Abstract: Fear of pain has been implicated in the development and maintenance of chronic pain behavior. Consistent with conceptualizations of anxiety as occurring within three response modes, this paper introduces an instrument to measure fear of pain across cognitive, overt behavioral, and physiological domains. The Pain Anxiety Symptoms Scale (PASS) was administered to 104 consecutive referrals to a multidisciplinary pain clinic. The alpha coefficients were 0.94 for the total scale and ranged from 0.81 to 0.89 for the subscales. Validity was supported by significant correlations with measures of anxiety and disability. Regression analyses controlling for measures of emotional distress and pain showed that the PASS made a significant and unique contribution to the prediction of disability and interference due to pain. Evidence presented here supports the potential utility of the PASS in the continued study of fear of pain and its contribution to the development and maintenance of pain behaviors. Factor analysis and behavioral validation studies are in progress.

Abstract: Tonic basal release of nitric oxide (NO) by vascular endothelial cells controls blood pressure (BP) in the basal state. In these studies we investigated the effects of chronic inhibition of basal NO synthesis in the rat for a 2-mo period. Significant systemic hypertension developed in chronically NO-blocked rats compared to controls. Marked renal vasoconstriction was also observed with elevations in glomerular blood pressure (PGC) and reductions in the glomerular capillary ultrafiltration coefficient (Kf). Chronically NO-blocked rats also develop proteinuria and glomerular sclerotic injury compared to controls. These studies therefore describe a new model of systemic hypertension with glomerular capillary hypertension and renal disease due to chronic blockade of endogenous NO synthesis. These observations highlight the importance of the endogenous NO system in control of normal vascular tone and suggest that hypertensive states may result from relative NO deficiency.

Abstract: We report an attempt to quantitate the relative contributions of the history, physical examination, and laboratory investigation in making medical diagnoses. In this prospective study of 80 medical outpatients with new or previously undiagnosed conditions, internists were asked to list their differential diagnoses and to estimate their confidence in each diagnostic possibility after the history, after the physical examination, and after the laboratory investigation. In 61 patients (76%), the history led to the final diagnosis. The physical examination led to the diagnosis in 10 patients (12%), and the laboratory investigation led to the diagnosis in 9 patients (11%). The internists' confidence in the correct diagnosis increased from 7.1 on a scale of 1 to 10 after the history to 8.2 after the physical examination and 9.3 after the laboratory investigation. These data support the concept that most diagnoses are made from the medical history. The results of physical examination and the laboratory investigation led to fewer diagnoses, but they were instrumental in excluding certain diagnostic possibilities and in increasing the physicians' confidence in their diagnoses.

Abstract: Mutations in the gene coding for the p53 tumor suppressor protein are common in a variety of human cancers. To assess the role of a putative mutated p53 protein in human lung cancer, a monoclonal antibody recognizing it was used in an immunoperoxidase detection system. A total of 114 cases of Stage I and II adenocarcinomas and squamous cell carcinomas were studied. The staining pattern was always intranuclear and heterogeneous. When the median or mean survival time was compared between cases, p53 accumulation had a statistically significant negative prognostic value. This was supported by a Kaplan-Meier survival plot of p53 producers and nonproducers. In 7 of 24 Stage II cases that were negative for p53 in the primary tumor, metastatic regional lymph nodes were p53-positive. These latter cases had greatly reduced survival times. Thus, p53 accumulation in primary tumors (and regional lymph nodes) may identify a subgroup of lung cancer patients with a prognosis of more aggressive disease.

