disease (GOLD). Among the important objectives of GOLD are to increase awareness of COPD and to help the millions of people who suffer from this disease and die prematurely of it or its complications. The first step in the GOLD program was to prepare a consensus report, Global Strategy for the Diagnosis, Management, and Prevention of COPD, published in 2001. The present, newly revised document follows the same format as the original consensus report, but has been updated to reflect the many publications on COPD that have appeared. GOLD national leaders, a network of international experts, have initiated investigations of the causes and prevalence of COPD in their countries, and developed innovative approaches for the dissemination and implementation of COPD management guidelines. We appreciate the enormous amount of work the GOLD national leaders have done on behalf of their patients with COPD. Despite the achievements in the 5 years since the GOLD report was originally published, considerable additional work is ahead of us if we are to control this major public health problem. The GOLD initiative will continue to bring COPD to the attention of governments, public health officials, health care workers, and the general public, but a concerted effort by all involved in health care will be necessary.

Information about psychotropic drug prescribing in one general practice was compared with overall information about such prescribing in Australia. Over the period 1978-81 there was a significant reduction in the number of prescriptions issued for psychotropic drugs. Prescriptions for hypnotics, sedatives and minor tranquilizers were more common than those for antidepressants or major tranquilizers. The indications for prescribing were distributed equally among three groups of conditions: psychological disorders, sleep problems and physical diseases. Patients for whom psychotropic drugs were prescribed had a greater overall morbidity and used medical care services more than other patients. Only 9% of patients in 1981 were issued with a prescription for one of the four most commonly prescribed psychotropic drugs. A relatively small proportion of patients, most of them elderly women, received the largest proportion of the prescriptions.

The increasing availability of whole-genome sequence data is expected to increase the accuracy of genomic prediction. However, results from simulation studies and analysis of real data do not always show an increase in accuracy from sequence data compared to high-density (HD) single nucleotide polymorphism (SNP) chip genotypes. In addition, the sheer number of variants makes analysis of all variants and accurate estimation of all effects computationally challenging. Our objective was to find a strategy to approximate the analysis of whole-sequence data with a Bayesian variable selection model. Using a simulated dataset, we applied a Bayes R hybrid model to analyse whole-sequence data, test the effect of dropping a proportion of variants during the analysis, and test how the analysis can be split into separate analyses per chromosome to reduce the elapsed computing time. We also investigated the effect of imputation errors on prediction accuracy. Subsequently, we applied the approach to a dataset that contained imputed sequences and records for production and fertility traits for 38,492 Holstein, Jersey, Australian Red and crossbred bulls and cows.

Powder-bed based additive manufacturing techniques are of high interest for the medical sector and recent trial studies have shown their feasibility. Due to the rapid improvements made in the machinery and the related changes in the type and characteristics of the utilized power source, optimizations regarding the fabrication parameters tend to differ amongst various machines. In this study, a parameter optimization was undertaken for a biocompatible dental CoCrMo alloy on a SLM 280HL machine, featuring a 400 W fibre laser. It was shown that the availability of higher laser powers enables a more energy efficient fabrication. Moreover, parameter sets for fast and economic fabrication, as well as for high density and fine-grained microstructure, were defined.

Since 1988 a number of public figures in Australia have argued that prohibitions on the use of cannabis, cocaine, and heroin should be relaxed because current drug policies do not reflect the comparative risks of licit and illicit drug use, and many of the social costs of illicit drug use are consequences of prohibition. They have advocated a variety of proposals for reforming drug policy. Proponents of "controlled availability " have proposed that currently illegal drugs should be provided under medical supervision to persons who are dependent on them. Critics concerned about the social costs of drug policies have advocated policies to undercut the black market by increasing the availability of currently illicit drugs. The mechanisms proposed for achieving this end have included de facto and de jure decriminalization of drug use, and the provision of currently illicit drugs to licensed adults by a government monopoly. Despite a vigorous debate, drug policies have not changed because reformers have failed to convince the public that their policies will not increase the use of illicit drugs, and hence the prevalence of drug dependence and drug-related problems.