Abstract: Recently, several authors have questioned the potential biasing effects of social desirability response sets on the findings reported in organizational behaviour research. This paper reports the results of two studies designed to examine this issue. In the first study, a meta-analytic review of 33 studies that have examined the relationships between social desirability response sets and organizational behaviour constructs was conducted. The results of this study indicated that social desirability, as traditionally measured in the literature, is significantly (although moderately) correlated with several widely used constructs in organizational behaviour research. The second study was designed to see if the meta-analytic results would be replicated using a new measure of socially desirable responding. The results from this study indicated that even though a number of the measures commonly used in organizational behaviour research are significantly correlated with Paulhus' (1989) impression management dimension, the majority of these correlations are generally small to moderate in size, and controlling for socially desirable responding had little impact on the nature of the relationships reported. The importance of these findings for those using self-report questionnaires is then discussed.

Abstract: Membrane and cytoskeletal structures are known targets of oxidative injury. Brains from patients with Alzheimer's disease have cytoskeletal abnormalities and platelet and possible neuronal membrane lesions. The authors have recently demonstrated that superoxide anion is a powerful inducer of heat-shock protein synthesis, and have also shown that in response to oxidative stress or hyperthermia, intracellular levels of antioxidant enzymes increase to several folds. Whether the aforementioned mechanisms play a role in Alzheimer's disease has been suggested but is not totally established. While exploring this possibility, tissue sections from five brains with Alzheimer's disease and five neuropathologically normal age-matched controls were immunostained with polyclonal antibodies against superoxide dismutase (CuZn- and Mn- forms) and catalase. A standard avidin-biotin-peroxidase method was used for antigen detection. A subgroup of neurofibrillary tangles (15-25%) and senile plaques (50%) showed immunoreactivity for both enzymes with a staining pattern similar (but not identical) to that usually observed with antibodies against ubiquitin. Senile plaques displayed a granular pattern of immunostaining. Amyloid cores in mature classical plaques remained unstained. In addition, occasional elements with features consistent with reactive glial cells were strongly immunostained. Tangle-free neurons in both diseased and control brains showed weak to absent intracytoplasmic immunoreactivity. The immunoreactivity was totally abolished by preincubation of the primary antibodies with the corresponding purified antigens. These findings support the hypothesis that oxidative stress may be involved in the pathogenesis of Alzheimer's disease.

Abstract: This study coded and categorized 2404 motivators and demotivators freely listed by 308 college students prior to and following their being prompted to consider teacher behaviors as contributing to motivation level. Twenty categories of motivators and 20 categories of demotivators emerged; four of each reflected context factors, six of each structure/format factors, and ten of each teacher behavior factors. While teacher behaviors accounted for approximately 44% of both motivators and demotivators, negative teacher behaviors were perceived as more central to students' demotivation (i.e., were listed without prompting) than positive teacher behaviors were perceived as central to motivation. Structure/format factors were more frequently mentioned as demotivators and context factors, such as desire to know the material, grade or credit motivation, and personal desire for accomplishment, as motivators. Abstract: The unprecedented rate of depletion of the stratospheric ozone layer will likely lead to appreciable increases in the amount of ultraviolet-B radiation (UV-B, 280-320 nm) reaching the earth's surface. In plants, photosynthetic reactions and nucleic acids in the mesophyll of leaves are deleteriously affected by UV-B. We used a fiber-optic microprobe to make direct measurements of the amount of UV-B reaching these potential targets in the mesophyll of intact foliage. A comparison of foliage from a diverse group of Rocky Mountain plants enabled us to assess whether the foliage of some plant life forms appeared more effective at screening UV-B radiation. The leaf epidermis of herbaceous dicots was particularly ineffective at attenuating UV-B; epidermal transmittance ranged from 18-41% and UV-B reached 40-145 μm into the mesophyll or photosynthetic tissue. In contrast to herbaceous dicots, the epidermis of 1-year old conifer needles attenuated essentially all incident UV-B and virtually none of this radiation reached the mesophyll. Although the epidermal layer was appreciably thinner in older needles (7 y) at high elevations (Krumholtz), essentially all incident UV-B was attenuated by the epidermis in these needles. The same epidermal screening effectiveness was observed after removal of epicuticular waxes with chloroform. Leaves of woody dicots and grasses appeared intermediate between herbaceous dicots and conifers in their UV-B screening abilities with 3-12% of the incident UV-B reaching the mesophyll. These large differences in UV-B screening effectiveness suggest that certain plant life forms may be more predisposed than others to meet the challenge of higher UV-B levels resulting from stratospheric ozone depletion.

Abstract: Nasal delivery is a potential alternative for systemic availability of drugs restricted to intravenous administration, such as peptide and protein drugs. Although nasal delivery avoids the hepatic first-pass effect, the enzymatic barrier of the nasal mucosa creates a pseudo-first-pass effect. The xenobiotic metabolic activity in the nasal epithelium has been investigated in several species including humans. The Phase I, cytochrome P-450 enzymes have been studied extensively for their toxicological significance, since these enzymes metabolize inhaled pollutants into reactive metabolites which may induce nasal tumors. The cytochrome P-450 activity in the olfactory region of the nasal epithelium is higher even than in the liver, mainly because of a three- to fourfold higher NADPH-cytochrome P-450 reductase content. Phase II activity has also been found in the nasal epithelium. The delivery of peptides and proteins has been hindered by the peptidase and protease activity in the nasal mucosa. The predominant enzyme appears to be aminopeptidase among other exopeptidases and endopeptidases. The absorption of peptide drugs can be improved by using aminoboronic acid derivatives, amastatin, and other enzyme inhibitors as absorption enhancers. It is possible that some of the surfactants, e.g., bile salts, increase absorption by inhibiting the proteolytic enzymes. Thus, in addition to the permeation barriers, there also exists an enzymatic barrier to nasal drug delivery, which is created by metabolic enzymes in the nasal epithelium.

Abstract: The transport barrier of the epithelial presents one of the major problems limiting the effective use of these tissues as alternate delivery routes for macromolecules such as peptides and proteins. In the present study, two membrane transport properties, namely, the permeability and permselectivity of the shunt pathway, were investigated and compared in various tissues including the nasal, tracheal, bronchial, buccal, rectal, vaginal, corneal, epidermal, duodenal, jejunal, ileal, and colonic epithelia. Membrane permeability was evaluated using a combined method based on electrical conductance and flux measurements of a hydrophilic fluorescent probe, 6-carboxy fluorescein (CF). Membrane permselectivity or the charge discriminating ability of the membrane was evaluated by KCl diffusion potential measurements. The results indicate that all epithelia under investigation possess a relatively high degree of permeation barrier and are highly selective for the absorption of positively charged solutes. Shunt path permeability was found to vary greatly among tissues from different epithelia, whereas membrane charge selectivity was relatively constant in these tissues. A good correlation was observed between membrane electrical conductance and steady-state flux of CF, indicating a paracellular transport of the compound. The rank order of the intrinsic membrane permeability was as follows: intestinal approximately nasal greater than or equal to bronchial greater than or equal to tracheal greater than vaginal greater than or equal to rectal greater than corneal greater than buccal greater than skin. Membrane permselectivity, expressed as the ratio of transport number (positive over negative), ranges from 1.78 for the buccal to 1.33 for the rectal epithelium.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract: The application of Rasch modeling to survey responses and official records of 139 Hispanic and 300 African-American males in the sixth through eighth grades at four Chicago inner-city schools is used to construct incremental measures of gang involvement and delinquency. Scale sequence and regression analysis suggest that different social processes operate in gang involvement for the two ethnic populations. In both sets of cross-sectional data, the fitting of linear structural models shows gang involvement to be an effective post hoc estimator of delinquency for these youth, whereas delinquency is not an effective estimator of gang involvement.

It appears that cognitions, rather than hedonics, drive the observed olfaction effects.

Abstract: The depth of penetration of Ultraviolet-B (UV-B, 300 and 320 nm) and visible (680 nm) light was measured in foliage of Abies lasiocarpa and Picea engelmannii using a fibre-optic microprobe. Measurements were made on foliage at four times during development: needles were sampled from within expanding buds (in bud); within 72 h of emergence from the bud scales (emergent); from elongating branches (elongating); and from foliage that emerged the previous summer (mature). Light attenuation in pre-emergent needles of both species was steep and showed strong wavelength dependence. Short wavelength 300-nm light was attenuated strongly in the developing epidermal layer, but a significant proportion of this potentially damaging UV-B radiation penetrated into the mesophyll. For A. lasiocarpa and P. engelmannii, 99% attenuation of 300-nm light occurred at 51 and 96 μm, respectively, well within the mesophyll. At this stage, however, the bud scales were opaque to light below 400nm. As the epidermal cell walls and cuticle continued to develop and chlorophyll accumulated following emergence from the bud scales, light attenuation, particularly of UV-B radiation, increased. Although no UV-B is transmitted through the epidermis-hypodermis of mature needles, small but measurable quantities of 300- and 320-nm light were measured in the photosynthetic mesophyll of post-emergent and elongating needles. Thus, shortly after emergence from the bud scales in mid-June to mid-July, when incident UV doses are highest, absorption of UV-B radiation by potentially sensitive chromophores in the mesophyll may disrupt physiological and developmental processes in these species. Soluble UV-absorbing pigments accumulated during needle maturation for P. engelmannii but not A. lasiocarpa, suggesting that, for A. lasiocarpa at least, the development of effective UV screening properties in the epidermis may not be related to the induction of soluble flavonoids.

Abstract: Four hundred consecutive hip fractures were studied prospectively. Two hundred forty-seven patients were classified as unhealthy (poor cardiac status, pneumonia, cancer history, bowel obstruction history, malnutrition, dehydration, stroke history, renal failure history, cirrhosis). Twenty-two percent of this unhealthy group died, while only 6% of the remaining healthy group died. Death rates varied with admission activity level and mental status but not when patient health status was factored out. After factoring out health status, age was associated with higher death rates only in patients older than age 85. Confusion, a change of mental status in the hospital, occurred in 25% of patients. Confusion was associated with a medical complication in 94% of cases, was the presenting symptom of a medical complication in 79% of cases, and was associated with a 39% death rate. Major medical complications occurred in 9% of the healthy group (29% of them died) and 21% of the unhealthy group (64% of them died). Major medical complications in unhealthy, shut-in patients were associated with an 80% death rate. Vigorous urinary tract monitoring and early treatment of bacteriuria decreased death rate. Postfracture malnutrition was associated with higher complication rates. Hip surgery performed within 72 hours on patients with acute medical illnesses in addition to their fracture was associated with a higher death rate. Whether a patient walked postfracture seemed not to be correlated with the death rate. Patients who were not walking prefracture but treated by internal fixation had a 34% failure rate.

Abstract: Pigeons were exposed to two different reinforcement schedules under different stimulus conditions in each of two daily sessions separated by 6 hr (Experiments 1 and 2) or in a single session (Experiment 3). Following this, either a fixed-interval (Experiment 1) or a variable-interval schedule (Experiments 2 and 3) was effected in both stimulus conditions. In the first two experiments, exposure to fixed-ratio or differential-reinforcement-of-low-rate schedules led to response-rate, but not pattern, differences in subsequent performance on fixed- or variable-interval schedules that persisted for up to 60 sessions. The effects of reinforcement-schedule history on fixed-interval schedule performance generally were more persistent. In Experiment 3, a history of high and low response rates in different components of a multiple schedule resulted in subsequent response-rate differences under identical variable-interval schedules. Higher response rates initially occurred in the component previously correlated with high response rates. For 3 of 4 subjects, the differences persisted for 20 or more sessions. Previous demonstrations of behavioral history effects have been confined largely to between-subject comparisons. By contrast, the present results demonstrate strong behavioral effects of schedule histories under stimulus control within individual subjects.

Abstract: Despite concerns about the restrictiveness of care and treatment procedures and settings for special populations, the concept of restrictiveness has seldom been defined or measured. This study describes the development of the Restrictiveness of Living Environments Scale (ROLES) for measuring the restrictiveness of residential settings for children and youths, including their own homes. Based on a literature review, the authors defined restrictiveness in terms of limits placed on freedom of movement or choice by the physical facility, by rules or requirements, and by conditions of entry and departure. The definition, along with a list of 27 residential settings for children, was sent to 389 child care professionals, who were asked to rate the restrictiveness of the settings on a 10-point scale, using a rating method designed to yield an interval scale. Ratings from 159 respondents, rounded to the nearest 5, were used to determine the relative position of the settings on the scale.

Abstract: Theophylline is metabolized in the liver by one or more cytochrome P-450 enzymes. To assess the amounts and types of these human cytochromes P-450, we incubated theophylline with microsomes prepared from 22 different human livers in the presence of NADPH, and measured simultaneous rates of 1- and 3-N-demethylations to 3-methylxanthine (3-MX) and 1-methylxanthine (1-MX), respectively; and 8-hydroxylation to 1,3-dimethyluric acid (1,3-DMU). Under optimal conditions, 3-MX, 1-MX, and 1,3-DMU formation proceeded with mean Km values of 2.05, 1.93, and 5.34 mM and Vmax values of 2.28, 2.48, and 23.4 pmol/mg/min, respectively. Formation of 3-MX and 1-MX correlated best with amounts of the immunoreactive protein HLd (P-450IA2) (p less than 0.05), whereas formation of 1,3-DMU correlated with the microsomal content of HLp (P-450IIIA3) and HLj (P-450IIE1). In immunoinhibition experiments, incubations conducted with a polyclonal anti-rat P-450c/d antibody, the formation of all the three theophylline metabolites (p less than 0.05) was significantly inhibited. However, addition of isoform-specific anti-rat-P-450d antibodies to the microsomal mixture significantly inhibited 1-N-demethylation, selectively, with little (if any) inhibition of 3-N-demethylation or 8-hydroxylation. Nonspecific cytochrome P-450 inhibition was ruled out by showing that erythromycin N-demethylation, an activity catalyzed by HLp, was unaffected by either anti-P-450c/d (P-450IA1/IA2) or anti-P-450d. Anti-rat-P-450p antibodies failed to block formation of theophylline metabolism, but did inhibit erythromycin N-demethylase.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract: A questionnaire asking respondents to list adjectives describing four hypothetical stutterers (a typical 8-year-old female, 8-year-old male, adult female, and adult male stutterer) was completed by 103 elementary and secondary teachers. The majority of reported adjectives were negative stereotypical personality traits, indicating perceptions of stutterers similar toother groups, including speech-language pathologists. Implications of these findings and suggestions for pre-service and continuing education programs for teachers are discussed.

Abstract: Trichloroethylene (TCE) oxidation was examined in 9 different methanotrophs grown under conditions favoring expression of the membrane associated methane monooxygenase. Depending on the strain, TCE oxidation rates varied from 1 to 677 pmol/min/mg cell protein. Levels of TCE in the reaction mixture were reduced to below 40 nmolar in some strains. Cells incubated in the presence of acetylene, a selective methane monooxygenase inhibitor, did not oxidize TCE.

Abstract: One in five eighth‐graders is at serious risk of failing in school or dropping out before graduating from high school. While other characteristics have been identified, few descriptions of the attitudes of academically at‐risk students toward communication have been provided. This study examined the attitudes toward communication of 2,793 academically at‐risk students at fourteen urban, large, predominantly minority middle or junior high schools from throughout the U.S. Primary attention was devoted to students' fear of communication and self‐perception as competent communicators. Compared to national norms, academically at‐risk students were found to be more apprehensive of communication and lower in self‐perceived communication competence. Several pedagogical strategies for responding to academically at‐risk students in the classroom are suggested.
